A polimiosite (PM) é uma enfermidade sistêmica idiopática caracterizada por um processo inflamatório não supurativo que acomete a musculatura esquelética e manifesta-se clinicamente por fraqueza muscular proximal e simétrica. Relatamos um caso interessante e incomum de PM cuja manifestação inicial foi o envolvimento da língua de uma mulher de 54 anos com glossodínia e extensa investigação prévia inconclusiva. Na revisão da literatura realizada encontramos somente uma descrição análoga; portanto, achamos relevante esta publicação, que constitui provavelmente o segundo caso descrito na literatura internacional. INTRODUÇÃOA polimiosite (PM) é uma doença sistêmica do tecido conjuntivo, cujo achado clínico predominante é a fraqueza muscular proximal e simétrica. Possui incidência variável geograficamente, com cerca de um caso para cada 100 mil habitantes, havendo predominância no sexo feminino. Sua fisiopatologia é desconhecida, e dentre os mecanismos mais prováveis sugere-se injúria muscular mediada por vírus ou insulto microvascular por auto-antígenos musculares (1)(2)(3) . A fraqueza muscular é o principal sintoma que induz a suspeita diagnóstica, sendo simétrica, bilateral e proximal, acometendo músculos da cintura escapular e pélvica e progredindo para a musculatura proximal dos membros (4) . O início é freqüentemente gradual e progressivo. O comprometimento da musculatura esquelética da parede posterior da faringe e do terço proximal do esôfago pode dar origem à disfagia alta, com refluxo de alimentos para vias aéreas altas e disfonia. O comprometimento do flexor do pescoço ocorre em dois terços dos casos, o que pode ocasionar dificuldade para sustentar o pescoço. Os sintomas constitucionais incluem fadiga, febre baixa, perda de peso, artralgia ou artrite de pequenas e médias articulações (5) . O envolvimento cardíaco geralmente se traduz por taquicardia, arritmias, defeitos da condução átrio-ventricular e bloqueios de ramos. Raramente há insuficiência cardíaca congestiva, seja por miocardite, seja por fibrose miocárdica (6) . ABSTRACT
Introduction. Skin disorders can be the first manifestation of occult diseases. The recognition of typical paraneoplastic dermatoses may anticipate the cancer diagnosis and improve its prognosis. Although rarely observed, the sudden appearance and/or rapid increase in number and size of seborrheic keratoses can be associated with malignant neoplasms, known as the sign of Leser-Trélat. The aim of this report is to unveil a case of a patient whose recently erupted seborrheic keratoses led to investigation and consequent diagnosis of bladder cancer. Case Presentation. A 67-year-old man was admitted to the intensive care unit due to an exacerbation of chronic obstructive pulmonary disease (COPD). On physical examination, multiple seborrheic keratoses on the back of the hands, elbows, and trunk were observed; the patient had a 4-month history of these lesions yet was asymptomatic. The possibility of Leser-Trélat syndrome justified the investigation for neoplasia, and a bladder carcinoma was detected by CT-scan. The patient denied previous hematuria or any other related symptoms. Many of the lesions regressed during oncologic treatment. Conclusion. Despite the critics on the validity of the sign of Leser-Trélat, our patient fulfills the description of the disease, though urinary malignancy is a rare association. That corroborates the need of further investigation when there is a possibility of paraneoplastic manifestation.
A hiperuricemia pode causar desde a deposição de cristais de urato monossódico na cavidade articular até a "angústia clínica", provocando a prescrição desnecessária de fármacos. Neste caso, o paciente terá mais ganho de efeitos colaterais que benefícios. O alopurinol é o uricorredutor mais prescrito, sendo que reações de hipersensibilidade ocorrem em até 10% de seus usuários, com mortalidade de 20%. Com este relato de caso objetivamos caracterizar a síndrome de hipersensibilidade ao alopurinol (SHA), enfatizando o correto uso dessa droga.Palavras-chave: síndrome de hipersensibilidade ao alopurinol (SHA), hiperuricemia, efeitos colaterais. ABSTRACTHyperuricemia can lead to deposition of monosodium urate crystals in the joint cavity or to "physicians' anguish", causing prescription of unnecessary drugs. In this case, more side effects than benefits will be observed. Allopurinol is the most prescribed urate lowering drug and hypersensivity reactions occur in up to 10% of patients taking the drug, with mortality of 20%. With this case report we intend to characterize the Allopurinol Hypersensivity Syndrome (AHS), emphasizing the right use of the drug.
A linfohistiocitose hemofagocítica caracteriza-se por ativação e proliferação excessiva de linfócitos e macrófagos. Quando associada à artrite idiopática juvenil é também conhecida por "síndrome de ativação do macrófago", sendo uma complicação potencialmente fatal desta doença. Apresentamos o caso de uma mulher de 26 anos portadora de artrite idiopática juvenil (poliartrite, fator reumatóide negativo), com diagnóstico aos 13 anos, em uso de antiinflamatórios não esteroidais (diclofenaco, nimesulide). Admitida com quadro de resposta inflamatória sistêmica, febre, linfonodomegalia, esplenomegalia, anemia, trombocitopenia, hipofibrinogenemia, hiperferritinemia, hipertrigliceridemia e achados de hematofagocitose na medula óssea. Os autores discutem aspectos relacionados com a patogênese, diagnóstico e tratamento desta doença pouco conhecida.Palavras-chave: síndrome da ativação do macrófago, linfohistiocitose hemofagocítica, artrite idiopática juvenil.
