Objective: to identify prevalence and factors associated with anxiety and depression in nursing professionals who work to cope with COVID-19 at a university hospital. Methods: a cross-sectional observational study using a sociodemographic questionnaire and Hospital Anxiety and Depression Scale, with 88 nursing professionals. Data were analyzed using absolute and relative frequency and Statistical Package for the Social Sciences. Results: there was prevalence of anxiety (48.9%) and depression (25%). The majority of the sample consisted of women over 40 years old, married or in a common-law marriage, white, with higher education or graduate degree, with an income above 3,000.00 reais, public servants, working 40 hours a week and working in the hospital from 1 to 5 years. Conclusion: we must consider the impact on mental health nursing caused by COVID-19 and intervene with coping strategies to minimize the suffering of professionals.
Objectives: to analyze the diagnostic criteria for ventilator-associated pneumonia recommended by the Brazilian Health Regulatory Agency and the National Healthcare Safety Network/Centers for Disease Control and Prevention, as well as its risk factors. Methods: retrospective cohort study carried out in an intensive care unit throughout 12 months, in 2017. Analyses included chi-square, simple linear regression, and Kappa statistical tests and were conducted using Stata 12 software. Results: the sample was 543 patients who were in the intensive care unit and under mechanical ventilation, of whom 330 (60.9%) were men and 213 (39.1%) were women. Variables such as gender, age, time under mechanical ventilation, and oral hygiene proved to be significant risk factors for the development of ventilator-associated pneumonia. Conclusions: patients submitted to mechanical ventilation need to be constantly evaluated so the used diagnostic methods can be accurate and applied in an objective and standardized way in Brazilian hospitals.
Objective: To identify the frequency and factors associated to incontinence associated dermatitis in elderly people. Methods: Cross-sectional study with 202 elderly patients admitted to a university hospital between September 2017 and January 2018. Data collection included: cognitive screening, sociodemographic and clinical questionnaire. It was performed exploratory and descriptive analysis, where prevalence and ratios (PR) were calculated. Results: Prevalence of injury was 9.4%, age range 70-79 years (13.5%), African American (21.4%), hospitalization period ≥ 21 days (44.4%), in use of nasogastric tube (33.3%), medical device (11.3%), restricted mobility (18.5%). It was associated with hospitalization period, use of nasogastric tube, restricted mobility, medical devices, and cognition. Conclusion: It was confirmed an average frequency of incontinence associated dermatitis in elderly patients and association to factors such as hospitalization period, immobility, cognition, use of nasogastric tube and devices. It must be highlighted the importance of prevention measures, early detection, assessment and monitoring of this type of injury.
Resumo Objetivo Identificar os fatores associados à condição de acamado entre idosos brasileiros, analisando-se condições socioeconômicas e de saúde e utilização de serviços de saúde. Método Estudo transversal com dados secundários provenientes da Pesquisa Nacional de Saúde (2013). Contou com a população de indivíduos com ≥60 anos (n=11.177). A condição de acamado foi considerada variável dependente e as características sociodemográficas, condições de saúde e utilização de serviços médicos como independentes. Foram realizadas análises de regressão logística, e reportada a razão de chance bruta (RCbruta) e ajustada (RCajustada) com o intervalo de confiança de 95% (IC95%). Resultados Verificou-se que 4,9% dos idosos eram acamados. Essa condição mostrou-se associada ao sexo masculino (RCajustada=1,45; IC95%=1,13-1,84), analfabetismo (RCajustada=1,37; IC95%=1,11-1,70) e quantidade de doenças crônicas, sendo idoso com cinco ou mais doenças crônicas apresentava 4,96 (IC95%=2,78-8,85) mais chances do que os sem doença. As doenças associadas à condição acamado foram: episódio de acidente vascular cerebral (RCajustada=3,03; IC95%=1,29-8,43), diagnóstico de hipertensão arterial sistêmica (RCajustada=1,71; IC95%=1,31-2,24), alterações nos níveis de colesterol (RCajustada=2,08; IC95%=1,37-3,17) e depressão (RCajustada=5,64; IC95%=2,42-13,14). Ainda, idosos que precisaram de atendimento relacionado à própria saúde (RCajustada=16,94; IC95%=7,15-40,16), internamento (RCajustada=8,10; IC95%=4,20-15,54) e atendimento emergencial no domicílio nos últimos 12 meses (RCajustada=1,78; IC95%=4,20-15,54) e que consideraram a condição de saúde geral ruim (RCajustada=2,68; IC95%=2,05-3,51) apresentaram mais chances de ser acamado. Conclusão o estudo permitiu identificar importantes fatores associados à condição acamado de idosos brasileiros com destaque para sexo e escolaridade, as variáveis clínicas de doenças crônicas, e utilização mais frequente de serviços de saúde.
Justificativa e Objetivo: A sepse é um desafio para a saúde pública, sendo os protocolos fundamentais para seu manejo. Objetivou-se avaliar a implantação de um protocolo assistencial de manejo de sepse e caracterizar os pacientes submetidos a avaliação clínica. Método: Estudo descritivo, do tipo transversal realizado na unidade de Pronto Atendimento (PA) do Hospital Universitário Regional dos Campos Gerais. O protocolo foi elaborado a partir da literatura, com foco nos pacientes adultos com critérios de síndrome da resposta inflamatória sistêmica (SIRS), sepse, sepse grave e choque séptico, sendo descrito por meio de fluxograma. Fizeram parte do estudo os pacientes atendidos no PA entre julho a setembro de 2016 submetidos ao protocolo (n=50), sendo os dados coletados por meio dos prontuários eletrônicos e analisados descritivamente. Resultados: Para implantação do protocolo realizou-se capacitações da equipe atuante no PA, para aceitação e alinhamento de conhecimentos quanto os aspectos conceituais, técnicos e logísticos. O protocolo é composto por 3 etapas: abordagem inicial ao paciente com sepse; pacote 3/6 horas – otimização hemodinâmica e pacote opcional -otimização de Saturação venosa central de oxigênio/ pressão venosa central. Os pacientes submetidos ao protocolo tinham idade média de 66 anos (dp=±13,71), a maioria era 56% mulheres, apresentaram hipotensão (96%), elevação da creatinina (76%) e leucocitose (70%) entre as SIRS, realização imediata de exames (100%), início da antibioticoterapia (74%) e óbito (72%). Conclusão: a implantação do protocolo subsidiou a identificação precoce dos pacientes, qualificação do cuidado, ganho operacional e as características dos pacientes condizentes com quadros graves de sepse.
