Die Ausstellung soll die Problematik der Behandlung infizierter Frakturen veransehaulichen. Grundlegend fiir die Demonstration ist die These, dal~ die Heilung einer infizierten Fraktur ganz wesentlich vom Stabilit/~tsgrad der Fraktur abh/~ngt. Sowohl bei einer fehlerhaften instabilen Osteosynthese als aueh bei einer konservativen Frakturbehandlung, selbst wenn diese korrekt durehgeffihrt wird, sind zumindest minimale Bewegungen im Frakturbereich nicht vermeidbar. Hierdureh wird nicht nur die Frakturheilung gehemmt, sondern auch die Infektion unterhalten bzw. versehlimmert; der fortbestehende Infekt hemmt wiederum die Knochenheilung. Dieser Kreislauf negativer Weehselwirkungen zwisehen Kuochenheflung und Infektion, der anhand eines Schemas dargestent wird, fiihrt meist zu dem Endzustand einer infizierten Pseudarthrose. Anhand yon rSntgenologischen Verlaufskontrollen naeh fehlerhaften Nagelungen und Verplattungen wird gezeigt, dal3 die Instabflit/~t Frakturheflung und Infektion immer negativ beeinfluBt. Dem wird ein Schema gegeniibergestellt, aus dem hervorgeht, dab dureh die korrekte Osteosynthese ein g/instigerer Heflverlauf erzielt werden kann. Durch die absolute Stabilit/~t der Fraktur wird der oben skizzierte Cireulus vitiosns durchbroehen. Die vSllige Ruhigstellung, d.h. auch die Anssehaltung yon Mil~robewegungen im Frakturbereieh, fSrdert die Knoehenheflung und hemmt dureh den Fortfall der st/indigen Bewegungsreize die Infektion. Trotz fortbestehenden Infektes kann die Fraktur naeh korrekt durehgeffihrter Osteosynthese lest werden. Naeh Ausheflen der Fraktur, sp/~testens nach Implantatentfernung, klingt in den meisten F/~nen aueh die Osteomyelitis ab. Die Verwendung yon autologer Spongiosa-Plastik unterstfitzt dabei den ProzeB der Frakturheilung. Die absolute Stabilit~t einer Fraktur kann nieht auf konservativem, sondern aussehlieBlieh auf operativem Wege erzielt werden. Die hierzu gezeigten Beispiele aus unserem Krankengut illnstrieren die Mittel und Wege der gegenw/irtig an der Bonner Klinik durchgef/ihrten Verfahren zur Stabilisierung infizierter Frakturen: Verschraubung einer re-frakturierten, infizierten Arthrodese des Sehultergelenkes, Verschraubung und Versorgung mit Abstiitzplatte bei infizierter Tibiakopf-Trfimmerfraktur, Kompressionsplatte bei infizierter Tibiasehaft-Fraktur, Versorgung yon frischen und ~lteren Tibia-Frakturen mit /£ul~eren Spannern. Dem letzteren Verfahren wird besondere Aufmerksamkeit gewidmet, da die
Chest radiographs can reflect increasing pulmonary insufficiency of shock lung. In the early and intermediate stages, there is almost pure interstitial oedema which, in the later stages becomes alveolar-interstitial. In addition one may find evidence of broncho-pneumonia. Respiratory insufficiency is due to abnormalities of perfusion and distribution and is aggravated by disturbance of oxygen diffusion. The latter is due to an increase in the alveolo-capillary diffusion distance in the presence of interstitial oedema. It was not possible to demonstrate quantitatively significant precapillary shunts greater than 25 mu.
