El siglo XX estuvo marcado por las guerras mundiales, lo que ocasionó un aumento en la prevalencia de pacientes con lesiones medulares traumáticas, los cuales tenían una mortalidad de 95% en las primeras dos semanas posteriores a la lesión. El 80% de esa mortalidad era relacionada a infecciones de vías urinarias y urosepsis. El objetivo del trabajo es presentar las aportaciones de Ludwig Guttmann, quién se dedicó a otorgar un manejo integral a estos pacientes y los efectos causados al tracto urinario inferior por daño a la médula espinal. Inició con el cateterismo intermitente como tratamiento a la estasis urinaria y observó una disminución significativa en el riesgo relativo de muerte. De igual forma incluyó la rehabilitación física y ejercicio como pilar del tratamiento en las etapas tempranas a la lesión; lo que estableció el nacimiento de los Juegos Paraolímpicos.
