BackgroundFew studies of dengue have shown group-level associations between demographic, socioeconomic, or geographic characteristics and the spatial distribution of dengue within small urban areas. This study aimed to examine whether specific characteristics of an urban slum community were associated with the risk of dengue disease.Methodology/Principal FindingsFrom 01/2009 to 12/2010, we conducted enhanced, community-based surveillance in the only public emergency unit in a slum in Salvador, Brazil to identify acute febrile illness (AFI) patients with laboratory evidence of dengue infection. Patient households were geocoded within census tracts (CTs). Demographic, socioeconomic, and geographical data were obtained from the 2010 national census. Associations between CTs characteristics and the spatial risk of both dengue and non-dengue AFI were assessed by Poisson log-normal and conditional auto-regressive models (CAR). We identified 651 (22.0%) dengue cases among 2,962 AFI patients. Estimated risk of symptomatic dengue was 21.3 and 70.2 cases per 10,000 inhabitants in 2009 and 2010, respectively. All the four dengue serotypes were identified, but DENV2 predominated (DENV1: 8.1%; DENV2: 90.7%; DENV3: 0.4%; DENV4: 0.8%). Multivariable CAR regression analysis showed increased dengue risk in CTs with poorer inhabitants (RR: 1.02 for each percent increase in the frequency of families earning ≤1 times the minimum wage; 95% CI: 1.01-1.04), and decreased risk in CTs located farther from the health unit (RR: 0.87 for each 100 meter increase; 95% CI: 0.80-0.94). The same CTs characteristics were also associated with non-dengue AFI risk.Conclusions/SignificanceThis study highlights the large burden of symptomatic dengue on individuals living in urban slums in Brazil. Lower neighborhood socioeconomic status was independently associated with increased risk of dengue, indicating that within slum communities with high levels of absolute poverty, factors associated with the social gradient influence dengue transmission. In addition, poor geographic access to health services may be a barrier to identifying both dengue and non-dengue AFI cases. Therefore, further spatial studies should account for this potential source of bias.
ObjectiveTo analyze serum fatty acids concentrations during healthy pregnancy and evaluate whether socioeconomic, demographic, obstetric, nutritional, anthropometric and lifestyle factors are associated with their longitudinal changes.Study designA prospective cohort of 225 pregnant women was followed in the 5th–13th, 20th–26th and 30th–36th weeks of gestation. Serum samples were collected in each trimester of pregnancy and analyzed to determine the fatty acids composition using a high-throughput robotic direct methylation method coupled with fast gas-liquid chromatography. The independent variables comprised the subjects’ socioeconomic and demographic status, obstetric history, early pregnancy body mass index (BMI), dietary and lifestyle parameters. Analyses were performed using linear mixed-effects models.ResultsThe overall absolute concentrations of fatty acids increased from the 1st to the 2nd trimester and slightly increased from the 2nd to the 3rd trimester. Early pregnancy BMI, inter-partum interval and weekly fish intake were the factors associated with changes in eicosapentaenoic + docosahexaenoic acids (EPA+DHA) and total n-3 polyunsaturated fatty acids (PUFAs). Early pregnancy BMI, age and monthly per-capita income were inversely associated with the changes in the n-6/n-3 ratio. Alcohol consumption was positively associated with the n-6/n-3 ratio.ConclusionEarly pregnancy BMI was positively associated with EPA+DHA and total n-3 PUFAs, while presenting a reduced weekly fish intake and a lower inter-partum interval were associated with lower levels of n-3 PUFAs. A lower per-capita family income and a drinking habit were factors that were positively associated with a higher n-6/n-3 ratio.
O Brasil é um dos países mais afetados pela pandemia de COVID-19 e o real número de mortes pela doença torna o cenário ainda mais desafiador. O objetivo deste estudo foi estimar o excesso de mortes e suas diferenças em adultos com 20 anos e mais em Manaus (Amazonas), Fortaleza (Ceará), Rio de Janeiro e São Paulo, de acordo com o local de ocorrência do óbito, características demográficas e trajetória ao longo do tempo. Os dados foram obtidos no Sistema de Informações sobre Mortalidade e na Central de Informações do Registro Civil Nacional. As estimativas de óbitos esperados foram obtidas por meio de modelos aditivos generalizados quasi-Poisson com ajuste de sobredispersão. Entre 23 de fevereiro e 13 de junho de 2020, foram registradas 74.410 mortes naturais nas quatro cidades, com excesso de mortes de 46% (IC95%: 44-47). O maior excesso de mortes ocorreu em Manaus, 112% (IC95%: 103-121), seguido por Fortaleza, 72% (IC95%: 67-78), Rio de Janeiro, 42% (IC95%: 40-45) e São Paulo, 34% (IC95%: 32-36). O excesso de mortes foi maior nos homens e não significativo nas Semanas Epidemiológicas (SE) 9-12, exceto em São Paulo, 10% (IC95%: 6-14). Em geral, o pico de mortes excedentes ocorreu nas SE 17-20. O excesso de mortes não explicado diretamente pela COVID-19 e de mortes em domicílios/via pública foi alto, especialmente em Manaus. A elevada porcentagem de mortes excedentes, de mortes não explicadas diretamente pela COVID-19 e de mortes fora do hospital sugerem alta subnotificação de mortes por COVID-19 e reforça a extensa dispersão do SARS-CoV-2, como também a necessidade da revisão de todas as causas de mortes associadas a sintomas respiratórios pelos serviços de vigilância epidemiológica.
RESUMO:Objetivo: Os objetivos deste estudo foram analisar e descrever a presença da infraestrutura de saneamento básico nas áreas urbanas do Brasil, contrastando os perfis dos domicílios indígenas com os de não indígenas. Métodos: Trata-se de um estudo transversal com base nos microdados do Censo 2010. As análises foram baseadas em estatísticas descritivas (prevalências) e na construção de modelos de regressão logística múltipla (ajustados por covariáveis socioeconômicas e demográficas). Estimaram-se as razões de chance para a associação entre as variáveis explicativas (covariáveis) e de desfecho (abastecimento de água, esgotamento sanitário, coleta de lixo e saneamento adequado). O nível de significância estatística estabelecido foi de 5%. Resultados: Entre os serviços analisados, o esgotamento sanitário mostrou-se o mais precário. Em relação à cor ou raça, os domicílios com responsáveis indígenas apresentaram as menores frequências de presença de infraestrutura sanitária no Brasil Urbano. Os resultados das regressões ajustadas mostraram que, em geral, os domicílios indígenas se encontram em desvantagem quando comparados aos de outras categorias de cor ou raça, especialmente quanto à presença do serviço de coleta de lixo. Essas desigualdades foram de maior magnitude nas regiões Sul e Centro-Oeste. Conclusão: As análises deste estudo não somente confirmam o perfil de precárias condições de infraestrutura de saneamento básico dos domicílios indígenas em área urbana, como também evidenciam a persistência de iniquidades associadas à cor ou raça no país. Manaus, capital do estado brasileiro do Amazonas, é o atual epicentro da epidemia na Amazônia com um aumento repentino de mortes que preocupa gestores e sociedade. O objetivo do estudo foi analisar o excesso na mortalidade geral, segundo Semanas Epidemiológicas (SE), visando a identificar mudanças potencialmente associadas à epidemia em Manaus. Dados de mortalidade geral e grupos de causas foram obtidos na Central de Informações do Registro Civil Nacional e no Sistema de Informações sobre Mortalidade, para 2018, 2019 e 2020. Analisou-se faixa etária, sexo, local de ocorrência do óbito, SE, ano-calendário e causas de morte. Calcularam-se razões entre as mortes ocorridas em 2019/2018 e 2020/2019 para avaliar o excesso de mortes, com intervalos de confiança no nível de 5%. Não observou-se excesso de mortalidade geral significativo nas razões 2019/2018, independentemente da SE. Já as razões de 2020/2019 passaram de 1,0 (IC95%: 0,9-1,3) na SE 12 para 4,6 (IC95%: 3,9-5,3) na SE 17. Observou-se excesso de mortalidade geral com a progressão da idade, especialmente em indivíduos com 60 anos e mais, os quais concentraram 69,1% (IC95%: 66,8-71,4) das mortes. A razão 2020/2019 para óbitos em domicílio/via pública foi de 1,1 (IC95%: 0,7-1,8) na SE 12 e de 7,8 (IC95%: 5,4-11,2) na SE 17. A explosão da mortalidade geral em Manaus e a elevada proporção de óbitos em domicílio/via pública expõe a gravidade da epidemia em contextos de grande desigualdade social e fraca efetividade de ações governamentais, em especial aquelas voltadas ao enfrentamento das desigualdades sociais e para a garantia e fortalecimento do Sistema Único de Saúde.
BackgroundThe leprosy transmission chain is very complex and, in order to intervene in this transmission, more must be known about the factors linked to falling ill. There are doubts as to the influence of population size, population density and the disease’s magnitude in detection rate trends. This paper aimed to identify factors associated with detection of leprosy in an endemic municipality of Rio de Janeiro State, Brazil.MethodsThis ecological study in Duque de Caxias municipality, Rio de Janeiro State, Brazil, used neighbourhoods (bairros) as the unit of analysis. Selecting new cases of leprosy detected from 1998 to 2006, the analysis examined clinical, socioeconomic and service variables using a Poisson log-Normal model.ResultsIn the municipality overall, 2572 new cases were detected, a rate of 3.70 cases/10,000 inhabitants. The results describe a heterogeneous distribution of cases and rates in the municipality. The final model displayed a significant association with indeterminate clinical form (β = 2.599), proportion of homes with running water (β = -2.334) and presence of a decentralised health care unit (β = 0.524).ConclusionAlthough the results indicate progress towards elimination of the disease in the municipality, high rates continue to be detected in municipal sub-regions. The following question can thus be posed: over how wide a geographical area could the disease be thoroughly eliminated, given this heterogeneity within a small municipality?
