Mental disorders in Latin-America are highly prevalent and represent a significant burden on users and their families. These people most of the times have to deal with the stigma attached to the diagnosis they receive. Stigma towards mental illness causes negative consequences for patients and family members, becoming the main barrier to full social inclusion. Considering the above, the first objective of this paper is to analyze the main characteristics of stigma towards mental illness and the psychological and social variables with which it is associated. Secondly, we describe the main strategies for reducing different types of stigma. Finally, we propose approaches to assess and reduce stigma in the context of Latin-America and the Caribbean.Key words: Stigma, discrimination, mental disorders, social inclusion. RESUMENLos trastornos mentales en Latinoamérica son altamente prevalentes y representan una carga significativa para usuarios y familiares. Dichos individuos usualmente deben lidiar con el estigma que se asocia al diagnóstico que reciben. El estigma hacia la enfermedad mental provoca consecuencias negativas en los pacientes y sus familiares, transformándose en la barrera principal para lograr su plena inclusión social. Considerando lo anterior, el primer objetivo del presente documento es analizar las principales características del estigma hacia la enfermedad mental, y las variables psicológicas y/o sociales con las que se ha asociado. En segundo lugar, se describen las estrategias preponderantes para reducir los diferentes tipos de estigma. Finalmente, se proponen abordajes para evaluar y reducir el estigma en el contexto de Latinoamérica y el Caribe. INTRODUCCIÓNEn América Latina, se ha determinado que los trastornos mentales son altamente prevalentes y representan una significativa carga para quienes los padecen.1 Dichos individuos usualmente deben lidiar, en la interacción diaria con otras personas o instituciones, con barreras y obstáculos sociales. Al respecto, un tipo de interacción social que puede presentar consecuencias negativas para las personas con trastornos mentales es lo que se ha denominado "estigmatización" o, simplemente, "estigma". Debido a los prejuicios y actos discriminatorios hacia las personas estigmatizadas, es frecuente que éstas presenten un bajo nivel de autoestima y una menor calidad de vida, además de una baja adherencia a los tratamientos y una reducción significativa de sus redes sociales.2 También se ha observado que, frecuentemente, dichas personas no pueden acceder a espacios laborales o educacionales normalizados, ni tampoco pueden establecer relaciones de amistad o de pareja. SUMMARYThe employability and access to the productive market are considered key elements for the full integration of the patient with mental disability. The aim of this review was to describe and analyze the scientific literature for the main employability strategies: traditional vocational rehabilitation (sheltered work and social firms) and supported employment (particularly in its mode of individualized supported employment). The results of the review suggest that individualized supported employment is the most effective approach in obtaining employment for people with mental disabilities.Key words: Mental disability, employability, sheltered work, social firms, supported employment. RESUMENLa inserción laboral y el acceso al mercado productivo son considerados elementos claves para la plena integración del paciente con discapacidad mental. El objetivo de la presente revisión fue describir y analizar la literatura científica correspondiente a las principales estrategias de inserción laboral: rehabilitación vocacional tradicional (empleo protegido y empresas sociales) y empleo con apoyo (particularmente en su modalidad de empleo con apoyo individualizado). Heart energy metabolism and its role in the treatment of heart failure It is unknown why heart failure progresses even when patients are treated with the best therapy available. Evidences suggest that heart failure progression is due to loss of neurohumoral blockade in advanced stages of the disease and to alterations in myocardial metabolism induced, in part, by this neurohumoral activation. Alterations in cardiac energy metabolism, especially those related to substrate utilization and insulin resistance, reduce the efficiency of energy production, causing a heart energy reserve deficit. These events play a basic role in heart failure progression. Therefore, modulation of cardiac metabolism has arisen as a promissory therapy in the treatment of heart failure. This review describes myocardial energy metabolism, evaluates the role of impaired energy metabolism in heart failure progression and describes new therapies for heart failure involving metabolic intervention.
