Objective: to evaluate and compare the sociodemographic characteristics, depressive symptoms, anxiety and perceived stress of formal and informal caregivers of elderly people with Alzheimer's disease. Method: Quantitative, cross-sectional and comparative study with 44 caregivers that were divided into two groups of 26 informal caregivers (IC) and 18 formal caregivers (FC). The Instrument for Characterization of the Caregiver, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI) and Perceived Stress Scale (PSS) were applied. Results: Of those IC, the majority were women (96.2%), mean age of 52.9 years, majority represented by sons (daughters) (65.4%). They presented, on average, depressive symptoms (10.1%), anxiety symptoms (11.5%) and scored 32.1 in the PSS. The FC group, the majority were women (94.4%), mean age of 45.2 years were not related to the elderly people (66.7%). They presented, on average, depressive symptoms (7.1%), anxiety symptoms (6.4%) and scored 31.7 in the PSS. Conclusion: Similarities were shown in the sociodemographic aspects, but the groups differ in the health profile, revealing an alert for the planning of interventions looking for health promotion and disease prevention. Implications for practice: The study contributes to the improvement of the caregivers´ quality of life.
Objective To evaluate the burden and psychological symptoms of informal caregivers of the elderly during the COVID-19 pandemic. Method Cross-sectional study with 50 caregivers from the Gerontology Outpatient Clinic in the interior of São Paulo, evaluated in 2021 via teleconsultation by the Hospital Anxiety and Depression Scale and the Zarit-Brief Burden Interview. Poisson’s multivariate regression was applied to the two instruments scores according to the set of characteristics of the participants. Results Psychological symptoms were highlighted in caregivers with longer time in exercise (p=0.01; p=0.001) and who lived in the same environment with the elderly (p=0.04; p=0.02). Burden was associated with age (p<0.001) and living with the elderly (p=0.001). Conclusion There is a need for interventions that attenuate psychological symptoms and burden in older caregivers, who live with the elderly and have been working for a longer time.
Background: Multiple sclerosis (MS) and Alzheimer disease (AD) are neurodegenerative diseases with distinct pathophysiology but similar burdens. Early cognitive impairment in MS is unusual but, due to disease modifying therapies (DMT) advancements and bigger life expectancy, its coexistence with AD has become more common. Objective: To report a coexistence case of MS and AD. Methods: Retrospective case report and literature review. Results: 58-year-old patient presented with work issues, behavioral changes, gait disturbance and unbalance. It was disclosed an impairment of attention span, multitasking, executive dysfunction, and loss of memory for recent events. A PET/CT showed hypometabolism in frontal lobes and CSF analysis disclosed oligoclonal bands and increased TAU protein levels. The patient was initially treated with donepezil, with poor response. A later brain MRI showed typical demyelinating MS lesions. The patient was treated with Natalizumab due to high lesion load and functional impairment. Conclusion: As MS patients live longer, it’s important to recognize age-related comorbidities such as AD. In our patient a poor relapse perception contributed for a late MS diagnosis. The evaluation with PET/CT and increased TAU levels in CSF highly suggests a coexistence with AD. There are no peer-reviewed studies regarding coexisting MS and AD. Further research is necessary to better understand the clinical, demographic, and neuropathological features of the coexistence of both diseases.
RESUMO Objetivo Avaliar sobrecarga e sintomas psicológicos dos cuidadores informais de idosos durante a pandemia da COVID-19. Método Estudo transversal com 50 cuidadores do Ambulatório de Gerontologia do interior de São Paulo, avaliados em 2021 via teleconsulta pela Escala Hospitalar de Ansiedade e Depressão e Entrevista de Sobrecarga de Zarit. A Regressão multivariada de Poisson foi aplicada nos escores dos dois instrumentos em função do conjunto de características dos participantes. Resultados Destacaram-se os sintomas psicológicos nos cuidadores com maior tempo em exercício (p=0,01; p=0,001) e que viviam no mesmo ambiente que o idoso (p=0,04; p=0,02). A sobrecarga associou-se com sua idade (p<0,001) e morar junto ao idoso (p=0,001). Conclusão Urgem intervenções atenuantes dos sintomas psicológicos e de sobrecarga nos cuidadores com idade avançada, que residem junto ao idoso e desempenham a função há maior tempo.

Método: em 2021, foram avaliados 50 pares de cuidadores informais e idosos pelo perfil sociodemográfico, de saúde, Escala Hospitalar de Ansiedade e Depressão (HADS), Entrevista de Sobrecarga de Zarit-Brief (ZBI-12) e informações sobre grau de dependência dos idosos para atividades básicas e instrumentais (ABVD e AIVD). Modelos de regressão avaliaram os escores ZBI-12 e HADS em função das características dos cuidadores e idosos assistidos. O protocolo de pesquisa foi aprovado pelo Comitê de Ética em Pesquisa. Resultados: alta morbidade e dependência para ABVD foram significativamente associadas a maiores níveis de sobrecarga do cuidador. Essas características perderam significância quando ajustado o modelo por idade do cuidador, tempo que presta o cuidado e convivência com o idoso. Conclusão: esses resultados revelam a necessidade de intervenções para garantir suporte aos cuidadores quanto a diminuição da sobrecarga relacionada ao cuidado.ABSTRACTObjective: to analyze the association between the characteristics of elderly people and caregivers and their impact on caregiver burden. These characteristics lost significance when we adjusted the model for the caregiver's age, time that provides care, and living with the elderly. Modelos de regresión evaluaron las puntuaciones de ZBI-12 y HADS en función de las características de los cuidadores y ancianos asistidos. El Comité de Ética en Investigación aprobó protocolo de investigación. Resultados: la alta morbilidad y la dependencia de BADL se asociaron significativamente con niveles más altos de sobrecarga del cuidador. Estas características perdieron su importancia cuando ajustamos el modelo por la edad del cuidador, tiempo que proporciona cuidados y convivencia con los ancianos. Conclusión: estos resultados revelan la necesidad de intervenciones para garantizar apoyo a los cuidadores para reducir la sobrecarga relacionada con el cuidado.
Resumo Introdução As intervenções cognitivas podem ser importantes estratégias de apoio aos cuidadores de idosos com doença de Alzheimer (DA), uma vez que melhoram sua saúde física e mental. Objetivo Avaliar e analisar os efeitos de intervenção psicoeducativas com enfoque em treino cognitivo sobre a cognição, ansiedade, estresse e sobrecarga em cuidadores de idosos com doença de Alzheimer. Método Trata-se de um ensaio clínico randomizado, controlado e cego realizado com 37 cuidadores informais divididos em Grupo Intervenção Domiciliar (GID) e Grupo Controle (GC). Os participantes foram avaliados antes e após a intervenção por meio do Addenbrooke’s Cognitive Examination-Revised, Escala de Estresse Percebido, Inventário de Depressão e Ansiedade de Beck e Escala de Sobrecarga de Zarit. Resultados Houve melhora significante no GID para sintomas de estresse (p= 0,0027), cognição (p= 0,003; p= 0,008), atenção/orientação (p= 0,004), memória (p= 0,017) e fluência verbal (p= 0,023). Quanto ao GC, apesar da melhora nos parâmetros, essa não foi significativa. Conclusão: A intervenção em cuidadores de forma domiciliar é uma importante ferramenta para a melhora na cognição e estresse em cuidadores de idosos com DA, mas não parece ter a mesma eficácia para sintomas de sobrecarga, o que evidencia a necessidade de estratégias específicas para este domínio.
