PurposeSeveral parameters have been described for determining the success or failure of dental implants. The surface properties of transgingival implant components have had a great impact on the long-term success of dental implants. The purpose of this study was to compare the tendency of two periodontal pathogens to adhere to and colonize zirconia abutments and titanium alloys both in hard surfaces and soft tissues.MethodsTwelve patients participated in this study. Three months after implant placement, the abutments were connected. Five weeks following the abutment connections, the abutments were removed, probing depth measurements were recorded, and gingival biopsies were performed. The abutments and gingival biopsies taken from the buccal gingiva were analyzed using real-time polymerase chain reaction to compare the DNA copy numbers of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and total bacteria. The surface free energy of the abutments was calculated using the sessile water drop method before replacement. Data analyses used the Mann Whitney U-test, and P-values below 0.05 find statistical significance.ResultsThe present study showed no statistically significant differences between the DNA copy numbers of A. actinomycetemcomitans, P. gingivalis, and total bacteria for both the titanium and zirconia abutments and the biopsies taken from their buccal gingiva. The differences between the free surface energy of the abutments had no influence on the microbiological findings.ConclusionsZirconia surfaces have comparable properties to titanium alloy surfaces and may be suitable and safe materials for the long-term success of dental implants.

RESUMEN: La implantología ha evolucionado rápidamente en los ultimos 40 años y el empleo de biomateriales en la recuperación de tejidos es cada vez más frecuente. Por esta razón, es necesario conocer el proceso de reparación y la reacción producida entre los los tejidos huéspedes y los biomateriales a nivel histológico, ya que ello contribuirá al éxito, en el largo plazo, de los tratamiento rehabilitadores. Clínicamente, se ha podido observar la efectividad de diferentes compuestos, pero cuando han sido analizados histológicamente, han demostrado algunas variaciones que, muchas veces, las hacen inaceptables para la terapia odontológica. Los objetivos de esta revisión es conocer la forma en que se produce la cicatrización ósea en torno a diferentes biomateriales y como se desarrolla la cicatrización ósea en rehabilitación sobre implantes con diferentes protocolos de carga.

Las técnicas de remoción de injertos intraorales están bien descritas en la literatura internacional; sin embargo, siempre es importante recordar las condiciones que pueden influir en la presencia de complicaciones transquirúrgicas y postquirúrgicas. Por otra parte, las técnicas de reconstrucción ósea son realizadas por muchos cirujanos dentistas, con diferentes grados de formación, lo que puede permitir el desconocimiento de algunos puntos importantes cuando se realice el procedimiento quirúrgico. Los autores realizan una revisión de la literatura con énfasis en las condiciones de la técnica quirúrgica, tipos de complicaciones y su manejo. Finalmente, podemos señalar que después de protocolizar las técnicas quirúrgicas, el siguiente paso es el manejo de las complicaciones, que pueden disminuir con una correcta conducta quirúrgica.

Introduction and Objective: Medication-related osteonecrosis of the jaw (MRONJ) is a recently described adverse effect associated with anti-resorptive and anti-angiogenic agents. The objective of this article is to report a case of MRONJ treated surgically using a ultrasonic device (piezosurgery). Case report: A 66 year-old female being treated with a bisphosphonate (alendronate) for more than 4 years developed MRONJ of the lower jaw after dental extraction. Two debridement attempts of necrotic bone tissue and primary intention closure with conventional techniques were performed, without success. Finally, treatment with antibiotics and piezosurgery leaded to disease remission. Conclusion: Even though more studies are needed to create a protocol that might include this therapeutic modality, specially when there was no success with conservative management or conventional treatment techniques, the use of ultrasonic devices is an option for treatment of MRONJ.
