In 25 patients with untreated essential hypertension and 25 healthy controls, erythrocyte intracellular Cl- concentration and activity as well as Na+ activity were measured. Intracellular Cl- concentration in essential hypertensive patients was 70.6 +/- 11.3 as compared with 84.4 +/- 9.5 mmol/l in the controls (P less than 0.001). Intracellular Cl- activity was 77.5 +/- 13.0 mmol/l of cell water in hypertensive patients, the control value being 100.8 +/- 11.0 mmol/l of cell water (P less than 0.001). In the hypertensive group intracellular Na+ activity was 14.3 +/- 4.1 as compared with 7.1 +/- 1.8 mmol/l of cell water in the normotensive group (P less than 0.01). From these results it is suggested that in essential hypertension not only disturbances of cation metabolism, but also of anion metabolism, occur. Possibly the Cl- changes reflect a decreased Cl- inward transport due to an altered Na-K cotransport.
ZusammenfassungFragestellung Wird die Häufigkeit von Bronchitis bei Kindern unter nächtlicher Belastung mit Straßenverkehrslärm erhöht? Welche Bedeutung haben dabei lärmbedingte Erhöhungen von Cortisol in der ersten Nachthälfte? Methodik Aus den Teilnehmern einer Beobachtungsstudie wurden 68 Kinder mit bzw. ohne Bronchitisbefund sowie mit niedriger bzw. hoher Verkehrslärmbelastung ausgewählt. Die Verkehrslärm-und Stickstoffdioxid-Belastung wurde vor repräsentativen Kinderzimmerfenstern gemessen. Auf der Grundlage dieser Messwerte wurden allen Kindern Schallpegelund Abgasklassen sowie kombinierte Immissionsklassen zugeordnet. Die Cortisolausscheidung der Kinder wurde in der ersten und zweiten Nachthälfte bestimmt. Außerdem wurden ihre Arztkontakte wegen Bronchitis retrospektiv für bis zu 5 Jahren aus den Karteien ermittelt. Die statistische Auswertung umfasste auch multiple Regressionsanalysen unter Berücksichtigung wesentlicher Co-Variablen. Ergebnisse Nächtliche Belastung mit Straßenverkehrslärm erhöht die Ausscheidung von Cortisol in der ersten Nachthälfte. Kinder, die ohne Lärmbelastung häufig Bronchitis gehabt hatten, zeigten keine Cortisolerhöhungen. Die Arztkontakte wegen Bronchitis stiegen bei Kombinationsbelastung mit Lärm und Abgasen dosisabhängig und signifikant an. Eine erhöhte Cortisolausscheidung in der ersten Nachthälfte war mit einer signifikanten Zunahme der Arztkontakte wegen Bronchitis korreliert. Schlussfolgerungen Die Häufigkeit von Bronchitis wird bei Kindern durch kombinierte Belastung mit Kfz-Lärm und Abgasen stärker erhöht als durch Abgase allein. Straßenverke-hrsbelastungen sind prinzipiell eine Kombination von Lärm und Abgasen.
