ÖZETDünya Sağlık Örgütü, adölesan dönemini 10-19 yaşları arasındaki dönem olarak tanımlamakta ve dünyada her altı kişiden birinin adölesan olduğunu bildirmektedir. Bu dönemde gerçekleşen fiziksel ve psikolojik değişiklikler, madde bağımlılığı, korunmasız cinsel ilişki ve istenmeyen gebelikler gibi bazı riskli davranışlara neden olabilmektedir. Hem gelişmiş, hem de gelişmekte olan ülkelerde adölesan gebelikler önemli bir sağlık sorunudur. Adölesan gebeliğin risk faktörleri, çok yönlü ve karmaşıktır. Abortus, erken doğum tehditi, gebelikte hipertansiyon, anemi, düşük doğum ağırlığı, konjenital anomali riski, artmış neonatal mortalite ve emzirme problemleri bunlar arasında sayılabilir. Birçok açıdan riskli bir dönemde olan adölesan anneler arasında emzirmeye başlama, emzirmeye devam etme oranlarının az olması da, anne ve bebeğin sağlığını olumsuz etkileyen ve göz ardı edilen önemli sorunlardandır. Düşük sosyoekonomik durum, partner yokluğu, sosyal destek yetersizliği, olumsuz anne ve eş desteği, ağrı, kültürel etkiler, bilgi eksikliği, olumsuz deneyimler, emzirme niyeti, emzirmeye ilişkin tutum ve öz-yeterlilik emzirmeyi etkileyen faktörler olarak belirtilmektedir. Anahtar kelimeler: Adölesan, annelik, emzirme ABSTRACTWorld Health Organization defined the adolescent period between the ages of 10-19 and declared that one out of every six people are adolescent. Physical and psychological changes in this period might cause risky behaviors such as drug abuse, sexual intercourse with no protection and unwanted pregnancy. Adolescent pregnancies are an important health problem in both developed and developing countries. Risk factors of adolescent pregnancy are complicated and miscellaneous. Abortus, threat of premature birth, hypertension in pregnancy, anemia, low weight of the newborn, congenital anomaly risk, increased neonatal mortality and breast feeding problems are some of them. For adolescent mothers, who are in a risky period for many aspects, low rates of starting and continuing breast feeding is another important and neglected problem affecting mother's and newborn's health negatively. Low socio-economic condition, absence of partner, inadequate social support, negative support of parents and couple, pain, cultural factors, lack of knowledge, negative experiences, breast feeding intention, manner regarding breast feeding and self-sufficiency were reported as factors affecting the breast feeding.
Bu araştırmanın amacı hemşirelerin bireysel ve profesyonel değerleri arasındaki kuşaklararası farklılıkları belirlemektir. Gereç ve Yöntem: Tanımlayıcı ve karşılaştırmalı tipteki bu çalışma, İzmir'deki bir üniversite hastanesinde çalışan, X kuşağındaki 85(doğum tarihi 1965-1979 yılları arasında olanlar) ve Y kuşağındaki 79 (doğum tarihi 1980-1999 yılları arasında olanlar) olmak üzere toplam 164 hemşire ile yürütülmüştür. Veriler, araştırmacılar tarafından 30 dakikalık süre içerisinde yüz yüze görüşme yöntemi ile Birey Tanıtım Formu, Portre Değerler Anketi ve Hemşirelerin Profesyonel Değerleri Ölçeği-Revize kullanılarak toplanmıştır. Verilerin değerlendirmesinde tanımlayıcı istatistikler, Ki Kare, Kruskal Wallis, Mann-Whitney U testi ve spearman korelasyon analizi kullanılmıştır. Bulgular: Araştırmaya katılan hemşirelerin %51.8'i X kuşağı, %48.2'si ise Y kuşağıdır (n=164). X kuşağındaki bireylerin %96.4'ü kadın olup, %52.9'u üniversite mezunu, %72.6'sı evli; Y kuşağındaki bireylerin ise %93.7'si kadın, %51.9'u üniversite mezunu, %58.2'si evlidir ve bu özellikler açısından X ve Y kuşağı arasında istatistiksel olarak fark yoktur. X kuşağındaki hemşirelerin Portre Değerler Anketi ve Hemşirelerin Profesyonel Değerler Ölçeği puan ortalamaları arasındaki fark istatistiksel olarak anlamlı değildir. Çalışmaya katılan X ve Y kuşağındaki hemşirelerin Portre Değerler Anketi ile Hemşirelerin Profesyonel Değerleri Ölçeği puan ortalamaları arasında pozitif yönde anlamlı bir ilişki olduğu belirlenmiştir. Sonuç: Çalışmada, X ve Y kuşağındaki hemşirelerde bireysel ve profesyonel değerler açısından fark olmadığı belirlenmiştir. X ve Y kuşağı hemşirelerin birlikte çalışması, sürekli iletişim ve etkileşim halinde bulunmalarının sonucu etkilediği düşünülmektedir.
Bu çalışmanın amacı, ilk kez vajinal doğum gören erkek hemşirelik öğrencilerinin doğum deneyimlerini açığa çıkarmaktır. Yöntem: Nitel araştırma yöntemi kullanılarak yapılan çalışma Eylül-Aralık 2017 tarihleri arasında, İzmir'de bir devlet üniversitesinin hemşirelik lisans programı kapsamında kadın sağlığı ve hastalıkları hemşireliği dersini alan, doğumhane uygulamasında ilk kez vajinal doğum gören ve çalışmaya katılmayı kabul eden 14 erkek öğrenci ile yürütülmüştür. Veriler, Kişisel Bilgi Formu ve Yarı Yapılandırılmış Derinlemesine Görüşme Formu kullanılarak, bireysel derinlemesine görüşme yöntemi ile toplanmış, geleneksel (konvansiyonel) içerik analizi yöntemi ile analiz edilmiştir. Bulgular: Çalışmada iki ana tema (doğum algısı ve tercihler) ve altı alt tema (beklenti, deneyim, kazanım, doğum şekli, doğuma girme ve doğumhanede erkek hemşire) belirlenmiştir. Öğrenciler doğum eylemini izlerken genellikle korku, stres, üzüntü ve mutluluk gibi duyguları bir arada deneyimlediklerini ve kendilerini doğum yapan annenin yerine koyduklarını belirtmişlerdir. Erkek öğrencilerin çoğu, doğum eylemini izledikten sonra annesine duyduğu sevgi ve minnettarlığın daha çok arttığını, ileride (n=11) eşine destek olmak amacıyla doğuma girmek istediğini ve öğrencilerin yarısı (n=7) ileride eşinin normal doğum yapmasını istediğini belirtmiştir. Sonuç: Doğum eylemini izlemek erkek öğrencilerin, kadını ve önemini anlamalarına katkı sağlamıştır. Ayrıca erkek öğrenciler doğum eylemini izledikten sonraki olumlu kazanımları, mezuniyet sonrası erkek hemşirelerin meslek hayatlarında kadınların ihtiyaçlarını belirlemede ve bakımlarını planlamada etkili olacaktır.
Objective: The purpose of this study was to explore the effect of gestational diabetes mellitus (GDM) on depression and breastfeeding self-efficacy during pregnancy and the postpartum period. Methods: This descriptive study was conducted in Obstetrics and Perinatology clinics of two university hospitals in Turkey between July 2016 and June 2017. Women were followed up two times. The first follow-up was performed face-to-face in the gestational week 34th to 38th and the second one was performed by telephone in the 8th week of the postpartum period. In the first follow-up, 104 pregnant women with GDM and 133 pregnant with non-GDM women were interviewed. In the second follow up, 30 women could not be reached in both groups. Data were collected by the Individual Description Form, Edinburgh Postpartum Depression Scale, and Breastfeeding Self-Efficacy Scale-Short Form. Descriptive statistics, repeated measures analysis of variance and correlation analysis were used in the data analysis. Results: The depression risk of mothers with GDM was found higher compared to non- GDM mothers. No significant difference was found between the depression and breastfeeding self-efficacy mean scores in the antenatal and postpartum period of women by the presence of GDM. There was no significant difference between the depression and breastfeeding self-efficacy mean scores in the antenatal and postpartum period by the presence of GDM and some confounding variables. As the breastfeeding self-efficacy level of mothers with GDM both in the antenatal and postpartum period increased, the depression risk decreased. Conclusion: In the study, it has been concluded that GDM does not have an impact on depression and breastfeeding self-efficacy during pregnancy and in the postpartum period.
