Surgeons should be aware of the re-increase in anterior capsular volume or restretching trait of the anterior capsule over time, even after successful arthroscopic Bankart repair and capsular shift. In this study, women, elite athletes, and those with frequent dislocations were at high risk of capsular restretching. An increase in capsular volume was related to redislocation and positive apprehension sign as well as with Rowe score.
Mucormycosis is an opportunistic fungal infection primarily caused by fungi belongs to class Zygomycetes, order Mucorales and family Mucoracae. It is common in patient with diabetes mellitus (DM), systemic immunocompromise and iron overload. Here by we present a case report of Rhino-Orbito Cerebral Mucormycosis which is fatal if undiagnosed and unterated in correct time and way.
The goal of this study is to find a cure for the masticatory muscle disease known as hemimasticatory spasm (HMS). This retrospective investigation intends to find a more efficient therapeutic approach for HMS patients by examining the clinical effectiveness of masseteric nerve avulsion performed on HMS using a temporomandibular arthroscope. A clinical study was piloted where the subjects were treated for masseter nerve avulsion by TMJ arthroscopy. The follow-up was done till 2 years, and the subjects were evaluated for the various characteristics like difficulties, masticatory abilities, and various sounds at the joint for the various functions of the joint. The diagnosis was done using the electrophysiological electromyogram (EMG) at the designated time intervals. There was a complete remission in all the subjects. The masseter nerve avulsion was effective since the scores lowered. Within 3 years of the operation, electrophysiological EMG depicted no discharge potential with a high frequency, and the total efficiency when paired with the clinical effectiveness was deemed acceptable. The maximal masseter power between the treated and nontreated sides was comparable. The mastication, on comparing, was also as effective as the normal side; however, lower mastication was noted in the first year. The avulsed nerve tissues lacked any apparent demyelination. Masseteric nerve avulsion with temporomandibular arthroscope assistance provided acceptable and stable total effectiveness for the intervention of the hemimasticatory spasm. While the strength of the muscle of the afflicted side was only marginally reduced, its masticatory effectiveness was optimally preserved.
Aim: The purpose of the present research was to assess the preference as well as grading of maxillectomies in the Mucormycosis cases by operating surgeons. Methodology: An observational study conducted among 43 cases of confirmed CAM which included 35 males and 8 females, with an average age of 56 years. The surgical approach adopted in our cases included endoscopic surgical debridement with Denker’s approach including mandatory pterygopalatine and infratemporal fossa exploration. All cases were intraoperatively scored using our designed intraoperative scoring assessment tool for mucormycosis. Postoperatively patient recovery was assessed using C reactive protein levels and weekly imaging. Results: Although an early observation in the post op period we observed higher mortality among cases reporting with high scores as per our intraoperative reporting system. At the end of 2 months of completed treatment we report 6 cases of mortality among whom 5 cases were found to have scores (>25) and one reported with a score of 18. Conclusion: This assessment helped to grade the disease severity and also gives an insight about the postoperative prognosis too.
Aim: The purpose of the present study was to evaluate the comparison of laser versus cautery usage in various maxillofacial surgical procedures. Methodology: In this randomized double-blind clinical trial, 40 individuals were randomly allocated to two groups: group 1 (G1) consisted of 20 individuals assigned to treatment with diode laser and group 2 (G2) consisted of 20 individuals assigned to treatment with electrocautery. The following transoperative parameters were evaluated: bleeding, temperature, and surgical technique parameters (energy deposited on tissue, flow rate, and time of incision). The postoperative parameters evaluated were as follows: pain, functional alterations (chewing, speaking), analgesic medication intake, swelling, healing of the wound area, and patient satisfaction. Results: Among the 40 individuals included in the study, four (two in G1 and two in G2) did not complete the entire follow-up. Therefore, 36 individuals (18 in G1 and 18 in G2) participated. Participants in G1 and in G2 had similar demographic characteristics. No difference regarding the trans- or postoperative parameters evaluated was observed between G1 and G2 (p > 0.05). Also, no difference regarding the time for healing was observed between groups. Conclusion: Diode laser seems to be as effective and safe as electrocautery when applied under similar conditions.
