SUMMARY An engineering‐economic analysis of the behaviours of each of two pre‐combustion gas‐turbine combined cycles with CO2 capture (an Integrated Gasification Combined Cycle (IGCC) and an Integrated Reforming Combined Cycle (IRCC)) is described. Their economic performances have been evaluated in terms of the break‐even electricity selling price. The results show that the proposed pre‐combustion power plant efficiency values (37% and 43.7% for the IGCC and the IRCC respectively) are significantly lower compared to a conventional plant value (55.3%). The CO2 emissions of the latter are less than half those of the conventional plant. Introducing an hypothetical carbon tax equal to 50£/tCO2, the break‐even selling price (BESP) for the proposed IGCC and IRCC plants is 4.40 p/kWh and 4.10 p/kWh respectively, while for the conventional plant amounts to 4.73 p/kWh. A sensitivity analysis has been carried out by varying the most influential investment parameters. The analysis has revealed that, considering the uncertainties associated with these key parameters, a substantial risk that the BESP could exceed the first value obtained is so prominent in some cases to alter their ranking order with respect to the most competitive technology. The final part of the analysis is a Monte‐Carlo simulation to determine the impact of simultaneous variations of all the variables, subject to uncertainty, on the break‐even selling unit price for the generated electricity. From the simulation, it derives that the BESP value ranges from 3.13 p/kWh to over 6.00 p/kWh for the IGCC case with a 50% probability of exceeding the first value obtained (4.40 p/kWh). In the IRCC case, the range of possible value for the BESP is from 2.85 p/kWh to 6.10 p/kWh, and there is a 60% probability that the actual BESP would exceed the first value obtained (4.10 p/kWh). Body surface area (BSA) is probably the most used tool for indexing physiological functions. The most relevant past and present measuring methods have been examined, and an analysis of selected methods used to measure and to estimate was performed. The evaluation of methods demonstrates a relationship between past and present measuring methods, but also highlights the doubtfulness of their suitability due to the criteria used to select the population for measurements and to estimate BSA by formulae, due to the weight variance according to the same height and the lack of a numeric estimate of the body shape. The use of BSA formulae generated from a population with anthropometric measures different from those of a population where the formulae are applied, and the change in today’s BSA towards 1.73 m2 can induce risks by incorrectly estimating GFR and in planning dialysis prescriptions.
A young woman presented with nephrotic syndrome due to membranous nephropathy and a localized form of giant lymph node hyperplasia (Castleman’s disease) occurring as a large intra-abdominal mass. Five months after surgical removal of the mass, only mild proteinuria persisted. Twenty months later a second kidney biopsy showed a near-normal morphology. Six years later the patient was free of symptoms and had normal urinalysis. Reviewing the pertinent literature, it seems to be the first case of a biopsy-proven cure of membranous nephropathy in a patient with membranous nephropathy and Castleman’s disease.
Oltre dieci anni fa venne osservata e analizzata la marginalità in Italia della dialisi peritoneale (DP). Questo lavoro rivisita questo problema della DP in Italia e nel mondo, valutandone le possibili cause su basi economiche e medi-che. Sono state studiate le possibili relazioni esistenti fra prodotto interno loro pro-capite (PIL/pC) e DP %, utilizzando i dati di prevalenza della DP 2007–2008 di 106 Paesi e associando a tali dati quelli dei corrispondenti PIL/pC della World Bank. Non è risultata nessuna correlazione diretta fra PIL/pC e DP %, ma è risultata un'evidente tendenza dei Paesi con reddito maggiore a ricorrere meno alla DP rispetto ai Paesi con reddito minore. Ciò è stato definito più chiaramente rapportando il PIL/pC con la DP % normalizzata sul reddito (DPn): questo approccio ha dimostrato un'evidente e significativa differenza fra Paesi con maggiore e minore PIL/pC, questi ultimi avendo una quota di DPn assai più elevata rispetto ai primi (fra cui l'Italia), in cui la scelta dialitica è apparsa chiaramente basata su altri fattori che non il PIL/pC, assai presumibilmente su un miglior rapporto rimborso/costo e su un minore affidamento verso la DP. Per quest'ultimo aspetto sono stati presi in considerazione dei problemi persistenti nell'esercizio della DP, il drop out per peritonite, che, anche se molto ridotto, merita ancora una valutazione in tutte le statistiche, e la perdita di efficacia peritoneale, dovuta ai perduranti danni legati alle soluzioni di dialisi, ridotti ma non ancora aboliti: si deve ritenere che rimborsi inadeguati e problemi clinici pesino negativamente sul trend della diffusione della DP.
Valutazione economica -È evidente che l'aspetto economico assume un peso notevole nella selezione di una terapia, quando si tratti di una procedura cronica di lunga durata e di alto costo, e vi siano più modelli comparabili in efficacia. Al tempo stesso, il concetto di "aspetto economico" può cambiare molto se considerato dall'erogatore della terapia o dal finanziatore, e in ambedue a seconda della loro posizione operativa: erogatore privato → [finanziatore pubblico/privato] ← erogatore pubblico, e a seconda del regime sanitario generale (sistema assistenziale diretto totale, sistema misto, rimborso imposto, rimborso concordato o fluttuante in regime di mercato libero assoluto).
