RESUMENLos estudios demuestran que la sobrevida después de un paro cardíaco disminuye el 10% por cada minuto de atraso en la desfibrilación y que la tasa de supervivencia es del 98% cuando se consigue en 30 segundos. En la atención de un paro cardíaco es primordial que se incluya en la capacitación la utilización de los desfibriladores externos semi-automáticos (DEA). El objetivo de este estudio fue comparar la Habilidad Psicomotora y el Conocimiento Teórico de legos en la técnica de la resucitación cardiopulmonar (RCP) utilizando el DEA, antes y después de la capacitación. La muestra estuvo formada por 40 empleados administrativos de una institución pública que recibieron capitación en la técnica de RCP, utilizando el DEA, en laboratorio. El aumento significativo de aciertos en los ítems del instrumento de evaluación de la habilidad Psicomotora y del Conocimiento teórico, después de la capacitación, indica que hubo mejora en el desempeño de los participantes para realizar la RCP con el uso del DEA. DESCRIPTORES ABSTRACTStudies demonstrate that, for every minute delayed on defibrillating a heart arrest patient, survival chances decrease by 10%, and that the same chances of survival are 98% effective when it is employed within 30 seconds. While attending a heart arrest patient, it is crucial that the use of external semi-automated defibrillator (AED) is included in the training. The purpose of the present study is to compare Psychomotor Ability and the Theoretical Knowledge of lay people on cardiopulmonary resuscitation (CPR) using AED, before and after training. This sample was composed of 40 administrative workers of a public institution that were trained on CPR technique using EAD, as an experiment. The significantly higher scores in the assessment instrument items of Psychomotor Ability and Theoretical Knowledge, after training, indicates that the participants have presented improvements in their performances. Questão freqüente no uso da Escala de Coma de Glasgow (ECGl), na fase aguda, em pacientes internados devido ao trauma crânio-encefálico (TCE) é a subestimação decorrente de situações impeditivas como intubação endotraqueal/traqueostomia, sedação e edema palpebral. O objetivo deste estudo foi identificar e determinar a subestimação na pontuação total da ECGl quando se utiliza a pontuação 1 nas situações de impedimento para a sua avaliação. A amostra estudada foi de 76 pacientes internados com TCE no Hospital das Clínicas da FMUSP. Em 42 (55,3%) pacientes, não havia impedimentos e foram realizadas 136 avaliações. Em 34 (44,7%), havia impedimentos caracterizados por intubação ou traqueostomia, podendo estar ou não associados com edema palpebral e sedação, e o total de avaliações foi de 310. A pontuação nesses pacientes total variou de 3 a 11, com os escores mais freqüentes de 3 e 6. Pelos valores estimados pela regressão linear, a partir das pontuações obtidas em AO e MRM foram obtidas as seguintes subestimações: média=1,03 ±1,36, mediana=0,54 (intubação ou traqueostomia); média=0,40 ±0,79, mediana=0,00 (intubação ou traqueostomia + sedação); média=0,57 ±0,96, mediana=0,27 (intubação ou traqueostomia + sedação + edema palpebral). Conclui-se que, no TCE grave, a pontuação total da ECGl fixando a MRV em 1, embora subestimada, encontra-se próxima da real.
Objective: Identify factors of injured motorcyclists associated with hospital length of stay. Methods: A retrospective cross-sectional study of motorcyclists with acute traumatic injury admitted to three reference trauma hospitals in São Paulo. Medical records of patients and necropsy reports were analyzed to extract variables that could be associated with length of stay, followed by an analysis by multiple linear regression to verify associated factors. Results: One analysis of 91 motorcyclists showed that the following were associated with long length of stay (p<0.05): increased severity of trauma and infectious complications, pressure ulcers, rhabdomyolysis, and acute respiratory distress syndrome. Pressure ulcers and surgical site infections were predictors of long length of stay and death was a predictor of reduced length of stay. Conclusion: The factors associated with length of stay resulted from both traumatic injury and the care provided to injured motorcyclists. ResumoObjetivo: Identificar fatores associados ao tempo de internação hospitalar de motociclistas acidentados. Métodos: Estudo transversal, retrospectivo sobre motociclistas com lesões traumáticas agudas atendidos e internados em três hospitais referência para trauma de São Paulo. Prontuários de pacientes e laudo de necropsia foram consultados para obter variáveis que poderiam se associar ao tempo de internação, seguida da análise de regressão linear múltipla para verificar fatores associados. Resultados: Análise de 91 motociclistas mostrou que o aumento da gravidade do trauma e as complicações infecciosas, úlcera por pressão, rabdomiólise e síndrome da angustia respiratória aguda associaram-se com maior tempo de internação (p<0,05). A úlcera por pressão e a infecção do sitio cirúrgico foram fatores preditores do aumento do tempo de internação e o óbito como preditor de redução da internação. Conclusão: Os fatores que se associaram ao tempo de internação resultaram tanto das lesões traumáticas quanto do processo assistencial prestado aos motociclistas acidentados.
Introducción: La hidrocefalia es una complicación frecuente entre los pacientes neurológicos y su tratamiento en casos agudos se hace a través de un catéter de derivación ventricular externa que permite monitorear y controlar la hipertensión intracraneal. El objetivo de este estudio fue describir la retención del conocimiento de los enfermeros después de la intervención educativa sobre cuidados relativos a la derivación ventricular externa. Materiales y Métodos: Estudio cuasi experimental con enfermeros de una unidad de terapia intensiva de adultos, en el que se evaluó la retención del conocimiento sobre el tema en tres momentos: antes, una semana y tres meses después del entrenamiento. Resultados: Después de una semana se observó una retención significativa, sin embargo no a los tres meses, dado que después de tres meses el índice de aciertos en las preguntas fue significativamente bajo con respecto a las fases anteriores. El desempeño de los participantes fue mejor en las preguntas relacionadas con el manejo del sistema. Discusión: El tiempo es un factor que interfiere en el aprendizaje y los estudios muestran que el conocimiento a los tres meses disminuye significativamente, y después de seis meses y un año casi por completo. Las conductas prácticas basadas en directrices de asistencia tienen mejor retención entre los profesionales, resaltando que resulta más fácil aprender habilidades técnicas que están asociadas con la vivencia profesional. Conclusiones: Se observó una retención de conocimiento significativa entre los profesionales en la primera semana posterior al entrenamiento, pero no a los tres meses después del entrenamiento.
