A new field of biomédical science, known as tissue engineering, applies the principles of biology and engineering to the development of functional substitutes for tissues and organs. Little is known about the influence of Keratinised Mucosa dimension around implants on the soft and hard tissue health The purpose of this case report is to understand the importance of healthy tissue around the implants for long term success and how biomimetic materials like PRF Membrane can be incorporated in treatment plan as an important modality in selected cases.
Maxillary defects, whether congenital or acquired, make a patient encounter an array of physical and psychological difficulties, leading to an extremely poor quality of life. Rehabilitation of such a patient is often challenging due to the extent of the defect area coupled with the absence of adequate retention caused by the size and weight of the prosthesis. Further, providing a proper seal of the oronasal communication is of utmost importance to restore function. Taking the above factors into consideration, a combination of hollow-bulb obturator consisting of a titanium framework and a flexible, snap-on silicone cap is an effective prosthesis providing a long-term treatment, increased retention, and a happy patient. The fabrication protocol included the use of computer-aided design, titanium along with laser welding, and an intraoral trial before final fabrication, hence, reconfirming the success of the prosthetic design. The maxillary obturator presented in this article eliminates several disadvantages associated with a conventional hollow-bulb obturator, thereby providing a novel, viable option for a maxillofacial prosthodontist.
ABSTRACT-Initially when dental implants were first introduced their success was assumed to be dependent mostly on the surgical technique and later their placement. However, without a regular program of clinical reevaluation, plaque control, oral hygiene instruction, and reassessment of biomechanical factors, the benefits of treatment often are lost and inflammatory disease in the form of recurrent periodontitis or peri-implantitis may result. Maintenance of the periodontal health is a critical factor in the long-term success of dental implant therapy. This article reviews the goals, types, and appropriate frequency of periodontal maintenance in dental implant therapy, as well as the incidence and etiology of peri-implant disease and strategies for management when recurrent disease develops during the maintenance phase of treatment.
Complete oral rehabilitation in patients with severely worn dentition is challenging due to the loss of occlusal vertical dimension, loss of tooth structure, uneven wear of teeth creating an uneven plane of occlusion, and parafunctional habits. The severe wear of anterior teeth facilitates the loss of anterior guidance, which protects the posterior teeth from wear during excursive movement. The collapse of posterior prosthesis teeth also results in the loss of the normal occlusal plane and the reduction of the vertical dimension. This case report describes 56-year-old female, who had the loss of anterior guidance, the severe wear of dentition, and the reduction of the vertical dimension. Occlusal overlay splint was used after the decision of increasing vertical dimension by anatomical landmark, facial and physiologic measurement. Once the compatibility of the new vertical dimension had been confirmed, interim fixed restoration and the permanent reconstruction was initiated. This case reports that a satisfactory clinical result was achieved by restoring the vertical dimension with an improvement in aesthetics and function.
The success of complete denture is enhanced by achieving the objectives of impression making, that is preservation of underlying tissue, retention, stability, support and esthetics. Flabby tissue is a common clinical finding affecting the alveolar ridges of both mandibular or maxillary arches. The use of conventional impression techniques for recording flabby tissues can lead to unstable and unsatisfactory denture. But some modification in the impression techniques can record the flabby tissues in an undistorted form and thus help to fabricate a stable denture. The problem with conventional denture is rigidity of material which comes in contact with tissue surface, leading to uneven distribution of load. A liquid supported denture allows uniform stress distribution due to its flexible tissue surface and provides improved patient's acceptance.
