The aim of the study was to evaluate the sensitivity and specificity of allergen-induced basophil CD164 upregulation in patients with seasonal allergic rhinitis caused by allergy to grass pollens. This study was performed in 24 patients with allergy to grass pollens, and in 25 healthy controls. The protocol for allergen-induced basophil CD164 upregulation consisted of whole blood samples processing and staining with anti-CCR3/anti-CD164 antibodies added to a buffer at the beginning of stimulation. We observed dose-dependent allergen-induced basophil CD164 upregulation with 100% of specificity in both used allergen concentrations (12 and 1.2 ng/ml). Higher allergen concentration resulted in 100% and lower concentration in only 70.83% sensitivity. We have observed in the patients statistically significant correlations between anti-IgE stimulation and both allergen concentrations (for 12 ng/ml, r = 0.71, p < 0.0001; and for 1.2 ng/ml, r = 0.64, p < 0.001). We conclude that assessment of allergen-induced basophil CD164 upregulation is a very useful method for in vitro determination of allergy to grass pollens. This method seems to be a very promising tool in laboratory testing of allergies to other allergens.
Introduction: Laboratory test play an essential role in the early detection and diagnosis in an Emergency Department. Laboratory tests are common w COVID-19, however, they are also encountered in diseases with similar presentation as COVID-19 but other etiopathogeneses. The aim: To compare morphology, procalcitonin, and C-reactive protein in patients admitted with the diagnosis of COVID-19 to the Infectious Hospital to the patients with the discharge diagnosis of pneumonia admitted to the Emergency Department in January and February 2019. Material and methods: The study group consisted of 46 COVID-19 patients (60.9% male) aged 63.3±15.3 and 48 pneumonia patients (56.3% male) aged 75.0±13.7. The COVID-19 patients were admitted to the Infectious Department of Boleslawiec County Hospital or were discharged from the Emergency Departments to the Infectious Departments. The age, gender, leucocyte count, lymphocyte count and percentage, hemoglobin, mean corpuscular volume, platelets number, procalcitonin level and c-reactive protein were retrieved from medical electronic records. Results: The hematology tests did not reveal significant differences in lymphocyte count and percentage, however the white cells number was significantly higher in pneumonia than in COVID-19 group. C-reactive protein did not differ between groups. There was a trend to the higher level of procalcitonin in pneumonia group and hemoglobin level in COVID-19 group. Mean corpuscular volume and leucocyte number were significantly higher in the pneumonia group. Conclusions: 1. Lower leucocyte number and lower mean corpuscular volume of erytrocyte in COVID-19 patients than in pneumonia patients may reflect differences in the reaction of the host to infectious factors or indicate predisposing factors to COVID-19 infection. 2. Further studies are indicated to confirm and explain obtained results.
Aim: To determine the presence and the importance of a difference in end tidal carbon dioxide between lateral decubitus positions for pulmonary embolism prediction. Material and methods: There were 32 patients aged 65.7±14.4 (16M, 16F) with pulmonary embolism and 15 patients aged 56.7±20.3 (10M, 5F) with excluded pulmonary embolism Capnography was performed in supine, left and right lateral decubitus position. The absolute value of the difference in end tidal carbon dioxide concentration between left and right decubitus position was called the delta index. Demographics and clinical data were collected. Results: The delta index was significantly higher in patients with pulmonary embolism vs those with excluded pulmonary embolism: 4 (3-5.5) mmHg vs 1 (1-2) mmHg p<0.001. Area under curve for the delta index was 0.92; 95% CI 0.83-1.0 p 3 mmHg to predict PE the sensitivity and specificity was 66% and 100%, respectively. Conclusions: The patients with pulmonary embolism had increased variability of end tidal carbon dioxide concentration while changing their position.
StreszczenieLeczenie pacjenta z zatrzymaniem krążenia w przebiegu rytmów niedefibrylacyjnych wymaga poszukiwania potencjalnie odwracalnych przyczyn. Nieoznaczalne stężenie glukozy w badaniu glukometrem nasuwa podejrzenie kwasicy cukrzycowej, odwodnienia i hiperkaliemii jako przyczyn zatrzymania krążenia. W przypadku dostępnego zapisu elektrokardiograficznego (EKG), wykonanego bezpośrednio przed zatrzymaniem krążenia lub po skutecznej resuscytacji, należy poszukiwać cech EKG sugerujących hiperkaliemię. Według niektórych autorów załamki P mogą być niewidoczne w czasie rytmu zatokowego przewodzonego do komór, ponieważ komórki mięśnia przedsionków są bardziej wrażliwe na podwyższone stężenie potasu niż komórki mięśnia komór i rytm ten jest bardzo specyficzny dla hiperkaliemii [1].W niniejszej pracy przedstawiono zapisy EKG pacjenta z ciężkimi zaburzeniami metabolicznymi w przebiegu źle kontrolowanej cukrzycy. Opis przypadkuMężczyzna 68-letni z wieloletnią cukrzycą leczoną insuliną, który nie kontrolował ostatnio glikemii, zgłosił nasilone osła-bienie i wymioty, co było powodem wezwania zespołu ratownictwa medycznego (ZRM). Stężenie glukozy było nieoznaczalne glukometrem. Po wykonaniu EKG przez ZRM (ryc. 1) u pacjenta wystąpiło nagłe zatrzymanie krążenia w mechanizmie asystolii. Pacjenta skutecznie zresuscytowano.
