Introduction: Orofacial granulomatosis is characterized by recurrent swelling affecting the lips, cheeks, and tongue. The rarity of this pathology and the lack of consensus in therapeutic management make the reporting of this clinical case relevant. Observation: A 48-year-old man consulted for labial and gingival orofacial granulomatosis. The treatment consisted of 40 mg/L injections of triamcinolone acetonide once weekly for 3 weeks. The symptoms improved after 1 week of treatment. Comments: The usual treatment for this condition targets the inflammation caused by the lesion. Corticosteroids (clobetasol, triamcinolone acetonide, prednisolone), monoclonal antibodies (infliximab, adalimumab), or TNF-α inhibitors are commonly used. Symptom recurrenceis frequently observed after treatment with corticosteroids. Biotherapies are often used as a second-line treatment. Conclusion: Orofacial granulomatosis symptoms are rare and difficult to diagnose due to its varying manifestations. Common treatments target one of the steps of the inflammatory response. The detection of specific cellular markers is a way to enable a more precise etiological diagnosis and allows for a more targeted therapy.
La Haute-Normandie présente plusieurs désavantages concernant la prise en charge bucco-dentaire des personnes en situation de handicap : une démographie médicale sinistrée, l’absence de faculté dentaire et de réseaux de soins odontologiques. Une enquête épidémiologique transversale a été réalisée entre novembre 2014 et février 2015 auprès de la cohorte des chirurgiens-dentistes de cette région à l’aide d’un questionnaire. Les thématiques abordées dans ce questionnaire portaient, d’une part, sur l’accessibilité et la mise aux normes des cabinets dentaires ainsi que sur les facteurs d’influence freinant leur mise en accessibilité, et, d’autre part, sur la prise en charge des patients porteurs de handicap par les praticiens de cette région : impact des mesures incitatives, facteurs d’influence compliquant la prise en charge et recours à des correspondants de structures privées et publiques. Si la prise en charge du handicap concerne l’immense majorité des praticiens, nous relevons une appréhension chez ces derniers à adapter leurs protocoles de soins à cette patientèle spécifique. Les attentes des chirurgiens-dentistes dans le domaine du handicap sont multiples : création de réseaux spécifiques de soins, meilleure coordination des acteurs médicaux et sociaux, mise en place de formations spécifiques concernant la prise en charge des patients porteurs de handicap.
L'augmentation de volume osseux crestal pré implantaire peut constituer aujourd'hui un réel défi chez certains patients dès lors que l'on souhaite obtenir une reconstitution implanto-prothétique stable, esthétique et pérenne. Différentes modalités d'augmentation du volume osseux maxillaire existent selon l'origine du greffon. Ainsi l'autogreffe osseuse, considéré encore comme le gold standard requiert un prélèvement peropératoire intra ou extra-oral. Les greffes osseuses allogéniques ont l'avantage de permettre de s'exonérer d'un prélèvement sur un site donneur et ainsi de diminuer la morbidité de cette chirurgie avec un taux de succès équivalent à l'autogreffe (Esposito et al., 2009 cochrane). Les techniques de reconstruction osseuses faisant appel aux substituts osseux avec membrane armée en titane montrent leurs limites en termes de hauteur de volume régénéré à 7 mm environ (Simion et al., 2001 clin oral impl res). La demande croissante de réhabilitations implantaires nous conduit à prendre en charge des patients de plus en plus âgés avec de ce fait très souvent des co-morbidités associées. Le concept d'élaboration d'un greffon osseux sur mesure à partir d'une imagerie tomodensitométrique est de ce fait séduisant pour le chirurgien oral en raison du gain de temps chirurgical en évitant le prélèvement sur un site donneur ainsi que la phase de découpe et de façonnage du greffon, tout en améliorant de façon significative la congruence de la surface du greffon en regard du site receveur et en facilitant son ostéosynthèse réduisant ainsi les risques de micro-mobilité et donc d'échec ( Antoun et al, 2013 aos).C'est à partir de cette innovation proposée par une société prestataire fournissant le greffon osseux sur mesure que nous vous présentons le cas clinique de reconstitution pré-implantaire maxillaire chez une patiente de 81 ans. Toutes les étapes depuis la consultation initiale, le bilan scannographique en passant par le choix du type du greffon (spongieux ou cortico-spongieux), la modélisation et conception 3D du greffon, et la chirurgie reconstructrice pré-implantaire sous anesthésie locale seront exposées. La réduction du temps chirurgical à moins de 45 minutes, les suites opératoires diminuées et le confort de notre patiente nous autorisent à penser que dans l'avenir ces techniques vont se développer du fait de nombreux avantages tant pour le chirurgien que le patient. 64ème Congrès de la SFCO, 03028 (2016)
