In two patients intrahepatic cholestasis and cholestatic liver disease probably due to N-propyl ajmaline were observed. In both cases there was a four-week-interval between the first ingestion of the drug and the onset of jaundice. In one case with rigors there was a temporary rise in temperature and cholestasis. In the other there was a two to three week prodromal period of pruritus and gastro-intestinal symptoms. Both cases were characterised by blood and pronounced tissue eosinophilia in the periportal zones, a marked increase of cholestatic and hepatic cell enzymes, serum bilirubin and cholesterol, a moderate increase in erythrocyte sedimentation rate and absence of leucocytosis. Pathological laboratory findings returned to normal spontaneously within 5 weeks in one case and within two weeks under steroid therapy in the other.
Eine Fettleber kommt in der westlichen Welt bei 20-30% der Erwachsenen vor. Etwa 10% von ihnen entwickeln eine nichtalkoholische Steatohepatitis (NASH) (24). Die Diagnose ist in Betracht zu ziehen, wenn bei einer Fettleber (Sonographie) mit erhöhten Transaminasen andere Ursachen wie chronische Hepatitiden unwahrscheinlich sind. Wichtig ist eine glaubhaft fehlende übermäßige Alkoholexposition. Histologisch ist die NASH von einer alkoholischen Steatohepatitis (ASH) nicht sicher unterscheidbar. In der Mehrzahl der Fälle verläuft sie klinisch und histologisch benigne; sie kann jedoch über eine Fibrosierung zur Zirrhose fortschreiten (Abb.1). Prognostisch ungünstig sind Adipositas, Diabetes mellitus, Hyperlipidämie, rascher Gewichtsverlust, hohe Transaminasen, der histologische Nachweis von Mallory-Körperchen und wahrscheinlich eine erhöhte Eisenbeladung der Leber. In Spätstadien kann sich ein hepatozelluläres Karzinom (HCC) entwickeln. Nach dem "two hit"-Modell entsteht die NASH auf dem Boden einer Leberverfettung ("first hit") durch Einwirkung zusätzlicher Faktoren, die eine Zellschädigung und eine Entzündung auslösen ("second hit"). Pathogenetisch spielen reaktive Sauerstoffspezies (reactive oxigen species, ROS) und eine mitochondriale Dysfunktion eine zentrale Rolle (24). kurzgefasst: Die NASH muss bei Patienten ohne Alkoholabusus mit Fettleber und erhöhten Transaminasen in die engere Differentialdiagnose einbezogen werden.
