73. Tracheaersatz am Menschen, kombinierte Eingriffe an Bronchien und Gef��en der Lunge

Abstract

In 8 years we performed 360 plastic procedures on the bronchial tree and the pulmonary vessels: 159 patients underwent plastic resection of the pulmonary vessels and 72 patients, a concomitant sleeve resection of the bronchus. Some of the patients who underwent plastic resection of both bronchial tree and pulmonary vessels died ofa bronchovascular fistula. When hemoptysis occurs postoperatively after this type of resection, we therefore suggest performing the secondary pneumonectomy immediately. In 4 patients with malignant tracheal tumors and severe hypoxia we used the Neville prosthesis for airway reconstruction. There were two early deaths, one from erosion of the innominate artery. In the remaining 2 patients, good palliation was achieved.Zusammenfassung. 360 plastische Operationen am Bronchial-und Lungengef'~6baum wurden in 8 Jahren vorgenommen. 159 Eingriffe an den GeffiBen haupts~ichlich als Tangential-oder Keilresektionen, und davon waren 72 kombiniert mit Bronchusanastomosen. Bei kombinierter Bronchus-und Gefii6plastik traten im Einzelfall letale Arrosionsblutungen durch Mikroabscedierung auf, ausgehend vonder Bronchusanastomose. Deswegen sollte bei postoperativen Hfimoptysen aus vitaler Indikation sofort die sekund~ire Pneumonektomie durchgeffihrt werden. In 4 malignen ausgedehnten Trachealstenosen bei drohendem Ersticken, wurde notfallm~i6ig eine Kunststoffprothese nach Neville eingesetzt. Uber eigene Erfahrungen mit dieser Tracheal-und Bifurkationsprothese wird berichtet. Schliisselwiirter: Trachealprothese -Trachealstenose -Bronchialcarcinom -Plastische Resektionsverfahren.
