Endoclip migration into the common bile duct following laparoscopic cholecystectomy (LC) is an extremely rare complication. Migrated endoclip into the common bile duct can cause obstruction, serve as a nidus for stone formation, and cause cholangitis. We report a case of obstructive jaundice and acute biliary pancreatitis due to choledocholithiasis caused by a migrated endoclip 6 mo after LC. The patient underwent early endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic sphincterotomy and stone extraction.
In 1998 we implemented a BCSH recommendation that addressograph labels should not be used on blood transfusion specimen tubes. Over a 12-month period before the ban was introduced our laboratory received 5964 red cell transfusion requests, 182 (3.1%) of which contained an error in the identification details (ID) supplied on the request form and/or specimen. Three of these errors were of the 'wrong patient' type, i.e. the sample belonged to a different patient from the one whose ID appeared on the specimen tube and request form. Over the 12 months after the ban was introduced 511 (8. 1%) of 6326 requests contained a labelling error, an increase in error rate of 165%; no wrong-patient errors were identified, however. In a survey, seven (29.2%) of 24 transfusion laboratories in the UK accepted specimens labelled with addressograph stickers; in four of these cases a local blood transfusion committee had agreed that the BCSH guideline should not be followed. We believe the BCSH guideline is valid; its implementation, however, has major financial and workload implications, which probably explains why many hospitals apparently do not comply with it.
Indonesia’s demand for natural silk is quite high for each year, reaching around 200 tons of raw silk threads and 250 tons of spun silk threads, but domestic production of raw silk threads has only reached 110 tons. This shows that Natural Silk has great potential to be developed, especially in South Sulawesi. This business is generally home industry which is relatively easy to work with, simple technology, labor-intensive, fast production, and high economic value. The research was conducted in Wajo Regency in 6 sub-districts of natural silk development. The time for collecting data and information to be used in this study was planned for March - October 2019. The type of data used is primary data and data (collected through field observations followed by direct interviews with 25 entrepreneurs using a questionnaire. The data collected is then analyzed. using the quantitative descriptive analysis to confirm the analysis. Increasing the production of mulberry plants in Wajo is needed for the long term not only as a food ingredient for silkworms but also as a biopharma plant or a mixture of cosmetic ingredients. However, improvements are needed in the quality of cocoon products as well as more advanced technological inputs and capacity building for farmers and their institutions.
The measurement of company performance usually uses the analysis of Return on Investment (Rol), Return in Earning (RoE), Business Rentability (Ru) and Economic Rentability (Re) to assess the level of profit against capital, which means the extent to which business activities have a positive impact on capital owners, but it has not yet described the impact on human resources. The solution offered is the measurement of the company’s performance using Value Added Rentabilty (Rnt), so that the impact on the welfare of business actors can be known. Data was collected from all tofu agro-industry business units in North Lombok Regency. Data collection is done by using a survey method based on a list of questions. The survey was conducted 2 (two) times before and after the earthquake, namely in June 2018 and June 2019 for 24 respondents of business owners. The results showed that Value Added Rentabilty (Rnt) before earthquake in 2018 was 216.82% and post-earthquake in 2019 was 316.81%, meaning that the income obtained by capital owners from profits and labor wages was greater after the earthquake than before the earthquake for one IDR use of capital.
