Deviations from a regular arch form become apparent very early during dentition development and coexist with specific orofacial dysfunctions. They are thus important indicators for the early detection of functional abnormalities, causing deviations from normal dentition development. In children with orofacial dysfunctions the development of a narrow maxillary dental arch should be prevented by myofunctional therapy and by educating the parents. Interceptive orthodontic measures to treat a narrow maxillary arch in primary and early mixed dentition should also focus on eliminating functional disturbing factors, such as orofacial dysfunctions. Interdisciplinary cooperation with specialists in other fields of medicine, e.g. otorhinolaryngology and speech therapy, is essential to achieve this goal.
The aim of this study was to analyze sagittal and vertical dentofacial dimensions in subjects with normal occlusions during the juvenile and adolescents age periods to establish age- and gender-specific lateral cephalometric standard values for Germans during their active growth period. The study group consisted of a sample of 32 untreated subjects with normal occlusions. Lateral cephalograms were analyzed at 11 consecutive stages, from 6-13 and from 15-17 years of age. A customized cephalometric analysis was used to measure 53 variables. Statistical comparisons of gender-specific differences were performed by means of Mann-Whitney U tests.Anterior and posterior cranial base lengths, midfacial length as well as mandibular length were recorded to be significantly larger in male subjects at the age of 6 years. For most of the linear measurements, significantly larger craniofacial distances were recorded in males from the age of 15 years onward. There were no statistically significant gender differences with regard to most angular measurements at subsequent age groups. Soft tissue analysis revealed flatter profiles in females than in males from the age of 10-11 years onward, while age-dependent changes in the soft tissue profile were similar in both genders.In untreated subjects with normal occlusion craniofacial development of the hard and soft tissues can be considered age- and gender-dependent. Therefore age- and gender-specific differences of linear craniofacial distances should be taken into account for diagnosis and treatment planning in children and adolescents. The present results can be used as reference values for children and adolescents of German origin.
In allen Fachgebieten der Zahnheilkunde hat sich der viel zitierte Paradigmenwechsel hin zur Prävention vollzogen. Allein in der Kieferorthopädie wird derzeit das Schwergewicht therapeutischer Einflussnahme auf sog. Spätbehandlungen gelegt. Darunter versteht man Therapien am Ende der Wechselgebissperiode und im bleibenden Gebiss. Mit den KIG (kieferorthopädische Indikationsgruppen der gesetzlichen Krankenkassen) wurden Richtlinien des Umfangs von Zahnstellungsund Okklusionsanomalien erhoben, die es möglich machen sollen, den Schweregrad einer Anomalie metrisch zu erfassen. Damit soll vorgebeugt werden, dass Behandlungen von Bagatellanomalien zulasten der Krankenkassen durchgeführt werden. Da sich aber kieferorthopädische Anomalien mit dem Fortschreiten der Gebissentwicklung und des Wachstums verändern, kommt es darauf an, die Prognose der Anomalie richtig einzuschätzen. Nur dann kann es gelingen, einer Fehlentwicklung frühzeitig zu begegnen. In diesem Sinne ist die Milchgebissperiode die günstige Zeit, um belastende Faktoren zu erkennen. Das schließt nicht aus, dass auch danach kieferorthopädische Prävention nützlich ist, versteht sich doch das Fach in seiner Gesamtheit als Prävention für den Langzeiterhalt des Gebisses. Merke: Die KIG orientieren sich an metrischen Abweichungen der Zahnstellung und der Okklusion. Sie berücksichtigen dabei die Prognose einer Fehlentwicklung wenig oder gar nicht. Status quo Das Bemühen, Indizes zu entwickeln, um die Behandlungsbedürftigkeit zu definieren, ist nicht neu. In jüngster Zeit hat sich der IOTN-Index (Index Orthodontic Treatment Need) für Bewertungen des Anomalieumfangs durchgesetzt. Auch dieser Index ist nicht in der Lage, die noch zu erwartende Entwicklung zu bewerten. Anders verhält es sich mit dem DMF-oder dmf-T-Index. Er erlaubt eine klare Aussage über den Gesundheitszustand der Zähne, allerdings unabhängig vom Lebensalter. Um bei Kindern das Kariesrisiko vorausschauend zu erkennen, wurde dieser Index weiterentwickelt und altersabhängig definiert. In Anlehnung daran wurde zusätzlich die kieferorthopädische Prognose einbezogen und die Definition des "kieferorthopädischen Risikokinds" formuliert [1 -3]. Diese Definition berücksichtigt den Einfluss der die Zahnreihen und Kiefer umgebenden peri-und enoralen Weichteile als Funktion oder Dysfunktion. Unabhängig davon, ob dieser funktionelle Einfluss als ein äußerer oder ein anlagebedingter angesehen wird, ist dessen Bewertung und in der Folge dessen Beeinflussung möglich.
