Purpose Given the complexity and difficulties of the health sector, the question that this study attempts to answer is as follows: what are the main results of studies on business process management (BPM) and lean in the health sector? The purpose of this paper is to analyze the results of studies that address the promotion of BPM and lean in the health sector. Design/methodology/approach To conduct a survey of published studies using the BPM and lean approach in healthcare, a search was performed in the Scopus, Web of Science, and PubMed databases in two steps. The first step consisted of the collection and analysis of data related to the BPM approach in the health sector, based on a survey of published articles on this topic. The second step consisted of the collection and analysis of data related to the use of lean in the same sector, based on the survey of systematic reviews published in major databases. Findings The main results of the studies that address the promotion of BPM and lean in the health sector are as follows: a reduced length of hospitalization, increased patient satisfaction, increased patient efficiency and safety, a reduction in the time of notification of infection, help with organizational change, understanding the end-to-end process, increased motivation, understanding the importance of some handoffs for patient safety, improvements in quality indicators in the emergency department, and improvements in the organization’s focus on high-level processes. By contrast, a single study showed difficulties in promoting management focused on processes in which decisions are unique and difficult to model. Originality/value The results may help in the identification of research gaps in the promotion of BPM and lean healthcare and in the development of relevant research. In addition, by understanding how health organizations are promoting management focused on processes and the results obtained with this approach, managers from other organizations, especially in this sector, can reflect and develop similar actions that seek to improve the quality of services offered, increase productivity and customer satisfaction, and reduce costs, errors, and waiting times.
O centro cirúrgico, por suas particularidades e características, constitui uma das unidades mais complexas do ambiente hospitalar. Assim, percebe-se que o bom desempenho de um centro cirúrgico está diretamente relacionado com a qualidade de seus próprios processos e com os processos dos serviços que o apoiam. Em Julho de 2012, com a portaria 194 – HCRP (5), é estabelecido as Diretrizes Gerais de Funcionamento do Bloco Cirúrgico do HC campus. O objetivo do presente estudo é avaliar o impacto do novo modelo gerencial do Bloco Cirúrgico do Campus implantado. Trata-se de séria histórica descritiva e retrospectiva com análise de alguns indicadores comparando a nova gestão com a fase prévia. Nota-se melhora progressiva em alguns indicadores analisados, alcançando metas estipuladas pelo pró- prio Planejamento estratégico vigente. O presente estudo pode ilustrar o impacto de um modelo gerencial dentro do Centro Cirúrgico de um Hospital Escola Terciário de grande porte, mostrando os avanços alcançados e os pontos com oportunidades de melhoria.
RESUMOIntrodução: o absenteísmo dos usuários de sistemas de saúde representa um grande problema para as organizações. O objetivo deste estudo foi identificar as razões para o absenteísmo dos pacientes em consulta médicas. Metodologia: pesquisa de campo nos ambulatórios de oncologia do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto. Os pacientes que não compareceram à primeira consulta em 2013 (356) foram contatados por meio telefônico, dos quais 299 não foi possível o contato por apresentarem telefone inválido, desligado, fora da área de cobertura ou por não atenderem a ligação. Resultados: 57 pacientes foram entrevistados e apontaram os motivos para falta. As respostas foram categorizadas. Os resultados mostraram que 27 pacientes (47%) não compareceram à consulta por motivos relacionados ao indivíduo ou comportamento, sendo "consultei-me em outro lugar" o mais recorrente (12,30%), seguido de "consulta coincidiu com outro compromisso" (7,02%) e "não me lembro o motivo" (7,02%). Individualmente, o motivo mais citado pelos pacientes foi "eu não faltei a consulta" (31,6%), o que sugere problemas de comunicação. Conclusão: as principais razões para o absenteísmo estão relacionadas ao indivíduo e que problemas de cadastro do paciente e comunicação dificultam a compreensão das causas do absenteísmo. ABSTRACT Introduction: absenteeism of health system users is a major problem for organizations. The study aimed to identify the factors of absenteeism in outpatients at the General Hospital of the Medical School of Ribeirão Preto oncology clinics. Methodology: patients who did not attend the first appointment in 2013 (356) were contacted by telephone, of which 299 were not able to contact because they presented Medicina (Ribeirão Preto. Individually, the most cited reason by the patients was "I did not miss the appointment" 18 (31.6%), thus suggesting communication problems. Conclusion:the main reasons for absenteeism are related to the individual and problems of patient registration and communication make it difficult to understand the causes of absenteeism.
RESUMO Este estudo objetiva identificar informações a serem utilizadas para analisar o desempenho do acesso à atenção secundária à saúde. Foram analisados os acessos à especialidade de cirurgia digestiva do Hospital Estadual de Ribeirão Preto (HERP) e do Hospital Estadual de Américo Brasiliense (HEAB). Foram entrevistados 31 atores, em 16 organizações de saúde, com o objetivo de identificar indicadores de desempenho. Os indicadores identificados para ambos foram: 'distribuição de vagas por município', 'situação após a primeira consulta' e 'Pedidos de Interconsulta (PI)'. No acesso ao HERP, acrescentou-se o 'tempo para triagem'. Os dados para análise foram extraídos do Sistema de Apoio a Regulação Assistencial (Sara), utilizado somente no acesso ao HERP; e o sistema do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto (módulo Athos), utilizado em ambos. Essas informações podem ser úteis para caracterizar os processos, compará-los e auxiliar na identificação de oportunidades de melhoria, tornando-os mais transparentes aos envolvidos. O acesso eficiente permite o uso racional de recursos e a ampliação do acesso dos pacientes aos serviços, uma vez que possibilita redução de desperdício, ociosidade e tempo de espera.PALAVRAS-CHAVE Eficiência. Sistema Único de Saúde.ABSTRACT This study aims to identify information to be used in order to analyze the performance of access to secondary health care. The accesses to the specialty of digestive surgery of the Ribeirão Preto State Hospital (HERP) and the Américo Brasiliense State Hospital (HEAB) were analyzed. Thirty-one actors, in 16 health organizations, were interviewed, with the purpose of identifying performance indicators. The indicators identified for both were: 'distribution of vacancies by municipality', 'situation after the first consultation' and 'requests for consultation (PI)'. In access to HERP, the 'time for screening' was added. The data for analysis were extracted from the Assistance Regulation Support System (Sara), used only to access the HERP; and the system of the Clinical Hospital of Ribeirão Preto Medical School (Athos module), used in both. This information can be useful for characterizing processes, comparing them and assist in the identification of improvement opportunities by making them more transparent to those involved.
