A Incontinência Urinária (IU) diz respeito a qualquer perda involuntária de urina. Essa doença pode ocorrer em ambos os sexos, no entanto, a IU ocorre majoritariamente em mulheres, estando muitas vezes associada à disfunção da bexiga ou dos músculos do assoalho pélvico, sendo mais frequente durante a gravidez, parto ou após a menopausa. A prevalência da IU aumenta consideravelmente na quinta década de vida das mulheres. Tal patologia pode ser dividida em tipos diferentes, sendo a Incontinência Urinária de Esforço (IUE), a Incontinência Urinária de Urgência (IUU) e a Incontinência Urinária Mista (IUM) as mais comuns. Para o diagnóstico de IU é necessário uma história e um exame físico detalhado, de forma que se consiga não somente confirmar o diagnóstico, mas também fazer a exclusão de outras condições como: epitélio uretral e vaginal finos e ressecados, obstrução intestinal, infecções, limitação da mobilidade, causas emocionais, terapia medicamentosa, endocrinopatias ou delirium. Além da anamnese e exame clínico, podem ser necessários alguns exames complementares para melhorar a investigação, dentre os exames, pode-se citar: urinálise, urocultura, função renal, diário miccional, ultrassonografia e o teste urodinâmico, exame normalmente realizado em pacientes com IU persistente ou com sintomas resistentes às medidas conservadoras. O tratamento da IU pode ser feito de várias formas, porém, nos casos de IUE ou Mista, desde que sejam classificadas como leves ou moderadas, o tratamento conservador por meio da fisioterapia deve ser a primeira opção devido a sua alta efetividade e acessibilidade nessas situações. No entanto, em outras ocasiões, o tratamento farmacológico e o tratamento cirúrgico também podem ser ótimas opções para o tratamento da IU, principalmente na IUU, contribuindo, dessa forma, para uma melhora considerável da qualidade de vida das mulheres que sofrem dessa doença.
A infecção do trato urinário (ITU) é uma doença extremamente comum no ambulatório, fato que gera um número significativo de internações, consultas médicas e aumento do absenteísmo, principalmente nas infecções recorrentes. Essa afeccção existe uma prevalência maior entre indivíduos do sexo feminino e possui diversos fatores de risco, como: mudança da flora vaginal, atrofia vulvovaginal, relações sexuais, bexiga neurogênica, cateterismo vesical e histórico de ITU tanto na infância como também na pré-menopausa. O patógeno mais relacionado à ITU é a Escherichia coli, sendo responsável por cerca de 80% das ITUs não complicadas. A patogênese da ITU inicia-se com a colonização do intróito vaginal ou meato uretral por microorganismos, principalmente, da microbiota intestinal. O transporte desses patógenos pela uretra até a bexiga é facilitado pela aderência das bactérias ao epitélio uretral. Essa adesão estimula as células da mucosa a produzirem citocinas que serão responsáveis pela mobilização de leucócitos para o sítio da infecção. Os sintomas clássicos da ITU englobam disúria, urgência urinária, polaciúria e dor suprapúbica. Ademais, sintomas como febre, taquicardia e sensibilidade costovertebral devem alertar quanto à possibilidade de pielonefrite. No que se refere ao diagnóstico de ITU, esse pode ser feito tanto de forma clínica, como também por meio da ajuda de alguns exames complementares, como o exame simples de urina, gram e urocultura. O tratamento mais usual de ITU é a administração oral de antibiótico, podendo ser complementado com medicamentos sintomáticos. No entanto, o médico deve sempre individualizar a terapêutica e instruir aquelas pacientes que possuem ITU recorrente em relação a mudanças comportamentais, dessa forma, além de simplesmente tratar um quadro agudo, o médico também estará contribuindo para a prevenção de novas infecções e de possíveis complicações associadas.
A doença celíaca (DC) é uma doença autoimune caracterizada por alterações intestinais associadas com a expressão de linfócitos T em indivíduos geneticamente predispostos que fazem a ingestão de glúten. A DC afeta aproximadamente 1% da população mundial. Suas manifestações clínicas clássicas aparecem nos primeiros dois anos de idade, enquanto seu segundo pico ocorre entre os 20 e 30 anos. A fisiopatologia da DC é uma interação entre a genética e o ambiente. Indivíduos com genes mais propensos à intolerância parcial à gliadina presente no glúten são indivíduos que possuem essa predisposição para DC. Assim, quando expostas aos derivados do glúten, as células T sensíveis tornam-se ativas, causando inflamação e atrofia da mucosa do intestino delgado, o que leva à má absorção. A maioria dos pacientes com DC não apresentam sintomas, todavia, os sintomas da DC, quando presentes, podem ser encontrados nos intestinos, em outras partes do corpo ou em ambos. Ademais, podem haver complicações em diferentes gravidades tanto para homens quanto para mulheres. O diagnóstico da DC requer uma associação de fatores. Considera-se que a DC possui três pilares que ajudam a confirmar sua presença: testes sorológicos, histologia duodenal e testes dietéticos. Estes são os maiores responsáveis pelo diagnóstico final da doença. O tratamento mais eficaz para DC é a remoção do glúten da dieta, juntamente com um monitoramento dietético rigoroso e mudanças no estilo de vida.
A Insuficiência Adrenal (IA) é um distúrbio que se caracteriza pela perda ou pela redução da função do córtex da glândula adrenal. Estas glândulas produzem hormônios, como glicocorticoides, mineralocorticoides e androgênios, bem como as catecolaminas, que são fundamentais para a manutenção da homeostase humana. A maioria das causas de IA na infância é hereditária, enquanto na idade adulta as etiologias mais comuns são a doença de Addison, doenças autoimunes, distúrbios infiltrativos/metástases, hemorragia e doenças infecciosas. Acerca das manifestações clínicas, estas se mostram, muitas vezes, inespecíficas, acarretando no atraso do diagnóstico. Dessa forma, a investigação diagnóstica da IA deve ser minuciosa, através de uma anamnese detalhada e por meio de exames laboratoriais, como a dosagem de cortisol basal e o teste de estímulo. Após identificado, o manejo terapêutico é baseado na reposição de hormônios adrenais, como a hidrocortisona, geralmente por meio de injeções IV ou IM, e pode ser acompanhado com solução salina isotônica e dextrose. Alcançando a estabilização hormonal do paciente, a dose dos glicocorticóides é reduzida e passada para uso oral. Ademais, a educação do paciente e de seus familiares e cuidadores é o componente mais importante para a crise adrenal, devendo-se instruí-los em relação à prevenção, medidas de emergência e o ajuste da dose de medicação. Além disso, é necessário realizar acompanhamento anual com o endocrinologista após o diagnóstico.
A Bronquiolite Viral (BV) é uma condição aguda que afeta o trato respiratório inferior. Especialmente comum durante os meses mais frios do outono e inverno, a BV é uma das principais causas de hospitalização em crianças. O Vírus Sincicial Respiratório (VSR) é frequentemente associado à BV e pode levar a complicações graves em bebês, incluindo letargia, febre, inapetência, otite média, entre outras. Na maioria dos casos, em lactentes, a doença se apresenta de forma leve e pode ser resolvida em algumas semanas. O diagnóstico da BV pode ser realizado clinicamente, com exames adicionais usados para avaliar possíveis complicações. Os critérios de internação incluem baixos níveis de saturação de oxigênio, sinais de desconforto respiratório moderado a grave, apneia e desidratação. O tratamento geralmente envolve medidas de suporte, garantindo uma oxigenação adequada, uma melhora da respiração e também atendendo as necessidades de fluidos e nutrição das crianças. Todavia, ainda existe uma grande necessidade de novos estudos a respeito da BV, objetivando-se sanar as lacunas do conhecimento evidenciadas no presente artigo.
