For a long time, the endothelial covering of the vessels has been considered an inert surface. On the contrary, the endothelial cells are active and dynamic elements in the interaction between blood and tissues. The control of the vessel basal tone is obtained by the complex balance between the relaxing and contracting endothelial factors. Previous clinical studies show that patients suffering from rheumatoid arthritis and other autoimmune rheumatologic pathologies are at high risk of death being prematurely affected by atherosclerosis and cardiovascular diseases. Blocking tumor necrosis factor (TNF)-α by biological drugs improves the endothelial function. The aim of our study was to evaluate the effects of two anti-TNF-α drugs (infliximab and etanercept) on the endothelial function by evaluating the flow-mediated dilatation (FMD), which was measured in the brachial artery before and after treatment and after 8-12 weeks. We enrolled 36 patients (average age 52 ± 9.8 years, 12 men and 24 women), 25 of them were affected by rheumatoid arthritis (RA) and 11 were affected by psoriatic arthritis (PsA) and they were divided into three groups: 10 patients were treated with etanercept, 13 patients were treated with infliximab, 13 patients were treated with DMARDs. We measured the common carotid intimal-medial thickness (ccIMT) and the endothelial function was evaluated by FMD measurement in the brachial artery, before treatment, 1 h after the beginning of treatment and after 8-12 weeks. No statistically significant difference between the three groups was found for the ultrasonographic evaluation of the carotid IMT. On the contrary, the differences between FMD values before and after the treatment in the patients treated with etanercept (13.1 ± 0.01 vs. 18.8 ± 0.01%, p < 0.01) and in the patients treated with infliximab (11.8 ± 0.09 vs. 16.7 ± 0.09%, p < 0.01) were statistically significant. Long-term evaluation for infliximab and etanercept was performed by comparing the FMD values, respectively, 8 and 12 weeks after the first treatment. After 8 weeks, FMD value was similar to the value recorded at enrollment in the infliximab group (11.9 ± 0.03 vs. 13.54 ± 0.04%, p = 0.236) and the FMD values in the etanercept group after 12 weeks showed a not statistically significant reduction of vasodilatating effect (13.01 ± 0.03 vs. 15.67 ± 0.02%, p = 0.197). In conclusion, the use of biological drugs in patients affected by autoimmune arthritis can modify the endothelial function, as indicated by the induced FMD changes, but the long-term effect tends to be considerably reduced.
Interstitial lung diseases (ILDs) are inflammatory diseases characterized by slow and progressive destruction of alveolar-capillary functional units, often leading to respiratory failure and death. A first stage of alveolitis and a following stage of fibrosis provoke an anatomical distortion of the peripheral airways and the interstitium, and for their smoldering evolution and non-specificity of symptoms ILDs may remain undiagnosed and untreated for a long time. In this review we exploited the immunopathogenetic aspects and the therapeutical approaches to this frequently unrecognized and severe disease. DefinitionInterstitial lung diseases (lLDs) are inflammatory diseases characterized by alveolar and lung interstitium alterations, which cause progressive scarring of lung tissue followed by a change in peripheral airways and lung blood vessels, often evolving into respiratory failure. The incidence in the general population is about 80 cases out of 100,000 inhabitants, for men, and 70 cases out of 100,000 inhabitants, for women. AnatomyThe damage to lung parenchyma is caused by activated inflammatory cells accumulated in the alveolar structures. The low respiratory tract is made up of alveoli (cluster emanations of ending bronchioles) and vascular network (arterioles, capillaries and lung venules) (Fig. la). Introdução: A piometra consiste em um processo inflamatório, com evolução associada a infecções bacterianas, no útero e órgãos do sistema genital, com maior incidência em nulíparas e em animais maiores de 4 anos. Essa, por sua vez, tem apresentado grande relevância na clínica de pequenos animais, uma vez que, sua ocorrência é cada vez mais frequente. Há duas formas pelas quais ela pode ser expressa, como aberta, sendo caracterizada por secreções pelo canal vaginal e como fechada, a forma mais grave, definida por distensão abdominal, sendo essa de fácil palpação. Objetivo: Descrever os achados laboratoriais e de imagem de uma cadela Shih-tzu, idosa, com piometra aberta. Relato de caso: Relata-se a descrição dos achados laboratoriais e de imagem de uma cadela shih-tzu, de 9 anos, pesando 7,4kg atendida na clínica Veterinária São Francisco de Assis. Durante a anamnese a tutora relatou que o animal havia entrado no cio a cerca de 30 dias. No exame físico foi possível constatar secreção vaginal e não foi detectado aumento de volume uterino através da palpação, além disso, os parâmetros vitais estavam dentro da normalidade. Foi solicitado um hemograma com pesquisa de hematozoário, um bioquímico (alanina aminotransferase, aspartato aminotransferase, colesterol, creatinina, fosfatase alcalina, glicose, triglicerídeos e uréia) e uma ultrassonografia. Resultados: No hemograma, constatou-se uma anemia macrocítica hipocrômica, acompanhada de uma hiperproteinemia, uma linfocitopenia, uma eosinopenia e uma monocitose relativa. Não houve presença de hematozoário. Ademais, o bioquímico não apresentou nenhuma alteração. A partir do exame ultrassonográfico, observou-se a presença de conteúdo anecóico na luz do útero, paredes espessadas e ecogênicas caracterizando assim, juntamente com o exame físico, um quadro sugestivo de piometra aberta, na qual foi recomendada a realização de ovariosalpingehisterectomia. Conclusão: A intervenção cirúrgica foi realizada com sucesso em outra clínica veterinária. Nessa, foi observada o ligeiro aumento do útero e a presença de secreção purulenta confirmando a suspeita da piometra. O animal se recuperou com êxito.
Introdução: Endometrite é o processo inflamatório do endométrio. Quando de ocorrência não infecciosa, mas induzida pelo contato do útero com o sêmen, é denominada de endometrite persistente pós cobertura (EPPC). Em éguas, a lavagem uterina tem se mostrado um método alternativo na prevenção e tratamento da EPPC.Objetivo: Levantar informações relevantes na literatura sobre o desenvolvimento da EPPC e os benefícios da lavagem uterina em animais susceptíveis.Materiais e métodos: A realização da pesquisa foi desenvolvida com base em literaturas e em plataformas científicas como Pubvet®, SciELO e Google Acadêmico (Scholar), utilizando os seguintes descritores: Endometrites, aborto em éguas, resposta inflamatória uterina e tratamento/prevenção de endometrites. Resultados: A EPPC é uma afecção induzida pelo contato do sêmen no endométrio. Em condições fisiológicas a inflamação endometrial é necessária, pois elimina resíduos seminais, como espermatozoides mortos ou defeituosos, e o excesso de plasma seminal. Quando os mecanismos de defesa do útero da égua são eficientes e conseguem eliminar o processo em até 48 horas, a égua é considerada resistente, quando os mecanismos de defesa falham, o animal passa a ser susceptível, tornando o quadro fisiológico um processo patológico, acarretando infertilidade e aborto, pois o aumento das prostaglandinas induz a luteólise, tornando a gestação inviável. Sendo os espermatozoides, e os seus produtos, os causadores de infiltrado neutrofílico no interior do útero, os tratamentos objetivam diminuir os problemas dos mecanismos de defesa. A lavagem uterina é uma terapia efetiva, pois promove a lavagem física do útero, eliminando os agentes causadores e diminuindo indiretamente o processo inflamatório. Esse método deve ser realizado após 6 horas a fertilização e sem ultrapassar 12 horas, após esse período a chance de sucesso é menor. A lavagem pode ser feita com solução fisiológica ou solução ozonizada, estudos apontam que a última apresenta maior eficiência em amenizar o processo inflamatório, pois o ozônio tem efeito oxidativo. Deve-se repetir esse procedimento por três vezes ou até que o líquido esteja límpido. Conclusão: Considerando a importância da reprodução equina, é possível ressaltar a lavagem uterina como tratamento eficaz na prevenção e tratamento da EPPC.
