Objetivo: Determinar a efetividade da intervenção entrevista motivacional no autocuidado dos pacientes com insuficiênciacardíaca. Método: Trata-se de uma revisão sistemática, atualizada em julho de 2018, realizada nas bases MEDLINE, CINAHL,Cochrane Library, Joanna Briggs Institute Library, PubMed, LILACS e Scopus com os descritores: heart failure/insuficiênciacardíaca, motivational interviewing/entrevista motivacional, self care/autocuidado e patient compliance/cooperação dopaciente. Principais resultados: Foram identificados, inicialmente, 25 estudos em potencial para inclusão nesta revisão, noentanto, após serem aplicados os critérios de elegibilidade, 14 estudos foram encaminhados a dois revisores para avaliação metodológica, diante disso, três estudos foram selecionados após alcançarem rigor suficiente para inclusão e síntese.Conclusão: Percebe-se nos estudos, que é o impacto da comunicação na relação paciente x profissional de saúde, o quepromove um elo fundamental e diretamente relacionado ao sucesso do tratamento. A síntese dos estudos evidenciou aefetividade da intervenção Entrevista Motivacional na adesão e no autocuidado dos pacientes com insuficiência cardíaca.Palavras-chave: Insuficiência Cardíaca; Entrevista Motivacional; Autocuidado e Cooperação do Paciente. ABSTRACTObjective: To determine the effectiveness of the motivational interview intervention in the self-care of patients with heartfailure. Objetiva-se identificar os diagnósticos de enfermagem da NANDA-I mais frequentes em uma clínica de insuficiência cardíaca, verificar a associação dos diagnósticos de enfermagem e outras variáveis independentes com o óbito. Estudo de coorte,retrospectivo com coleta de dados clínicos em prontuários e contato telefônico com os pacientes/familiares para a verificação dehospitalização/óbito. A análise ocorreu através do cálculo de frequências, mínimo, máximo, média, desvio padrão e regressão logística. Dentre os resultados foram identificados como diagnósticos de enfermagem mais frequentes a ansiedade, disfunção sexual, fadiga, intolerância a atividade e conhecimento deficiente, entretanto, não apresentaram associação com o óbito. Como previsores para óbito estão o sexo feminino, presença de internação hospitalar e não presença de obesidade. Conclui-se que as respostas dos pacientes à doença puderam ser identificadas através dos diagnósticos de enfermagem e de outras variáveis.
Objective: To verify the effect of a cognitive and motor stimulation program on self-care, quality of life, depression, functional capacity and cognition of patients with chronic heart failure. Methodology: This is a quasi-experimental study of the type before and after, with a sample of 21 patients from a clinic specialized in the treatment of heart failure located in the state of Rio de Janeiro. The patients received the intervention of cognitive and motor stimulation, through a weekly program, with eight sessions of 90 minutes that consisted of cooperative, pedagogical games, group dynamics and circular dance. The assessed outcomes were self-care skills, quality of life, depression, activities of daily living and cognition. Statistical analysis occurred through the analysis of frequencies, mean, median. Parametric (Paired T-Student) and nonparametric (Wilcoxon) tests were used considering the bivariate p value <0.05. The final sample consisted of 21 patients, with a mean age of 67.92 ± 11.48. Result: Although the primary self-care outcome did not improve (p = 0.175), the pre- and post-intervention data showed significant improvements in the outcomes cognition (p≤0.05), quality of life (p≤0.05) and depression (p≤0.05) showing a significant effect of the intervention. Conclusion: Participants evolved positively in the outcomes cognition, depression and quality of life. Self-care outcomes and activities of daily living did not have statistical significance in their scores. Evidencing the importance of conducting new studies and deepening research in the field of cognition and HF.
