Introducción y objetivos: la cirugía antirreflujo realizada por un cirujano experto es una opción para el tratamiento de mantenimiento para el paciente con enfermedad por reflujo gastroesofágico (ERGE) bien documentada. La "buena documentación" de la ERGE es difícil de llevar a cabo, dado que se trata de una entidad multifactorial, en la que la unión gastroesofágica, con sus condiciones tan especiales tanto anatómicas como funcionales, es determinante. Para evaluar preoperatoriamente tanto el diagnóstico como la indicación quirúrgica de los pacientes que han sido intervenidos mediante un procedimiento laparoscópico antirreflujo en nuestro centro, hemos estudiado retrospectivamente sus historiales clínicos. Métodos: se analizaron retrospectivamente los historiales clínicos de los pacientes de nuestra Área Sanitaria sometidos a un procedimiento laparoscópico antirreflujo desde diciembre de 1997 hasta febrero de 2007. Para evaluar el aseguramiento preoperatorio en el diagnóstico y la indicación quirúrgica, se tomaron los datos epidemiológicos, la sintomatología, las pruebas diagnósticas realizadas, tanto morfológicas como funcionales, la terapéutica médica pautada y la indicación quirúrgica recogida en la historia clínica de cada uno de los pacientes. La información obtenida fue analizada estadísticamente mediante los test bivariantes. Las diferencias obtenidas fueron significativas cuando el valor de p resultó igual o menor a 0,05. Resultados: se estudiaron 100 pacientes (50% mujeres, con una edad media de 51,31 ± 13,53 años) intervenidos mediante cirugía antirreflujo por vía laparoscópica. Su sintomatología previa duró 56,47 ± 61,33 meses. El 95% de los pacientes presentó una anormalidad anatómica. La monitorización del pH diagnosticó a 3 cuartas partes de ellos. Y la indicación quirúrgica más frecuente resultó la recurrencia o persistencia de la esofagitis a pesar de un adecuado tratamiento médico (52 casos). Conclusiones: según los datos recogidos retrospectivamente en sus historiales clínicos, nuestros pacientes tuvieron un diagnóstico preoperatorio bien documentado (incluyendo tanto el reflujo no erosivo como el erosivo) en el 100% de los casos y una indicación quirúrgica sólidamente fundamentada en el 94% de ellos.
Presentamos una evolución favorable tras reposición intestinal en una paciente con ictericia obstructiva causada por una obstrucción del asa aferente del procedimiento Billroth-II.Desde que en 1979 se registró el primer caso (1), la ictericia obstructiva secundaria a la obstrucción del asa aferente es una entidad extremadamente rara y tardía del procedimiento reconstructivo Billroth-II.De los casos registrados en la literatura, en alguna ocasión una pancreatitis aguda ha sido diagnosticada como la causa de la ictericia obstructiva (2).Nuestra paciente es una mujer de 83 años de edad a la que se le practicó una reconstrucción del tipo Billroth-II hace 50 años, que presentó un debut clínico semejando una colecistopancreatitis aguda, y en la que intraoperatoriamente se demostró una obstrucción del asa aferente como la causa de su ictericia obstructiva.
Hemos leído con atención el artículo titulado "Prolapso anal de un lipoma de colon sigmoide" (1). El caso es muy peculiar e interesante, por lo que felicitamos a sus autores.Ha traído cierta polémica en el Servicio, pues unos estaban a favor de la actuación y otros no. Le paso a comentar las conclusiones.Como bien se indica en la discusión del mismo artículo, se debe buscar "siempre realizar la intervención menos agresiva e intentando la resección local" (1,2).Pues bien, dado que se realizó una sigmoidectomía, suponemos que laparoscópica aunque no se especifica, nos preguntamos si se valoró la realización de una polipectomía endoscópi-ca, ya que el lipoma permitía el paso del endoscopio. Suponemos que sí, y que se desestimó por la base de implantación ancha que muestra la fotografía del artículo, aunque no se especifica.Lo que ya no nos atrevemos a valorar es si se planteó realizar una exéresis transanal mediante sección y ligadura simple del cuello del "pólipo", dado que, según se explica "el diagnós-tico de lipoma de colon sigmoide prolapsante a través del ano" así parece permitirlo.Ante una tumoración benigna y aislada, con posibilidad de acceso transanal, nos parece la opción menos agresiva, a la vista de la información suministrada en el artículo.Felicitándole por la revista y sin otro particular, reciba un cordial saludo.
Presentamos un caso de neumoperitoneo con gas intramural en un segmento entérico, de etiología incierta, con resolución espontánea sin tratamiento quirúrgico, donde las imágenes de la tomografía computerizada fueron determinantes en la decisión de la actitud terapéutica.
