Mutilações faciais decorrentes de cirurgia oncológica provocam deformações na fisionomia, levando os pacientes à depressão e ao isolamento social. Na impossibilidade de reconstrução cirúrgica, a reabilitação protética com próteses bucomaxilofaciais é o meio preferível para restabelecer a aparência estética e corrigir dificuldades de alimentação, deglutição e fonação, proporcionando reintegração social e melhorando a qualidade de vida. Este artigo visa relatar um caso de reabilitação com prótese obturadora maxilar em paciente pediátrico após cirurgia oncológica para tratamento de osteoma em palato, que provocou comunicação bucosinusal e perda parcial da maxila. Paciente do gênero feminino, 11 anos, procurou atendimento odontológico após cirurgia de excisão de um osteoma que originou comunicação bucosinusal. Após anamnese, exames intra e extra-oral e análise dos modelos de estudo optou-se pela realização de uma prótese parcial removível provisória superior com obturador palatino e grampos auxiliares de fios ortodônticos, pois a paciente encontrava-se em fase de crescimento. Após moldagem das arcadas superior e inferior, confecção do plano de cera superior e ajuste deste plano seguindo o perfil facial e oclusal da paciente, os modelos foram montados em articulador semi-ajustável e realizada a seleção e montagem dos dentes artificiais. A prova estética e funcional dos dentes mostrou o restabelecimento estético e fonético da paciente. Foi realizada moldagem funcional com boca fechada seguida da acrilização, instalação e proservações da prótese. Devido ao crescimento de praticantes de atividade esportiva houve um aumento do índice de traumatismo dentário. Desta maneira, o presente trabalho tem como objetivo descrever o caso clínico de um atleta com fratura dentária do elemento 11. Paciente de 14 anos, sexo masculino, compareceu à Clínica de Odontologia da Universidade Nilton Lins, queixando-se de fratura de dente anterior durante a prática de esporte. Após anamnese e exame clínico, optou-se pela restauração direta em resina composta do elemento fraturado e confecção do protetor bucal, como plano de tratamento, a fim de evitar traumas posteriores. Pode-se concluir que a restauração em resina composta direta do dente anterior fraturado foi realizada de maneira satisfatória, restabelecendo função e estética, além da harmonia do sorriso e a autoestima do paciente e que o uso de protetores bucais é importante para a prevenção de futuros traumas dentários durante a prática desportiva.Descritores: Traumatismos em Atletas; Resinas Compostas; Traumatismos Dentários; Protetores Bucais.
We report a unique case of a potassium aluminum sulfate oral dissolution leading to palatal chemical necrosis and extensive chemical ulcers on the tongue. The patient, a 47-year-old white, blind male, denied using cocaine or other illegal drugs that could cause such lesions. His self-medication started as a treatment for a traumatic ulcerative lesion on the hard palate. After palatal perforation, he started another self-medication routine, mixing corticoid cream and tandrilax tablets with a gauze obturator. Our treatment comprised the removal of all chemical factors, a surgical debridement, and a prosthetic obturator to resolve the communication. The 1-year follow-up showed no complications.
The polymerization process of total dentures causes dimensional changes, making it necessary to remounting these prostheses in a semi-adjustable articulator (ASA) to perform the occlusal adjustment. The objective of this study is to report a clinical case of oral rehabilitation with conventional full dentures, which was performed by the clinical remounting of the prostheses in ASA to perform the occlusal adjustment. A 72 years old male patient sought dental care at the Polyclinic of the State University of Amazonas. After anamnesis, extra and intraoral clinical examinations were indicated for conventional full dentures. After anatomical and functional impressions, working models were obtained, on which orientation plans were made. From the individualization of the upper orientation plane, registration of the facial arch and determination of intermaxillary relations, the models were assembled in ASA. After selection, assembly and aesthetic and functional testing of artificial teeth, the prostheses were polymerized, finishing and polishing. During the installation of the prostheses, there were few occlusal contacts and an anterior open bite. With the prostheses positioned on the edges, the facial arch was recorded to remounting the upper prosthesis in ASA and interocclusal registration in the central relation position for remounting of the lower prosthesis. The occlusal adjustment was performed by means of selective wear to obtain the correct centric and eccentric occlusal contacts. Clinical remounting of total prostheses in ASA is an efficient procedure for performing occlusal adjustment and obtaining bilateral balanced occlusion.
