A doença de Cushing (DC) é uma desordem endócrina que resulta do excesso crônico de cortisol no organismo. A principal causa dessa condição é a presença de tumores hipofisários corticotróficos, os quais produzem em excesso o hormônio adrenocorticotrófico e desencadeiam a hipersecreção de cortisol pelas glândulas adrenais. Esse desequilíbrio hormonal provoca uma série de sintomas clínicos variados, incluindo obesidade central, hipertensão arterial, fraqueza muscular, fragilidade óssea e distúrbios emocionais. Para diagnosticar a doença de Cushing, são realizados exames clínicos, laboratoriais e de imagem. Através dessas avaliações, é possível detectar os níveis elevados de cortisol no organismo e identificar a origem do problema, seja nos tumores hipofisários ou em outras regiões. O tratamento da DC geralmente envolve a remoção cirúrgica do tumor hipofisário responsável pelo distúrbio hormonal. No entanto, em alguns casos, pode ser necessária a utilização de outras opções terapêuticas, como medicamentos para inibir a produção de cortisol ou radioterapia para destruir as células tumorais. Avanços na compreensão genética da doença e a identificação de novos marcadores moleculares têm o potencial de aprimorar o diagnóstico e possibilitar terapias mais direcionadas aos pacientes. Essas descobertas podem resultar em tratamentos mais eficazes e com menos efeitos colaterais, visando melhorar a qualidade de vida dos indivíduos afetados pela DC. Apesar dos avanços, ainda existem desafios a serem superados no diagnóstico precoce e no manejo eficaz da doença de Cushing. Além disso, a busca por terapias mais efetivas e menos invasivas continua sendo uma meta importante para melhorar a vida dos pacientes e minimizar os impactos da doença.
A Síndrome Metabólica (SM) é uma condição clínica que é caracterizada pela presença de vários fatores de risco para doenças cardiovasculares, diabetes e outras patologias metabólicas. Os critérios para diagnóstico da SM incluem hipertensão arterial, níveis elevados de triglicérides, níveis reduzidos de HDL, hiperglicemia e aumento da circunferência abdominal. É comum que a obesidade esteja associada à SM, uma vez que a obesidade abdominal é considerada um dos critérios para o diagnóstico desta síndrome. A SM e a obesidade são problemas graves de saúde pública, pois estão relacionadas a um aumento no risco de desenvolver doenças crônicas, como doenças cardiovasculares, diabetes, acidente vascular cerebral e alguns tipos de câncer, além de comprometerem a qualidade de vida e a expectativa de vida. O tratamento da SM e da obesidade envolve, em geral, mudanças no estilo de vida, como uma alimentação saudável, atividade física regular e controle de peso. Em casos graves, pode ser necessário o tratamento médico, incluindo medicação para controlar os fatores de risco individualmente. Em conclusão, a SM e a obesidade são graves problemas de saúde que precisam ser prevenidos e tratados adequadamente. O diagnóstico precoce e o tratamento adequado são fundamentais para prevenir complicações graves e preservar a saúde a longo prazo. É crucial que os profissionais de saúde trabalhem em conjunto com os pacientes para desenvolver estratégias eficazes de prevenção e tratamento para combater essas condições.
Introdução: A taxa de mortalidade infantil (TMI) é a frequência de óbitos de crianças menores de um ano de idade a cada mil crianças nascidas vivas. Objetivo: Caracterizar a incidência dos óbitos e o desempenho da TMI em relação às principais variáveis sociodemográficas e clínicas da criança e da mãe, na Macrorregião Leste de Minas Gerais, de 2008 a 2019. Materiais e Métodos: Foram utilizados dados secundários de domínios público dos Sistemas de Informação de Mortalidade e de Nascidos Vivos da Secretaria Estadual de Saúde de Minas Gerais na construção de um estudo ecológico de series temporais e abordagem descritiva observacional de caráter transversal. Resultados: Foram registrados 1.517 óbitos e TMI acumulada de 14,3/1000 nascidos vivos (NV). Na análise ano a ano não foi possível observar um padrão temporal conforme observado no âmbito nacional e estadual, sendo que a curva da evolução temporal da TMI mostrou-se anárquica. O componente neonatal precoce registrou as maiores taxas. Na análise dos dados maternos, destacou-se por estarem atreladas às taxas mais elevadas, a baixa escolaridade (123,5 e 21,5/1000 NV), a idade reprodutiva em extremos (27,5 e 52,6/1000 NV) e o parto vaginal (15,7/1000 NV). Já em relação à criança, o sexo masculino (15,6/1000 NV), a raça/cor branca (20,3/1000 NV) e nascimentos ocorridos em domicílio (328,24/1000 NV) apresentaram maior TMI. Observou-se um alto percentual de mortes evitáveis (68%) destacando aquelas que são evitáveis por adequada atenção a mulher na gestação como a mais pronunciada. Conclusões: Com base nesses achados, nota-se a necessidade da gestão integrada nas redes de atenção à saúde materno-infantil ampliando assim a capacidade dos municípios de assistir com qualidade a gestação, o parto e o nascimento, a fim de atuar na redução da mortalidade infantil.
