Résumé -Objectif : L'intoxication par l'azide de sodium est rare, mais généralement mortelle : nous présentons deux cas d'intoxications fatales, objets d'analyses toxicologiques collaboratives entre 3 laboratoires de toxicologie. Méthodes : Les azotures ont été recherchés et dosés à l'aide d'une méthode de chromatographie liquide couplée à la spectrophotométrie UV à barrette de diodes après dérivation par le chlorure de benzoyle. Résultats : Des azotures ont été décelés dans différents milieux biologiques (urine, sang, contenu gastrique, poumons, cerveau, coeur…) ante, peri et post-mortem, mais l'interprétation des concentrations sanguines mesurées est délicate en raison de l'instabilité in vitro des azotures : la réalisation de prélèvements sanguins sur fluorure de sodium et la conservation des échantillons biologiques à -20°C semble pouvoir ralentir ce phénomène. Conclusion : D'une manière générale, malgré les données de la littérature, et les concentrations de cyanures observées, il demeure difficile d'évaluer les doses d'azide de sodium prises en fonction des concentrations sanguines d'azotures mesurées.Mots clés : Azide de sodium, intoxication, chromatographie liquide, concentrations post-mortem, instabilité Abstract -Objectives: The intoxication with sodium azide is rare, but most of time lethal. We report two fatal cases, for which analysis were performed in three different toxicology laboratories. Methods: Azide determinations were performed using a liquid chromatography coupled to a diode array detector method after a benzoyl chloride derivation. Results: Azide was detected in several ante, peri and post-mortem biological samples (urine, blood, gastric content, lungs, brain, heart, …) Due to its in vitro instability, the interpretation of the measured blood concentrations was hardly feasible. Nevertheless, blood samples collections on fluoride and preservation of the collected samples at -20°C seem able to limit this phenomenon. Conclusion: As only a few data are available from the literature, and even if they were combined with cyanide concentrations, the azide blood concentrations were not informative enough to estimate the toxic intake. 1 Cas cliniques Cas n°1Un enfant de 5 ans effectue un voyage de plusieurs centaines de kilomètres en voiture avec son père, divorcé, entre le domicile maternel et le domicile paternel. Pendant le trajet, peu avant l'arrivée, le jeune garçon est pris de nausées, vomissements et de somnolence. Aux urgences pédiatriques, puis en réanimation pédiatrique, les bilans biologiques révèlent une acidose méta-bolique (pH : 6,67) avec une lactacidémie majeure (12 mmol/ L ; valeurs normales de 0,7 à 2,2 mmol/L) et une méthémoglo-binémie à 6,9 %. Quelques heures plus tard, l'enfant présente une crise convulsive tonique généralisée avec un arrêt respiratoire et décède après 1 heure 30 de réanimation cardiopulmonaire.
