SUMMARYSeveral subtypes of anti-liver-kidney microsome antibodies (LKM) are known, LKM-1 antibodies associated with autoimmune chronic active hepatitis recognize P450 2Df). aeytochrome P450mono-oxygenase. The frequent association of anti-LKM-I antibodies and hepatitis C virus (HCV) infections and the probable existence of an infectious and autoimmune form of anti-LKM-lassociated hepatitis, requiring different therapeutical strategies, necessitates the exact determination of anti-LKM-1 specificities. Therefore, we compared various antibody tests (immunofluorescence. ELISA witb recombinant P450 2D6. and Western bloi with rccombinani and natural antigens and agargel double diffusion) with sera of 27 anti-LKM-1-positive chronic active hepatitis (CAH) patients, with 61 sera harbouring anti-mitoehondriai antibodies. !()() sera each from HCV-RNApositive and HCV-RNA-ncgative patients, and 50 sera of healtby persons. Western blot techniques using recombinant MS2-polymcrase P450 2D6 fusion protein were found to be the most sensitive and specific method for anli-LKM-1 antibody determination in routine laboratory. The recently recognized association of anti-LKM-l antibody and HCV infection was confirmed by the results of this study. In anti-HCV and HCV-RNA-positivc patients with anti-LKM-1 antibodies there was a preponderance of males with higher mean age and lower antibody tUres. The results support the hypothesis of the cxistcnee of an autoimmune as well as an infectious (HCV triggered) subgroup of anti-LKM-t-positive hepatitis.
43 serum specimens, positive for HCV antibodies by means of an HCV-screening fest, were analysed in four hepatitis C supplementary assays: HCV EIA supplemental assay, Wellcozyme western blot, Line immuno assay and Matrix dot immuno assay. Additional a reverse transcription polymerase chain reaction (RT-PCR) was applied to hepatitis-Cvirus RNA detection in the serum samples. The supplementary tests confirmed 63%, 81% f 92%, 94% ofthe results achieved by the screening assay. Only 50% ofthe tested samples showed identical results in all four supplementary tests. In 56% ofthe sera HCVspecific RNA was detected: A correlation between the antibody reactivitypatterns in the supplementary test and the RT-PCR results could not be seen.
Das Ziel dieser Studie war es, Methoden zu entwickeln, die einen kostengünstigen Einsatz einer validierten PCR-Analytik zum Nachweis von transfusionsmedizinisch relevanten Viren (HIV-1, HAV, HBV, HCV, Parvo B19) in Einzelblutspenden ermöglichen. Die HIV-1 -RT-PCR wurde als Modellsystem zur Testvalidierung bezüglich Sensitivität und Spezifität eingesetzt. Es wurden insgesamt 600 kodierte EDTA-Blutproben, von denen 6 mit HIV-1 -Viren (Viruskonzentration 0,1 TCID50/ml) gespiket waren, in 6 Pools zu je 100 Spenden zusammengefaβt. Nach Viruskonzentration mittels Ultrazentrifuge wurden die Nukleinsäuren isoliert und anschlieβend in 3 verschiedenen HIV-1-PCR-Systemen amplifiziert und analysiert. Durch anschlieβende PCR-Analysen wurden die 6 gespiketen Serumproben fehler-frei erkannt. Es konnte zusätzlich gezeigt werden, daβ Serumpools (lOOEinzelspenden), die mit je einem HCV-, HBV-, HAV- und Parvo-B19-Virus-positiven Serum kontaminiert waren, mit Hilfe von 4 verschiedenen PCR-Analysen richtig erkannt wurden.
Zusammenfassung: Mit Hilfe von molekularbiologischen Methoden wurden 88 Blutproben von Schwangeren auf Anwesenheit von Toxoplasma gondii spezifischer Nukleinsäure untersucht Seronegative und IgG-positive/IgM-negative Patientinnen waren stets im molekularbiologischen Direktnachweis negativ. Bei Patientinnen mit einer serologisch diagnostizierten Toxoplasmose konnte immer auch T. gondü spezifische Nukleinsäure in den Blutproben nachgewiesen werden. In den Fällen, bei denen mittels serologischer Testung einer Serumprobe,sofort keine eindeutige Aussage bezüglich eines Vorliegens einer akuten T. gondii Infektion möglich war, wurde in ca. 1/3 der Proben spezifische Nukleinsäure nachgewiesen. Auf Basis dieser Untersuchungsergebnisse wurde ein Ablaufschema einer Toxoplasmose Stufendiagnostik für die Schwangerschaftsvorsorge entwickelt. In seronegative and IgGpositive/IgM-negative patients specific DNA could never be found. Samples from women with an acute toxoplasmosis always reacted positive in the DNA amplification test. In about 1/3 of the blood samples which could not be classified at once by serological testing, toxoplasma specific DNA was detected. Based on these data, a scheme for toxoplasmosis screening during pregnancy is presented.
