The nose is undoubtedly an important facial feature. The rhinologic surgeon with interest in external nasal soft tissue surgery, oncologic or otherwise, may often require reconstruction of a soft tissue defect, from the medial canthus to the nasal ala laterally, to the nasal tip medially. The coverage needs tissues which resemble the adjoining skin texture and have a thickness which would fill the defect appropriately without appearing as a separate patch and upset the patient psychologically. Therefore we utilized local rotation and advancement flaps for the reconstruction of facial defects. Experience with these flaps is being shared.
<p class="abstract"><strong>Background:</strong> Effect of cortical mastoidectomy on graft uptake and graft mobility and thence shift of hearing thresholds was analysed in tympanoplasty in discharging ears.</p><p class="abstract"><strong>Methods:</strong> In this study, 60 subjects of safe chronic suppurative otitis media were selected from the outpatient clinics of Dayanand Medical College and Hospital, Ludhiana. All were subjected to tympanoplasty utilizing the underlay technique. Mastoid exploration was undertaken in the ears with persistent ear discharge. </p><p class="abstract"><strong>Results:</strong> Cortical mastoidectomy performed in 20 (33.3%) out of 60 patients and most of cases were done in superiorly based/superior cuff tympanoplasty group in our study. No statistically significance found between cortical mastoidectomy and different flap technique. Mean hearing gain is more with cortical mastoidectomy (16.85 dB) than without cortical mastoidectomy (13.05 dB) and graft uptake was 97.5% without cortical and 95% with cortical mastoidectomy.</p><p class="abstract"><strong>Conclusions:</strong> There was higher mean gain in thresholds of hearing in subjects with cortical mastoidectomy with tympanoplasty though uptake was almost equal to those without mastoidectomy.</p>
<p class="abstract"><strong>Background:</strong> The antero-inferior recess of the ear canal is not visible to the otologist, thereby effecting proper placement of the graft and thereby affecting its uptake .The visibility of this region in the adult population is assessed.</p><p class="abstract"><strong>Methods:</strong> The quadrants of tympanic membrane were examined in 60 subjects with perforated tympanic membrane undergoing tympanoplasty. The study was conducted in the department of ENT-HNS (ear, nose and throat-head and neck surgery), Dayanand Medical College and Hospital, Ludhiana. </p><p class="abstract"><strong>Results:</strong> With the microscope in 48 (80%) entire perforation could be visualized and in 12 (20%) margins were not in the line of view. 5 perforations were posterior superior and 12 were total perforations. Otoendoscope examination with zero degree could show the entire margin of the perforation.</p><p class="abstract"><strong>Conclusions:</strong> In 80% population the antero-inferior quadrant of the tympanic membrane is accessible to straight vision of the microscope.</p>
Background: Different tympano meatal vascularised flaps were compared in the tympanoplastic surgery in chronic suppurative otitis media.Methods: 60 cases with diagnosis of chronic suppurative otitis media were selected from ENT OPD of Dayanand Medical College and Hospital Ludhiana. 6 types of tympanomeatal flaps were used and 10 cases of each flap were taken. The study was conducted for the period of 1 year from June 2015 to August 2016.Results: Maximum numbers of patients were in the age group of 31-40 years (26.7%), followed by age group of >50 years (23.3%). Minimum age among these patients was 11 years and maximum 65 years. Mean age was 36.67. Graft failure was in 2 patients, 1 lies under age group 31-40 years and other was in more than 50yrs group. Female patients constituted 56.7% and male patients were 43.3% in our study population. Graft uptake was 93% in females and 100% in males. In our study on 60 patients, 41 (68.3%) patients had a rural and 19 (31.7%) an urban background.Conclusions: Age does not affect the results of tympanoplasty, whatever flap maybe utilised. Awareness in rural population regarding reconstructive ear surgery is there nowadays though there is still gender discrimination, with less number of ladies opting for this elective surgery.
<p>"Mucormycosis" term is used in context to a group of fungal diseases caused by the order mucorales. Typically with predilection for the pulmonary and the sino-nasal-orbital tissues, it can very rarely affect the laryngeal as well as the tracheal mucosa either as skip lesions or in continuity. The management of such an involved airway is quite challenging. Literature documents sporadic cases of laryngeal manifestation of mucormycosis. Reported here is such a case of a diabetic male presenting with dyspnoea of sudden onset. Video laryngoscopy revealed extensive glottic edema with a reduced glottis chink. Elective prophylactic tracheostomy followed by direct laryngeal trucut biopsy was undertaken. Histopathology was suggestive of mucormycosis and the patient was administered titrated intravenous Amphoterecin B. The response was favorable and the patient at present is symptom free. </p>
