Sleeve resection of the trachea and reconstruction by end-to-end anastomosis is seen as the method of choice in the treatment of short and long stenoses of the trachea following long-term artificial respiration and tumors. The experience with tracheal resection in 73 patients operated upon at 2 different thoracic units is analyzed. Postintubation was the main indication for operation in 63% of the patient population. The 30-day postoperative mortality rate was 8% with a total mortality of 19%. A satisfactory functional result was attained in 73% of the patients operated upon including those with malignant tumors. Restenoses occurred in 6 of the 73 patients who underwent surgery. Different approaches to tracheal resection were chosen in the 2 clinics and the possibility of mobilizing the larynx was used in a different way. A preference for resorbable material was noted regardless of the operative technique and has prevented restenosis. The diagnostic procedures considered mandatory, the indications for surgery and the various operative techniques are discussed.
EinleitungLiposarkome gehören mit einer Inzidenz zwischen 6 und 18 % unter den Sarkomen zu den häufigsten mesenchymalen Malignomen im Erwachsenenalter [7]. Dennoch ist die primäre Lokalisation im Mediastinum eine Rarität [25]; bis jetzt wurden weniger als 100 Fälle in der Literatur mitgeteilt [2,6,11,16,23]. In 8 years we performed 360 plastic procedures on the bronchial tree and the pulmonary vessels: 159 patients underwent plastic resection of the pulmonary vessels and 72 patients, a concomitant sleeve resection of the bronchus. Some of the patients who underwent plastic resection of both bronchial tree and pulmonary vessels died ofa bronchovascular fistula. When hemoptysis occurs postoperatively after this type of resection, we therefore suggest performing the secondary pneumonectomy immediately. In 4 patients with malignant tracheal tumors and severe hypoxia we used the Neville prosthesis for airway reconstruction. There were two early deaths, one from erosion of the innominate artery. In the remaining 2 patients, good palliation was achieved.Zusammenfassung. 360 plastische Operationen am Bronchial-und Lungengef'~6baum wurden in 8 Jahren vorgenommen. 159 Eingriffe an den GeffiBen haupts~ichlich als Tangential-oder Keilresektionen, und davon waren 72 kombiniert mit Bronchusanastomosen. Bei kombinierter Bronchus-und Gefii6plastik traten im Einzelfall letale Arrosionsblutungen durch Mikroabscedierung auf, ausgehend vonder Bronchusanastomose. Deswegen sollte bei postoperativen Hfimoptysen aus vitaler Indikation sofort die sekund~ire Pneumonektomie durchgeffihrt werden. In 4 malignen ausgedehnten Trachealstenosen bei drohendem Ersticken, wurde notfallm~i6ig eine Kunststoffprothese nach Neville eingesetzt. In Übereinstimmung mit internationalen Statistiken können 25 % aller Bronchial-Karzinom-Kranken zum Zeitpunkt der Diagnosestellung reseziert werden. Die 5-Jahres-Überlebensquote wird einheitlich mit 25 % angegeben. Als prognostische Faktoren werden TNM-Stadium, Zelltyp und biologisches Verhalten des Tumors, Immunlage sowie Lo-kalisation des Primärtumors und der befallenen Lymphknoten ge-nannt. Die angeführten Übersichtsstatistiken zeigen, daß Platten-epithel-Karzinome im günstigen Falle (TiNo) die beste 5-Jahres-Überlebensquote mit 54 % haben. Bei kleinzelligen Karzinomen werden extrem unterschiedliche Überlebenszeiten zwischen 0 und 20 % nach operativer Therapie genannt. Das operative Vorgehen weist eindeutig eine Zunahme der Lobektomien gegenüber den Pneumonektomien aus. Die postoperative Sterblichkeit bei der sogenannten erweiterten Pneumonektomie wird unterschiedlich zwischen 20 und 38 % genannt. Bei einfachen Pneumonektomien liegt sie zwischen 7–10 %, bei Lobektomien unter 4 %, teilweise sogar unter 2%. Entscheidend für den Verlauf ist die präoperative Risikoabgrenzung, wie Spirometrie, Bodyplethysmo-graphie, Rechtsherzkatheter, Perfusions- und Ventilationszintigramm. Bei plastischen Operationen am Gefäß- und Bronchialbaum werden 5-Jahres-Überlebensquoten unterschiedlich mit 36 % bzw. 9,8 % angegeben. Segment- oder Keilresektionen weisen eine 5-Jahres-Über-lebensquote von 56%, eine 15-Jahres-Überlebensquote von 26,8 % aus. 46 Pancoasttumoren wurden präoperativ bestrahlt. 12 (35 %) waren nach 5 Jahren noch beschwerdefrei. Primär palliative Operationen finden ihre Indikation in der Beseitigung von Beschwerden und Verbesserung der Lebensqualität. Die postoperative Betreuung des Bronchial-Karzinom-Kranken umfaßt unter anderem die zeitlich vorbestimmte Röntgenkontrolle sowie zusätzliche Untersuchungen nach onkologisch internistischen Gesichtspunkten.
