RESUMEN: Se ha propuesto que una de las causas de fracaso del tratamiento de canales es el paso de bacterias a través del foramen apical, las que por acción directa o por activación del sistema inmune generan una respuesta inflamatoria. El sellado apical con conos de gutapercha y cemento sellador es lo que impide el paso de estas bacterias, pero en presencia de humedad sufre de solubilidad y permite la filtración bacteriana. El objetivo de este estudio fue comparar la filtración bacteriana apical que se produce después de la obturación radicular con cementos Topseal® y Tubliseal®, para lo cual se realizó un estudio de tipo experimental en que se seleccionaron 26 dientes unirradiculares recientemente extraídos, que se instrumentaron con limas Protaper Universal® hasta F3, finalizando con un ajuste apical con lima K#45, siendo irrigados permanente y abundantemente con hipoclorito de sodio al 5%. Los dientes se dividieron aleatoriamente en dos grupos que se obturaron por compactación lateral y cemento Topseal®, G1 (n=12), y cemento Tubliseal®, G2 (n=12). Las superficies externas se sellaron con esmalte para uñas, excepto los 2 mm apicales y se montaron en un dispositivo para filtración bacteriana de doble cámara, dejando sumergidos los ápices en un caldo agar bilis esculina con Enterococcus faecalis. Se incubaron a 37ºC por 10 semanas y se determinó la filtración bacteriana mediante la aparición de turbidez en el medio en la cámara con caldo estéril. Se determinaron los días transcurridos para verificar el crecimiento bacteriano y la cantidad de especímenes positivos, valores que se registraron y analizaron mediante la prueba estadística de X 2. Las muestras obturadas con Topseal® presentaron mayor filtración bacteriana (66,64%) que las obturadas con Tubliseal® (58,31%), sin diferencias significativas (X 2 = 0,16), mostrando ambos altos niveles de filtración apical. cónico (TCHC) ha sido utilizada en la indicación de implantes y cirugía maxilofacial. En endodoncia, ya ha comenzado su uso, por la posibilidad de conocer las tres dimensiones del diente. Este estudio busca determinar la frecuencia de las diferentes condiciones patológicas más prevalentes en endodoncia, mediante el uso de TCHC, para lo cual se identificaron y cuantificaron hallazgos radiográficos de connotación endodóntica en 128 TCHC en pacientes chilenos entre 7 y 78 años, de ambos sexos, evaluando un total de 1.213 dientes. Las imágenes fueron obtenidas por un equipo SkyView 3D TCHC Panoramic Imager® 12 bits (MyRay, Imola, Italia), con una exposición de 90 Kvp, 10 mA y observadas mediante el software Skyview viewer manager ver 1.2.0.6 MyRay por 2 observadores previamente calibrados. Se observaron los dientes de manera independiente, en los ejes sagital, coronal y axial con un intervalo de corte de 0,5 mm. Los datos se introdujeron en la versión 17.0 del paquete estadístico SPSS® para Windows (SPSS Inc., Chicago, IL) para su análisis. Los hallazgos de connotación endodóntica fueron: lesión apical, terapia endodóntica realizada, taurodontismo, canales accesorios, dilaceraciones, ápices abiertos, reabsorción interna, reabsorción externa apical y cervical. Se encontró asociación significativa entre la presencia de lesiones apicales y dientes endodónticamente tratados (p= 0,00).
Introducción. Una de las propiedades del hipoclorito de sodio (NaOCl) es la disolución de tejido pulpar remanente después de la instrumentación. Los sistemas de activación del irrigante pretenden mejorar esta propiedad. Este trabajo tiene como objetivo determinar la capacidad de disolución de tejido de NaOCl con y sin activación sónica o ultrasónica en diferentes concentraciones. Metodología. 300 muestras de tejido conectivo de paladar de cerdo de 4,5 * 2 mm obtenidos 1 día antes del estudio, congelado a-15°C en 100% de humedad, fueron pesados en una balanza analítica y sometidos a la acción de NaOCl 1%, 3% y 5% con y sin activación durante 45 minutos, cambiando la solución cada 10 minutos. Se secaron en papel filtro y se pesaron nuevamente. Los datos se analizaron mediante tests Kolmogorov-Smirnov; Kruskal-Wallis y Mann-Whitney. Resultados y Discusión. NaOCl 1% tiene menor capacidad de disolución que mejora levemente al activarlo. NaOCl 3% tiene mejor capacidad de disolución que NaOCl 1%, pero la activación no la mejora significativamente. NaOCl 5% tiene mayor capacidad de disolución, que aumenta con la activación, sin significancia entre activación sónica y ultrasónica, lo que se debería a que la mucosa palatina porcina requiere más tiempo para su disolución completa, y a que el volumen de tejido utilizado es mayor que una pulpa dental. Conclusiones. Bajo las condiciones de este estudio, sólo NaOCl 5% mostró mayor capacidad de disolución con la activación. NaOCl 1% y 3% no mejoraron significativamente con la activación.
