Among adolescents from low socioeconomic background, the presence of fast-food retailers near schools is associated with accumulation of irregular eating habits and greater overweight. These findings suggest that obesogenic school neighbourhoods may contribute to social inequalities in overweight.
Universal school health services are expected to offer similar, needs‐based services to all students across schools, service providers and students’ socio‐economic statuses and health needs. This study investigates access to school health nurses in Finland. The objectives were to study the differences in access to school health nurse between service providers, schools, students’ characteristics and school health nurse resources. Access was examined through a nationwide School Health Promotion study, which is a self‐reporting, voluntary and anonymous survey for 8th and 9th graders (15 to 16‐year old, N = 71865). The ethical committee of the National Institute for Health and Welfare has approved procedure for the School Health Promotion study. Data on school health nurse resources and service providers were obtained from the national database (534 schools; 144 service providers). Multilevel logistic regression was used. Of the pupils, 15% of girls and 11% of boys reported difficult access to a school health nurse. The number of adolescents who reported difficult access ranged between service providers (0%–41%) and schools (0%–75%). Students with lower socio‐economic background, poorer well‐being at school, lack of support for studying and greater health needs reported difficult access more often. School health nurse resources were associated with difficult access only among boys, when resources were under the national recommendations. These findings raise concern about equality and unmet health needs in school health services.
It is important for healthcare professionals working with adolescents to be aware of prevalence in thinness and aware of health behaviour and family factors that are associated with thinness and extreme thinness in adolescence.
Kouluterveydenhuollon tulee olla suunnitelmallista, laadukasta ja tasoltaan yhtenäistä. Vuonna 2009 annettu asetus velvoittaa kuntia tekemään yhtenäisen toimintaohjelman lasten ja nuorten ehkäiseville terveyspalveluille. Tutkimuksessa arvioidaan kouluterveydenhuollon suunnitelmallisuutta ja yhtenäistä tasoa toimintaohjelmien avulla. Vertailukelpoista arviointia varten kehitimme toimintaohjelmamittarin. Mittari ja arvioinnin tulos kuvataan artikkelissa. Aluehallintovirastotkeräsivät toimintaohjelmat alueensa terveyskeskuksista 2011–2012 (N= 136, 87%). Arviointiin kehitettiin mittari, joka koostuu 38:sta säädöksiin perustuvasta indikaattorista. Nämä ryhmiteltiin seitsemään ulottuvuuteen (sitoutuminen, johtaminen, yhteistyö, voimavarat, laajat terveystarkastukset,muut terveystarkastukset ja terveysneuvonta, erityinen tuki). Mittarin maksimipisteet olivat 76. Toimintaohjelmien kouluterveydenhuoltoa koskevat osat arvioitiin mittarin avulla. Toimintaohjelmien kokonaispisteet vaihtelivat välillä 0–70 (0–92 % maksimista). Keskiarvo oli 39.6 (52 %). Seitsemästä ulottuvuudesta matalimmat pisteet sai voimavarat (2,7/8 pistettä) ja korkeimmat muut terveystarkastukset ja terveysneuvonta (7,3/12 pistettä). Suurten terveyskeskusten toimintaohjelmat saivat korkeammat pisteet (46,5) kuin pienempien (29,5). Ensimmäiset asetuksen mukaiset toimintaohjelmat olivat vaihtelevia ja niiden suunnittelussa todettiin puutteita. Kehitetyn mittarin avulla kouluterveydenhuoltoa voidaan arvioida vertailukelpoisesti yli koko maan. Nyt raportoidut tulokset toimivat perustasona kouluterveydenhuollon seurannassa. Mittaria voidaan soveltaa myös muille alueille ja hyödyntää kouluterveydenhuollon kehittämisessä sosiaali- ja terveydenhuollon uudistuksen yhteydessä.
