Objetivo: establecer la prevalencia y caracterización de las disfunciones sexuales en una población de mujeres sexualmente activas, con edades entre 18 y 72 años, en doce ciudades colombianas.Materiales y métodos: estudio de corte transversal descriptivo. Se incluyeron mujeres mayores de 18 años con actividad sexual en las últimas 6 semanas, y residentes en Colombia. Se excluyeron las mujeres analfabetas, embarazadas o en primeros 6 meses posparto, pacientes psiquiátricos, con déficit neurológico o historial de cáncer. El estudio se llevó a cabo en la consulta externa ginecológica de instituciones hospitalarias privadas en 12 ciudades del país (Bogotá, Medellín, Cali, Barranquilla, Cartagena, Cúcuta, Ibagué, Bucaramanga, Villavicencio, Pereira, Manizales y Armenia) entre junio de 2009 y diciembre de 2016. Se realizó muestreo consecutivo. Se aplicó el Índice de Función Sexual Femenina validado en español. Se midieron variables sociodemográficas, antecedentes de salud sexual y reproductiva, comportamiento sexual, frecuencia de disfunción sexual global y por tipo de disfunción evaluada. Se realizó un análisis descriptivo de la información utilizando medidas de frecuencia absoluta y relativa para los datos. Se hace descripción estratificada por edad (40 años o menos y mayor de 40 años).Resultados: de un total de 72.894 mujeres candidatas a ingresar se analizaron finalmente 50.991 (69,95 %). La edad promedio fue de 30,9 ± 10,8 años. La prevalencia de disfunción sexual en el grupo estudiado fue del 32,97 % (16.812 mujeres). La puntuación del Índice de Función Sexual Femenina (IFSF) en las mujeres afectadas fue de 24,07 ± 6,18 puntos. Se presentaron dificultades con el deseo (32,97 %), el orgasmo (21,93 %), la excitación (16,86 %), la lubricación (14,79 %) y dolor (7,56 %). La mediana de disfunciones sexuales por mujer fue de 2, que se hizo presente en el 64,16 %.Conclusión: en las mujeres colombianas existe una prevalencia de disfunciones sexuales cercana a la tercera parte de la población, caracterizadas principalmente por trastornos del deseo y del orgasmo. Se requieren intervenciones para establecer un plan diagnóstico y terapéutico inmediato.
Cistocele; Enfermedades de los genitales femeninos; Prolapso de órgano pélvico; Trastornos del suelo pélvico Resumen Objetivos: Evaluar los factores de riesgo relacionados con el prolapso genital y la prevalencia del mismo. Método: Se realizó un estudio retrospectivo, descriptivo, epidemiológico. Se incluyó a todo el grupo poblacional femenino mayores de edad, no obstétrico, que acudió al servicio de consulta externa/ambulatoria del Departamento de Ginecología de la Clínica La Sagrada Familia desde el 1 de enero de 2009 hasta el 30 de junio de 2012. Se revisaron las historias clínicas de las mujeres incluidas en el estudio para confirmar los criterios de selección de las participantes, así como el diagnóstico y la clasificación de la enfermedad, y se evaluaron los factores de riesgo relacionados con la aparición de prolapso genital: años de estudio ≤ 9, cirugía pélvica previa, diabetes, edad, episiotomía, estreñimiento, hábitos laborales y recreativos, hipotiroidismo, histerectomía previa, macrosomía fetal, menopausia, multiparidad, neumopatía crónica, número de embarazos (≥ 3), obesidad, parto instrumentado (fórceps), raza blanca y tabaquismo. Resultados: De las 13.824 mujeres que participaron en el estudio, 3.681 (26,6%) estaban histerectomizadas y se diagnosticó prolapso genital en 2.193 (68,38%). Entre las 10.143 (73,4%) no histerectomizadas se diagnosticó prolapso genital en 1.014 (31,62%). Del total de las pacientes con prolapso (n = 3.207/13.824), 1.662 (57,7%) tenían prolapso de grado i, 978 (56,5%) prolapso de grado ii, 459 (14,3%) prolapso de grado iii y 108 (3,37%) prolapso de grado iv. De las 3.207 pacientes con prolapso genital, el 57% fue sintomático (n = 1.828) y en el 43% asintomático (n = 1.379). El promedio de edad en los casos fue de 57,6 ± 11,4 años, y de 54,9 años ± 13 en los controles. El 84% de las pacientes sintomáticas consultaron por síntomas de disfunción del piso pélvico: sensación de peso vaginal (42%), masa vaginal (27%), IUE (18%) y 9% por menometrorragia e infección urinaria, y el otro 4% por otras condiciones ginecológicas. El número de embarazos osciló entre 0 y 15, con un promedio de 6,3 ± 4,2; el 38% con ≥ 6 gestaciones. El número de partos estuvo entre 1 y 12, con un promedio de 4,8 ± 3,9; la mayoría (45%) entre 1 y 3, el 39% de 4 a 6, y el 16% restante tenían más de 7 partos. La frecuencia de la consulta por prolapso genital es creciente, siendo el prolapso anterior sintomático el más frecuente
Objective: To evaluate the efficacy and tolerance of policresulen in the treatment of genitourinary syndrome of menopause Materials and methods: Controlled clinical trial, randomized, masked "double-blind". 321 women aged 40 or older, diagnosed with genitourinary syndrome of menopause, who had engaged in sexual activity in the last six weeks, and had visited a Level III private health care institution in Armenia, Quindío (Colombia), from February to December 2018 were included. Women under hormonal treatment and with absolute contraindications for the use of policresulen were excluded. Women were randomly assigned to two treatments: 117 received policresulen and 114 placebos (control group). Efficacy and tolerance were evaluated, as well as the adverse effects presented during the 12 weeks of follow-up. A nonstrict consecutive sampling was made. Results:The dose of policresulen was effective for symptom control, showing an efficacy of 76.92%, (n=90/117), versus 38.59%, (n=44/117) in the placebo group, with a statistically significant difference (p=0.003). Policresulen tolerance was adequate in 91.45% of women, compared to 92.98% of placebos, without statistically significant difference (p=0.375). The incidence of adverse effects was as follows: local burning sensation (4.27%), discharge of mucosal tissue fragments (2.56%) and vaginal candidiasis (1.7%) in the policresulen group, compared with, 5%, 1.75% and 1.75%, respectively, in the placebo group. There were fewer adverse effects in the placebo group, but there was no statistically significant difference between groups (89.74% vs. 92,1; p=0,27). Conclusion:The use of policresulen in women with genitourinary syndrome of menopause is effective for the management of the most frequent symptomatology. Although it is true that it is not the first therapeutic line of treatment, it should be considered in women for whom estrogen therapy is contraindicated or in those who do not wish to receive it. There was a low presence of adverse effects at the dose used, however, these were tolerable and did not require the interruption of the medication.
Objetivo. Evaluar y describir los resultados posoperatorios de la colpocleisis en una población de mujeres del Quindío. Materiales y métodos. Estudio de corte transversal. En una población de mujeres mayores de 69 años, con prolapso genital ≥ al grado III, sexualmente inactivas y sin interés por preservar la función sexual coital; se les realizó la colpocleisis de LeFort (entre 2009 y 2019). La investigación se efectuó en una institución prestadora de servicios de salud privada, de nivel III, en la ciudad de Armenia. Las mujeres fueron evaluadas antes del procedimiento, luego en las primeras 2 semanas, a los 6 meses, a los 12 meses y después cada año. Resultados. Se operaron 102 mujeres, 69 (67,64 %) por prolapso grado IV y 33 (32,35 %) grado III. La edad media fue de 75,39 ± 8,46 años, con un IMC de 27,48 ± 7,59. El tiempo quirúrgico promedio fue 45,93 ± 18,63 minutos. La mediana de la hemorragia transquirúrgica fue de 75 mL. El éxito quirúrgico alcanzado fue del 98,03 %. Conclusiones. La colpocleisis de LeFort, como tratamiento del prolapso genital ≥ al grado III, ofrece una tasa de mejoría cercana al 98,03 %, con mínimas complicaciones.
