La estenosis pilorica hipertrofica (EPH) es una entidad clinica reconocida como tal desde que Hirschprung, a fines del siglo pasado (1888) publica sus dos primeros casos debidamente estudiados, encontrandose en la literatura antecedentes poco claros que datan de principios del siglo XVII. (2) Desde un comienzo el cuadro fue tratado quirurgicamente, utilizando distintas tecnicas, pero con resultados muy pobres; hasta que Ramstedt y Fredet (en trabajos por separado) en 1912, conciben la te*cnica hoy en uso. Los resultados estadisticos, despue"s de mas de medio siglo de utilizaci6n de esta tecnica han mejorado ostensiblemente, ayudado por una mejor preparaci6n preoperatoria del enfermo, anestesia y conceptos de asepsia y antisepsia que se tienen hoy dia. Sin embargo, los fracasos iniciales de la cirugia obligaron en algunos centres pediatricos a tratar meMicamente a estos pacientes, logrando buenos resultados pero a costa de una larga hospitalizaci(5n y recursos terapeuticos engorrosos. (11) La EPH constituye en el nino menor de seis meses el cuadro quirurgico abdominal mas frecuente. (3) La EPH "es una enfermedad que nacio caminando": despues de los conceptos de Hirschprung, en la parte clmica, y de Ramstedt y Fredet en la parte quirurgica, practicamente nada ha cambiado en 70 anos: la unica diferencia esta en los nuevos conceptos que se tienen acerca de la etiologfa de la enfermedad. ETIOLOGIAHechos clinicos y estadisticos senalan que la EPH es una enfermedad de caracter genetico, en la cual intervienen ademas factores ambientales (7,26,5). Esto es lo que los genetistas Hainan multifactorial. Se hereda como autosomico dominante, no ligado al sexo y que requiere un umbral minimo de carga genetica (umbral que seria mas bajo para el hombre), bajo el cuat la enfermedad no se manifiesta. Esto viene a confirmar e! hecho de que para la descendencia de pacientes afectos de EPH es mayor la probabilidad de EPH en hijos de pacientes femeninos que en los hijos de pacientes masculines: ® Mujeres enfermas tuvieron: 20% de nhios enfermos y 7% ninas enfermas.Hombres enfermos tuvieron: 5% enfermos y 2,5% ninas enfermas.ninos En la descendencia de pacientes con EPH la incidencia global es de 6,9%.La incidencia en la poblacion general es variable, segun zona geografica y raza. Es ampliamente mas frecuente en Estados Unidos, Tnglaterra y Suecia con 1/200 nacidos vivos hombres y 1/1 .000 nacidas vivas mujeres, o de otro modo 0,5% y 0,1%, respectivamente, con una cifra global Rev.
La constipacion cronica es una patologia frecuente en la edad pediatrica y puede derivarse de numerosas causas (1). Suele ser dificil establecer el diagnostico etiologico, lo que conduce en ocasiones a conductas terapeuticas erradas, pudiendo practicarse un acto quirurgico innecesario o por el contrario, retardarse la solucion quirurgica indispensable (2). El tratamiento medico tambien suele ser mal Ilevado, ya sea porque no se consideran los multiples factores que influencian la constipacion o bien porque este tratamiento es mantenido por un periodo demasiado breve, sin que se logre restablecer la funci6n intestinal normal.Estos hechos nos motivaron a realizar un estudio retrospectivo de los pacientes que consultaron por constipacion cronica en las policlinicas de gastroenterologia y cirugia del Hospital Roberto del Rio, en los ultimos 6 anos. En este estudio se tomaron en cuenta solamente aquellos pacientes que fueron controlados por un periodo superior a 6 meses y que consultaron en varias ocasiones (mas de tres consultas), lo que nos permitio conocer su evolucion clinica a largo plazo. Se reviso detalladamente el cuadro clinico, examenes efectuados, tratamiento y evolucion posterior. Existio, al igual que en otras revisiones (2) predominio del sexo masculino, en una proporci6n 2:1 (74 hombres y 32 mujeres).Para calificar el grado de constipaci6n, se considero que esta era leve cuando cedia espontaneamente sin necesidad de medicamentos o enemas; moderada (grado II), si el paciente requeria de laxantes en forma rutinaria para obrar y grave (grado III), cuando se obtenian evacuaciones solo mediante el uso de lavados intestinales repetidos.El diagn6stico de enfermedad de Hirschsprung, sospechado por la historia clinica y la radiologia, se confirmo siempre con el estudio histologico de la pared rectal, obtenida nor biopsia segun la tecnica preconizada por Swenson (3) 6 en el acto quirurgico, al efectuarse el examen de la zona aganglionar resecada. RESULTADOSEn la tabla N? 1 se encuentran agrupados los pacientes de acuerdo al grado de constipacion que presentaban en el primer control. La mayor parte de los ninos tenian una constipacion entre moderada y severa, lo que los obligo a acudir al hospital y no continuar el tratamiento en el consultorio periferico. 405
