Introdução: a avaliação pré-anestésica (APA) é uma prática imprescindível no cuidado perioperatório, por oferecer condições de planejamento do ato anestésico e cirúrgico, mantendo a segurança física e emocional do paciente. Objetivo: apresentar uma revisão das principais orientações pré-anestésicas disponíveis na literatura. Metodologia: trabalho de revisão bibliográfica realizada nos domínios PubMed, SciELO, British Journal of Anaesthesia (BJA), Aneshesiology e Current Opinion. Resultados: as principais recomendações abordam a estratificação do risco anestésico/cirúrgico, jejum pré-operatório, controle da ansiedade e dos hábitos sociais, além do manejo das drogas de uso contínuo, como as medicações de ação cardiovascular, hipoglicemiantes, antiplaquetários, anticoagulantes, drogas modificadoras de doença reumática, agentes biológicos e antidepressivos. Conclusão: a APA tem como finalidade estratificar o risco anestésico e otimizar condições clínicas desfavoráveis, além de propor medidas de cuidados pré-operatórios que ofereçam segurança ao paciente.
RESUMOOs fármacos anestésicos utilizados para indução e manutenção de anestesia geral provocam alterações da dinâmica respiratória, fazendo-se necessário o uso de estratégias ventilatórias perioperatórias. A ventilação mecânica, apesar de ser uma terapia de suporte essencial, não é isenta de riscos. Dentre estes, podemos citar complicações pulmonares pós-operatórias (CPPs), que apresentam alta prevalência e potenciais implicações graves. Fatores cirúrgicos, anestésicos e do paciente contribuem para o desenvolvimento de CPPs. Essa revisão faz uma análise de artigos publicados recentemente na literatura sobre ventilação mecânica e suas consequências na morbimortalidade em pacientes cirúrgicos. Por várias décadas e até recentemente, o manejo ventilatório durante cirurgia esteve associado a altos volumes corrente (VC), ausência de pressão expiratória positiva (PEEP) e altas frações inspiradas de oxigênio (FiO 2 mecânica, por meio de estudos experimentais, observacionais e randomizados, indicam a necessidade de se instituir estratégias ventilatórias protetoras perioperatórias. Estas estratégias incluem, de maneira geral, a utilização de baixos VC, uso de PEEP, manobras de recrutamento alveolar e baixa FiO 2 . ABSTRACTThe use of anesthetic drugs for induction and maintenance of general anesthesia cause changes in respiratory dynamics, making it necessary to use perioperative ventilatory strategies. Mechanical ventilation, although it is an essential supportive therapy, is not without risk. Among these, they can mention postoperative pulmonary complications (PPCs), which have high prevalence and potential for serious implications. Many factors such as surgical, anesthetic and patient ones contribute to the development of PPCs. This review analyzes updated published literature on mechanical ventilation and its consequences on morbidity and mortality in surgical patients. For several decades and until recently, ventilatory management during surgery was linked with high tidal volumes (TV), absence of positive expiratory pressure (PEEP) and high inspired oxygen fractions (FiO 2 ). Increasing advances in the understanding of physiopathology of mechanical ventilation-induced lung injury through experimental, observational, and randomized studies indicate a need to institute perioperative protective ventilation. These strategies generally include the use of low CV, use of PEEP, alveolar recruitment maneuvers and low FiO 2 .
Objetivo: fazer uma análise qualitativa de uma entrevista com pais/responsáveis de crianças, anestesistas e psicólogos especializados em pediatria sobre o objeto de estudo que é a humanização do cuidado perioperatório através de um livro infantil específico e verificar a aplicabilidade do mesmo como uma forma de melhora do cuidado perioperatório. Metodologia: Foram entrevistados, após assinatura de termo de consentimento livre e esclarecido, responsáveis (4), anestesiologistas (4) e psicólogos (5) sobre sua opinião a respeito do livro “As aventuras do pequeno super feijão num hospital” e os possíveis impactos que o mesmo pode ter no perioperatório. Utilizou-se um questionário aberto, as entrevistas foram gravadas e transcritas para análise de conteúdo. Resultados: De forma geral, pode-se observar através respostas dos participantes, a possibilidade de impacto positivo com o uso do livro durante esse período tão estressante para pais, responsáveis e crianças. O conteúdo mimetiza o que a criança irá se deparar no centro cirúrgico, e diminuiria o impacto da separação do responsável com a criança. Conclusão: A utilização do lúdico pode revelar-se como um caminho para a humanização do cuidado à criança.
A cirurgia laparoscópica tem se tornado uma técnica cada vez mais frequente em nosso meio. Porém não está isenta de complicações. Embora rara, a embolia gasosa por CO2 é uma complicação grave, associada a uma mortalidade de até 28%. Esse é um relato de caso de um paciente do sexo masculino, 73 anos, que foi submetido a uma hepatectomia parcial videolaparoscopica para o tratamento de um adenocarcinoma. Após 200 minutos de cirurgia apresentou instabilidade hemodinâmica, diminuição da saturação de oxigênio, acidose respiratória e diminuição da ETCO2. Foi então desfeito o pneumoperitoneo e o paciente foi tratado com drogas vasoativas. O rápido diagnostico da embolia por CO2 foi importante para a rápida recuperação e ausência de sequelas no paciente.
