It is already established that sarcopenia is associated with adverse outcomes; however, few studies have focused on patients who have suffered an acute cardiovascular event. The use of SARC-F, a 5-item sarcopenia screening questionnaire, in these patients remains to be investigated. We aimed to investigate whether SARC-F can predict adverse outcomes in patients admitted to a hospital with a suspected infarction. This is a 1-year prospective cohort study. During hospitalization, patients completed the SARC-F questionnaire (scores ≥ 4 considered positive for the risk of sarcopenia). Length of hospital stay (LOS), new hospital admission, myocardial infarction, and cardiovascular mortality were collected via medical records and phone interviews. In total, 180 patients were evaluated. The median age was 60.6 years; 72.3% of the participants were men, and half of the sample had comorbidities. The median SARC-F score was 1.0 (interquartile range, 0–3.0), and 21.1% of the participants screened positive. Risk of sarcopenia was independently associated with longer LOS (odds ratio, 2.34; 95% CI, 1.09–5.04; p = 0.030) and hospital readmission (odds ratio, 3.73; 95% CI, 1.60–8.69; p = 0.002). One-fifth of post-acute cardiovascular event patients in this cohort screened positive for sarcopenia using the SARC-F screening questionnaire. Positive scores were associated with a longer LOS and hospital readmission.
Uma estratégia que vem sendo bastante difundida para otimização da demanda espontânea e programada na Atenção Primária à Saúde (APS) é o Acesso Avançado (AA), que consiste em um método moderno de agendamento médico que possibilita que o paciente seja atendido no mesmo dia ou em até 48 horas. Assim, o objetivo do estudo foi avaliar a implementação do AA na Unidade Básica de Saúde (UBS) Manoel Paulino dos Santos. Para o desenvolvimento da pesquisa utilizou-se o sistema do AA para (re)organizar o processo de trabalho das equipes e as formas de atendimento aos usuários da unidade que está localizada no município de Timbaúba dos Batista, no interior do Rio Grande do Norte (RN). O plano do AA foi traçado considerando a atuação de uma equipe de Estratégia de Saúde da Família (ESF). Inicialmente, a fim único de entendimento, para melhor desenvolver o plano de AA, foi realizado um levantamento das características da UBS e da sua área de abrangência, considerando ainda quais os problemas recorrentes na unidade. Esse levantamento aconteceu através de conversas em reuniões informais com funcionários da UBS. Assim, com base nas características e no potencial da demanda e oferta da unidade foi traçada novas metodologias de atendimento, estruturadas em fluxogramas de atendimento e na definição de cada responsabilidade dentro do percurso de atenção ao paciente. Considerando que a maior parte da pesquisa foi realizada durante o atual cenário da pandemia do Covid-19, foram também elaboradas estratégias específicas para esse período. Os principais resultados do estudo mostraram que a UBS em questão enfrentava grandes dificuldades oriundas da sobrecarga dos funcionários, do acolhimento ineficiente aos pacientes e da ausência de método para atendimento em tempos da pandemia. Com isso, a implementação do acesso avançado possibilitou uma maior eficiência no desenvolvimento das atividades da UBS, por meio da definição dos deveres e responsabilidades. Melhorou o atendimento aos pacientes com a substituição do sistema de fichas por dia pelo sistema por horários, e proporcionou atendimentos considerando as limitações ocasionadas pela pandemia. De modo geral, com a implementação do AA pôde-se notar uma maior agilidade nos atendimentos e uma atenuação de casos de sobrecarga dos colaboradores, ou seja, os atendimentos da UBS passaram a ser mais eficientes.
