Fragestellung: Ist die Informationslagc einer therapeutischen Kohortenstudie ausreichend, die Kriterien Wirksamkeit und Wirtschaftlichkeit zweier Therapie -schemata zu beurteilen? Studiendesign: In einer offenen prospektiven Studie wurden Patienten unter naturheilkundlicher und konventioneller Therapie beobachtet. Patienten: Eingeschlossen wurden 102 Patienten mil akuten loka-len, nicht pseudoradikulÄr ausstrahlenden RÜckenschmerzen auf dem Boden eines chronischen RÜckenschmerzleidens, die bei 2 von 5 Schmerzparametern (RÜckenschmerzen im Sitzen, Stehen. Liegen, bei Bewegung. wÄhrend der Nacht) eine SchmerzintensitÄt > 5 auf einer visuellen Analogskala 0–10 an-gaben. Interventionen: Die Patienten erhielten von naturheilkundlich orientierten Ärzten eine Therapie mil Harpagophytum-Extrakt 1800 mg mit 30 mg Harpagosid pro Tag als Mono- oder Co-Therapie (Gruppe J) oder von kon-ventionell therapierenden Ärzten eine konventionelle Therapie (Gruppe K). Hauptzielvariable: Als Messparameter diente der validierte Arhuser RÜckenschmerzindex, der vor Beginn der Therapie und nach 4 und 6 Wochen erhoben wurde, sowie eine mit der AOK/Heidelberg festgelegte relative Kostenberechnung. Die Erhebung der Untersuchung und die Dokumentation der Daten bei den niedergelassenen Ärzten erfolgte durch die Autoren, die nicht in die Beliandlung der niedergelassenen Ärzte eingriffen. Ergebnisse: Die Patientenkollektive (Gruppe J, n = 51; Gruppe K, n = 51) waren hinsichtlich der Ausgangswerte nicht ausgewogen verteilt, doch war die Gruppe J nicht bevorteilt (lÄngere Dauer der chronischen RÜckenschmerzen, multimorbider, hÖherer InvaliditÄtsindex). Die Dauer der akuten RÜckenschmerzen und der Arhuser RÜckenschmerzindex waren jedoch in beiden Gruppen verglcichbar. Die Therapie in Gruppe K bestand vor allem aus oral verabreichten nichtsteroidalen Antirheumatika (p < 0,001), Krankengymnastik (p < 0,001) und paravertebralen Injektionen mit gewebehÄrtenden Mitteln (p < 0,001). Die Anzahl schmerzfreier Patienten nach 4 bzw. 6 Wochen Therapie war in beiden Gruppen etwa gleich (Gruppe J: n = 16 bzw. 20; Gruppe K: n = 12 bzw. 23) (n.s.). Unter der 6wÖchigen Therapie besserte sich der Arhuser RÜckenschmerzindex in beiden Gruppen um mehr als 20%. Die relative Änderung der Einzelkomponenten Schmerz, InvaliditÄt und BcwegungseinschrÄnkung unterschied sich zwischen den Gruppen nicht. Jedoch nahm im zeitlichen Verlauf bei beiden Gruppen der Schmerzindex von der 4. zur 6. Woche ab (Gruppen J und K, p < 0,05), in Gruppe K besserten sich auch der InvaliditÄtsindex (p < 0,001) und der Arhuser RÜckenschmerz-Gesamtindex (p < 0,02). Bei selektiver Betrachtung der Subgruppe, die nur mit dem Harpagophytum-Extrakt behandelt wurde, war der Therapieerfolg bei entsprechender Ausgangssituation gleich. Unter der Harpagophytum-Therapie traten 5 leichte unerwÜnschte Ereignisse auf, die nicht zur Beendigung der Extrakteinnahme fÜhrten.
