Machina & Schmeidler (1992) show that probabilistic sophistication can be obtained in a Savage setting without imposing expected utility by dropping Savage's axiom P2 (sure-thing principle) and strengthening his axiom P4 (weak comparative probability). Their stronger axiom, however, embodies a degree of separability analogous to P2. In this note, we obtain probabilistic sophistication using Savage's original axiom P4 and a weaker analog of Savage's P2.
Bu araştırma, Sağlık hizmetleri meslek yüksekokulu öğrencilerinin sağlık algısı ile sağlıklı yaşam biçimi davranışlarının belirlenmesi amacıyla yapılmıştır. Tanımlayıcı tipteki bu çalışmanın evrenini Mart-Haziran 2019 tarih aralığında Gölhisar Sağlık Hizmetleri Meslek Yüksekokulunda eğitim ve öğretime devam eden tüm öğrenciler (890 öğrenci), örneklemi ise çalışmaya katılım sağlayan 600 öğrenci oluşturmuştur. Veriler, araştırmacılar tarafından geliştirilen “Kişisel Bilgi Formu”, “Sağlık Algısı Ölçeği (SAÖ)” ve “Sağlıklı Yaşam Biçimi Davranışları Ölçeği (SYBDÖ)” ile elde edilmiştir. Veriler; tanımlayıcı istatistikler, Mann-Whitney U testi, Kruskal-Wallis testi ve Spearman korelasyon analizi kullanılarak analiz edilmiştir. Çalışmada yer alan öğrencilerin %88.5’i 18-21 yaş aralığında, %70.8’i kadın ve %39.5’i tıbbi sekreterlik ve dokümantasyon bölümünde okumaktaydı. Öğrencilerin SAÖ toplam puan ortalaması 50.61±6.61 ve SYBDÖ toplam puan ortalaması ise 123.05±19.93 idi. Öğrencilerin okuduğu bölüm, sınıf ve genel sağlık durumu değişkenleriyle SAÖ ve SYBDÖ arasında istatistiksel olarak anlamlı fark tespit edilmiştir. Öğrencilerin SAÖ toplam puanı ile SYBDÖ toplam puanı arasında ise pozitif yönde zayıf bir ilişki belirlenmiştir (p=0.000, r=0.196). Meslek yüksekokulu öğrencilerinin sağlık algısı arttıkça sağlıklı yaşam biçimi davranışlarının da arttığı belirlenmiş olup sağlık algısı ve sağlıklı yaşam biçimi davranışlarını arttırmak için ders müfredatlarına seçmeli derslerin eklenmesi, kurs ve seminerlerin düzenlenmesi önerilebilir.
ronik böbrek yetmezliği, çeşitli faktörler nedeni ile böbrek fonksiyonlarının kronik, ilerleyici ve geri dönüşümsüz kaybı ile karakterizedir. 1 Böbrek hasarının başlangıcından itibaren tüm hastalık evrelerini tanımlamak için, "The National Kidney Foundation Kidney Disease Outcomes Initiative", (NKF-K/DOQI) (2006) kronik böbrek hastalığı ifadesinin kullanılmasını önermektedir ve böbrek hastalığının nedenine bakılmaksızın en az üç ay süren objektif böbrek hasarı ve/veya glomerüler filt-Hemodiyaliz Hastalarında Vasküler Erişim Yolları ve Hemşirelik Bakımı Ö ÖZ ZE ET T Kronik böbrek hastalığının beşinci evresinde, hayatın idamesi için diyaliz tedavisine ya da böbrek transplantasyonuna ihtiyaç duyulmaktadır. Bu amaçla sıklıkla hemodiyaliz (HD) tercih edilmektedir. Etkin HD sağlamak için büyük bir vasküler yol gerekmektedir; fistül, greft ve kateter tercih edilen yöntemler arasında yer almaktadır. Diğer vasküler yollara göre uzun süre kullanılabilir olması, daha az komplikasyon gelişmesi, maliyetinin az olması gibi avantajlara sahip olan fistülün kullanım oranının %65'in üzerinde tutulması önerilmektedir. Diyalizin her aşamasında vasküler yolun fizik muayenesinin ve bakımının yapılması, HD cihazında izlenen parametrelerde, laboratuvar ve radyolojik incelemelerde farklılıkların saptanması komplikasyonların erken tanılanmasıyla birlikte revizyonuna imkân vermektedir. Bu da ancak multidisipliner bir ekiple sağlamaktadır. Hemodialysis (HD) is the most preferred method. To apply HD you have to have a big vessel way as fistula, graft and central catheter. As cheap, useable, efective and less complications advantages of fistula, percentage use should be more tane 65%. Maintenance and physical examination of the fistula shoud be always in mind while HD is on. The physical examination and care of the vascular access should apply at all stage of dialysis. Laboratory values, palpation, inspection, auscultation and radiological examinations are important parameters for evaluating the vascular access. Changes in these parameters help to early diagnosis for problems that can revision vascular access. A multidisciplinary approach requires to establish the vascular access, efficient and long-term use of this way. In the multidisciplinary team, HD nurses/dialysis technician have more specific roles and higher professional autonomies to diagnose early complications. Acute pulmonary edema in pregnant women with pre-existing cardiac disease is an uncommon problem, which may lead to significant maternal morbidity and mortality. It may occur due to superimposed characteristic features of the physiological changes related to pregnancy. The management is difficult, because there is no controlled trials or guidelines. The authors present their management of combined spinal epidural anesthesia with non-invasive ventilation in a parturient with acute pulmonary edema, due to pre-existing multivalvular heart disease and pulmonary hypertension, who underwent an emergency cesarean section because of preterm labour and fetal distress.
