Nail involvement is an extremely common feature of psoriasis and affects approximately 10-78% of psoriasis patients with 5-10% of patients having isolated nail psoriasis. However, it is often an overlooked feature in the management of nail psoriasis, despite the significant burden it places on the patients as a result of functional impairment of manual dexterity, pain, and psychological stress. Affected nail plates often thicken and crumble, and because they are very visible, patients tend to avoid normal day-to-day activities and social interactions. Importantly, 70-80% of patients with psoriatic arthritis have nail psoriasis. In this overview, we review the clinical manifestations of psoriasis affecting the nails, the common differential diagnosis of nail psoriasis, Nail Psoriasis Severity Index and the various diagnostic aids for diagnosing nail psoriasis especially, the cases with isolated nail involvement. We have also discussed the available treatment options, including the topical, physical, systemic, and biological modalities, in great detail in order to equip the present day dermatologist in dealing with a big clinical challenge, that is, management of nail psoriasis.
This study indicates that PRP-enriched epidermal suspension transplant (LA-PEEST) has the potential to improve the rate of healing and repigmentation in vitiligo patches. Further investigations and larger controlled trials are required to establish this technique as a rapid surgical method to restore pigmentation.
Waardenburg syndrome is an autosomal dominant disorder with an incidence of 1 in 40,000 that manifests with sensorineural deafness, pigmentation defects of the skin, hair and iris and various defects of neural crest-derived tissues. This genetically heterogeneous disease accounts for >2 % of the congenitally deaf population. Mutations in the EDN3, EDNRB, MITF, PAX3, SNAI2, and SOX10 genes can cause Waardenburg syndrome. We here report a case of 12 year old female who presented with chief complaint of decreased hearing in both ears and had clinical features consistent with Waardenburg syndrome. She had a distinct white forelock of hair in the midline along with striking bilateral blue iris. Also a white depigmented patch was present on the right forearm. Both eyes had bright red fundal reflex with choroidal depigmentation. Her younger brother, the second case in this study, had similar blue eyes, white forelock of hair, depigmented skin patch and choroidal depigmentation but with normal hearing. Their father had a history of premature graying of hair. All the primary care physicians coming across a child with blue eyes and white forelock of hair should get the child's hearing tested at the first instance, if not already tested. An early diagnosis and improvement of hearing impairment with timely intervention are the most important for psychological and intellectual development of children with Waardenburg syndrome.
<p class="abstract">Pleomorphic adenomas (mixed tumors) are the most common benign tumor of the major salivary glands. In addition, they may also occur in the minor salivary glands of the hard and soft palate. Intranasal pleomorphic adenomas are unusual. We report a rare case of large sized pleomorphic adenoma arising from the nasal septum. A 42-year-old man presented with a 3 month history of multiple episodes of nasal bleeding and obstruction on right side of nose. On examination we found a non-tender firm mass extending upto the nasal vestibule which bled on probing. Computed tomographic scans revealed a mass in the right anterior nasal cavity and spur on left side. Paranasal sinuses, posterior choanae and nasopharynx were normal. An intranasal endoscopic approach was used to achieve a wide local resection along with coagulation of base and spurectomy on the left side. The mass was 2.5×2.0 cm with a broad based attachement of 1.0 cm on the nasal septum. The microscopic finding showed a lobular and duct-like structures consisting of a loose chondromyxoid stroma suggestive of a pleomorphic adenoma. Large sized nasal cavity mass with history of epistaxis and which bleeds on probing should be finally assessed under general anaesthesia. It should be excised endoscopically and subjected to histopathological examination<span lang="EN-IN">.</span></p>
