This review presents information from several studies that have demonstrated the antiviral activity of extracts ( Andrographis paniculata, Artemisia annua, Artemisia afra, Cannabis sativa, Curcuma longa, Echinacea purpurea, Olea europaea, Piper nigrum, and Punica granatum) and phytocompounds derived from medicinal plants (artemisinins, glycyrrhizin, and phenolic compounds) against SARS-CoV-2. A brief background of the plant products studied, the methodology used to evaluate the antiviral activity, the main findings from the research, and the possible mechanisms of action are presented. These plant products have been shown to impede the adsorption of SARS-CoV-2 to the host cell, and prevent multiplication of the virus post its entry into the host cell. In addition to antiviral activity, the plant products have also been demonstrated to exert an immunomodulatory effect by controlling the excessive release of cytokines, which is commonly associated with SARS-CoV-2 infections.
Introdução: Sarcoidose é uma doença granulomatosa multissistêmica com envolvimento predominantemente pulmonar e evolução clínica variável. Apesar de sua etiologia desconhecida, pesquisas indicam que predisposição genética, quadros infecciosos e fatores ambientais contribuem para desenvolver resposta autoimune na sarcoidose. O Sistema Nervoso é afetado em 3–10% dos casos, quadro denominado neurossarcoidose. As principais características da neurossarcoidose são neuropatia do nervo facial, neurite óptica, meningite, lesões parenquimatosas, hipofisite, neuropatia periférica e mielopatia. Objetivo: O objetivo deste estudo foi revisar a importância e utilização da biópsia no diagnóstico da neurossarcoidose. Material e métodos: Primeiramente, o site Descritores em Ciências da Saúde foi utilizado para definir os descritores de pesquisa. Posteriormente, os descritores “Sarcoidosis”, “Nervous System” e “Biopsy” foram pesquisados nas plataformas PubMed e Scielo. A busca incluiu artigos de revisão sistemática, ensaios clínicos randomizados e meta-análises publicados até dez anos atrás. Resultados: Foram encontrados 5 artigos elegíveis, todos revisões sistemáticas. Dois estudos não correlacionaram nenhum achado especificamente à neurossarcoidose. Outro artigo destacou um caso de neurossarcoidose diagnosticada por biópsia hepática percutânea guiada por ultrassom, revelando fibrose, granuloma não necrosante, células gigantes polinucleadas e corpos asteroides. Em outra revisão, a biópsia de pacientes com a neurossarcoidose definitiva revelou granulomas não caseosos no Sistema Nervoso Central (SNC). Ademais, esse artigo traz um protocolo de atendimento no qual a biópsia é indicada em todas as suspeitas de neurossarcoidose, exceto quando há, simultaneamente, hidrocefalia e diagnóstico prévio de sarcoidose, situação na qual o tratamento isolado é resolutivo. Outra revisão tratou sobre paquimeningite hipertrófica, processo inflamatório da dura-máter que pode ser uma manifestação da neurossarcoidose. Segundo o estudo, a biópsia dessas lesões pode revelar o processo primário da doença e, se restrito ao SNC, a biópsia meníngea é necessária para um diagnóstico final. É imperativo realizar análise patológica convencional e imunohistoquímica com marcadores para linfócitos, macrófagos e IgG4. O estudo destaca, também, que até a biópsia negativa é importante pois, quando negativa, indica paquimeningite hipertrófica idiopática. Conclusão: A biópsia constitui relevante método diagnóstico para neurossarcoidose, revelando, por exemplo, tal patologia como causa da paquimeningite hipertrófica. Um achado importante da biópsia para neurossarcoidose são granulomas não caseosos no SNC.
Introdução: A epilepsia, doença que afeta aproximadamente 50 milhões de pessoas no mundo, é caracterizada pela ocorrência imprevisível de convulsões, geradas por disparos de neurônios. As crises ocorrem pela perda de neurônios gabaérgicos ou hiperexcitação dos glutamatérgicos, o que gera hiperexcitabilidade do sistema nervoso central, determinando manifestações clínicas espectrais. A doença é manifestação de diversas outras, como encefalite e malformação arteriovenosa. Na histopatologia, costuma-se encontrar: displasia cortical, com lesões microscópicas diversas; esclerose hipocampal, com perda segmentar de neurônios e gliose; e encefalite de Rasmussen, com inflamação crônica. Apesar de ser utilizada em poucos casos, é preciso esclarecer o papel da biópsia na epilepsia. Objetivo: Analisar o papel da biópsia na epilepsia por meio de uma revisão sistemática da literatura disponível. Material e métodos: No site Descritores em Ciências da Saúde (DeCS) foram definidos os descritores "Epilepsy" e “Biopsy”. No PubMed e Scielo foi realizada uma pesquisa com tais descritores associados. Foram selecionados apenas ensaios clínicos randomizados, revisões sistemáticas e metaanálises. Não foi estabelecido critérios quanto ao ano de publicação. Os resultados foram analisados qualitativamente quanto às possíveis aplicações da biópsia no contexto da epilepsia. Resultados: Foram encontrados 11 artigos, dos quais 7 artigos foram elegíveis, sendo 3 ensaios clínicos, 3 revisões e 1 meta-análise. Dos excluídos, um era levantamento, outro estudo transversal, outro não randomizado e outro não detalhou metodologia. Apesar de correlacionarem biópsia e craniotomia e citarem biópsia para neoplasias do sistema nervoso central, nenhuma associou o procedimento à epilepsia. Um ensaio clínico citou biópsia como forma de diagnóstico de neurocisticercose, também sem associar à epilepsia. Outro ensaio avaliou o diagnóstico para lesões cerebrais por biópsia estereotáxica guiada por espectroscopia por Ressonância Nuclear Magnética, apesar de não associar esses meios de diagnóstico à epilepsia. A meta-análise indicou que três dos pacientes avaliados que possuíam glioma angiocêntrico foram submetidos à biópsia e não apresentaram recorrência de convulsões. Conclusão: Há poucos estudos sobre a aplicação da biópsia na epilepsia. Porém, a literatura disponível já demonstra a importância da biópsia para doenças que cursam com epilepsia, tais como lesões cerebrais e glioma angiocêntrico.
