We observed that depression is a common psychiatric disorder and has no significant effect on proinflammatory cytokine levels in HD patients; no important improvement in cytokine levels was observed after antidepressant therapy.

The depressive disorder in PD patients is a common psychopathology and has no significant effects on nutritional status and inflammation.

Amlodipine/valsartan therapy plus hydration did not reduce the risk of CIN in chronic kidney disease (CKD) Stage 2 patients who underwent elective CAG using a low-osmolar nonionic contrast medium. This is because there was a decrease in the glomerular filtration rate (GFR) using the Levey Modification of Diet in Renal Disease (MDRD) formula in the amlodipine/valsartan group and CIN occurred at a higher frequency in this group; ARBs and CCBs may be withheld before CAG in high-risk patients.

Objective: Tunneled cuffed dual-lumen catheters (TCCs) are commonly used for vascular access in hemodialysis (HD) patients. Catheter-related bloodstream infection (CRBSI) is the major problem leading to morbidity and mortality. We investigated whether 26% NaCl solution has any favorable effect on the infections and thrombosis caused by HD catheters. Methods: TCCs were locked with either 26% NaCl and heparin or standard heparin. The primer end point of the study was the CRBSI or thrombosis of the TCC. We compared the antimicrobial activity of the NaCl solutions (6.5%, 13%, 26%) with 0.9% NaCl solution by time-kill kinetic assay. All tests were performed in triplicate by incubation of test fluids with Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus, and Staphylococcus epidermidis. Results: The mean catheter survival was significantly higher in the 26% NaCl and heparin group (129.5 ± 50.1 catheter days to 103.3 ± 59.8, p = 0.008). CRBSI rates (10-15.4%) did not differ significantly between the two groups (p = 0.54). The hypertonic 13% NaCl solution had bactericidal effects on E. coli and P. aeruginosa, but had bacteriostatic effect on S. aureus and S. epidermidis. Conclusion: In this study we demonstrated that the 13% NaCl solution and more hypertonic NaCl solutions revealed potent in vitro antimicrobial properties against all checked Gram-negative microorganisms.

Although brucellosis is an endemic disease in Mediterranean countries, there are few reported cases of Brucella endocarditis and glomerulonephritis. We report a case of Brucella prosthetic mitral valve endocarditis and glomerulonephritis that could be managed by using only antibiotic therapy, including rifampicin, doxycycline and ceftriaxone, without a surgical procedure.

Horner syndrome is a rare complication which may occur following the insertion of a catheter in to the internal jugular vein. We present two cases of Horner syndrome (HS) which occured following internal jugular venous cannulation

OBJECTIVE: Our main purpose was to evaluate the epidemiological properties and mortality rate of this study and to evaluate the etiology and factors associated with mortality in patients with ARF. MATERIAL and METHODS:In this prospective observational study a total of 541 patients with ARF were assessed between January 2008 -January 2012. The etiological spectrum and predictors of mortality were evaluated in this population. RESULTS:The mean age was 64.9 ± 15.6 years and the majority of patients were male. The most common etiology of ARF was medical causes. Diarrheal disease and drug-induced ARF were the major medical causes. One hundred and fi fty four patients needed dialysis. The most common cause of death was cardiopulmonary failure. In multivariate analysis, dialysis necessity, age, C-reactive protein, serum sodium, serum phosphorus, serum albumin, and hemoglobin were independently associated with death after adjusting for other variables.CONCLUSION: ARF resulting from medical diseases was higher compared to surgical causes. Use of medication with antibiotics and non-steroid anti-infl ammatory drugs was predominantly increased; however the main cause was diarrheal disease. Hypoalbuminemia, anemia, dialysis requirement and hyperphosphatemia were found to be the main predictors of mortality in patients with ARF. Bu yüzden sebepleri ve ölümle ilişkili öngörüye katkıda bulunan belirteçler ayrıntılı bir şekilde araştırılmıştır. Bu çalışmamızda, ABY nedenlerinin yıllar içerisindeki değişiklikleri ile ölüme katkıda bulunan nedenleri ortaya koymayı amaçladık. BULGULAR: Çalışmamızdaki hastaların önemli bir kısmı erkek ve ortalama yaş 64.9 ± 15.6 yıl olarak bulunmuştur. En sık ABY nedenleri arasında oral alım bozukluğu-ishal ile ilaç kaynaklı nedenler göze çarpmıştır. Hastalarımızın 154'ü takip esnasında diyaliz gereksinimi doğmuştur. En sık ölüm nedeni ise kalp kaynaklı ölümler ilk sırayı almaktadır. Renal biyopsi yapılan hastalar içinde patolojik tanı açısından en fazla akut interstisyel nefrit tanısı konmuştur. Çok değişkenli analizde, diyaliz gereksinimi, yaş, C-reaktif protein, serum fosfor, albumin ve hemoglobin seviyeleri sağkalım ile bağımsız şekilde ilişkili bulunmuştur. GEREÇ ve YÖNTEMLER: SONUÇ:Daha önceki merkez verilerimiz gözönünde bulundurulduğunda, nonsteroid anti infl amatuvar ilaç ve antibiyotik kaynaklı ABY olgularında artış saptanırken, sağkalımla ilişkili parametreler benzer şekilde hipoalbuminemi, anemi, diyaliz gereksinmi ve hiperfosfatemi olarak bulunmuştur.

Son dönem böbrek yetmezliği nedeniyle 5 yıldır sürekli ayaktan periton diyalizi yapan elli dört yaşında kadın hasta Candida albicans'ın neden olduğu peritonit sebebiyle yatırıldı. Hastanın yapılan ultrasonografisinde; batında cilt altı dokuda loküle mayi saptandı. Kültürde E. coli üremesi olması üzerine sefoperazon-sulbaktam tedavisi başlandı. Tedavisinin 9. gününde burun kanaması olan hastanın protrombin zamanı 61.1 sn, aktive parsiyel tromboplastin zamanı 159,2 sn, INR 5,55 olarak saptandı. Hastanın bilinen karaciğer hastalığı, tıkanma sarılığı veya K vitamini emilimini bozacak hastalığı yoktu. Hepatit belirteçleri negatif ve karaciğer fonksiyon testleri normaldi. Koagülasyon testlerini bozacak varfarin, klasik heparin kullanmıyordu. Koagülasyon bozukluğu sefoperazon-sulbaktam tedavisine bağlandı ve tedavi kesildi. Tedavilerle kanaması durdu ve koagülasyon testleri normal aralıklara döndü. Ateşi devam eden hastaya, ampirik olarak meropenem ve vankomisin başlandı. Aspirasyon pnömonisi sonucu solunum yetmezliği gelişen hasta mekanik ventilatöre bağlandı. Yoğun bakım ünitesindeki takipleri sırasında aspirasyon pnömonisi nedeniyle hasta kaybedildi. Sefoperazonsulbaktam, vitamin K metabolizmasını etkileyerek koagülopatiye neden olması sebebiyle klinisyenler bu ilacı kullanırken dikkatli olmalıdırlar.
