Primary healthcare has made little use of information systems to assess the population's health situation due to the difficulty in understanding the reports. Generic definitions of actions are common, based on empirical observations. The current study aimed to evaluate whether the introduction of georeferenced indicators can serve to better identify individuals' health situation, which would help planning actions by health teams. Healthcare workers from eight teams answered a questionnaire at three different moments: the first, before reading the information system's reports; the second after reading; and the third after using georeferencing. The results showed a significant difference in the classification of the health situation using georeferencing when compared to the previous moments (p < 0.05). Georeferencing facilitated analysis of the health situation, fostering better monitoring of work processes. Finally, use of the data points to rationalization of actions and possible upgrading of healthcare. The study suggests the use of georeferencing in the work agenda to become an effective tool for orienting actions.
O objetivo do estudo é explorar a associação entre componentes do Piso de Atenção Básica (PAB) fixo e variável, fatores sociodemográficos e perfil epidemiológico com as despesas municipais em atenção primária à saúde no Rio Grande do Sul, Brasil. Foi realizado estudo ecológico com 496 municípios do Rio Grande do Sul. A variável despesa média municipal dos anos de 2011 a 2013 do bloco financeiro da atenção primária à saúde que representou as despesas efetivas com o repasse de recursos federais foi extraída do Relatório Gerencial da Sala de Apoio à Gestão Estratégica. Utilizou-se modelo de regressão linear múltipla. Para fins de ajuste do modelo, as variáveis foram agrupadas em cinco blocos de acordo com o objetivo do estudo. A despesa média com atenção primária à saúde foi de R$ 81,20 (DP ± 35,50) por habitante-ano. O bloco de variáveis que compõem o PAB fixo explicou 39% (R2 = 0,39) da variabilidade de despesas entre municípios, enquanto que, no bloco do PAB variável, o R2 foi 0,82, no bloco sociodemográficas, o R2 foi 0,26, no bloco de estrutura-desempenho, o R2 foi 0,46, e, no bloco de perfil epidemiológico, o R2 foi 0,15. No modelo final, a variável que esteve associada a maiores valores estimados de gasto com atenção primária à saúde foi a taxa de equipes de saúde da família. Municípios com o número de equipes entre 135 e 41 por 100 mil habitantes-ano possuem um gasto de R$ 51,00 per capita a mais do que municípios com o número de equipes entre 0 e 8. Despesa em atenção primária à saúde parece estar mais atrelada às políticas federais de indução do que a fatores associados com a demanda em saúde, como o perfil demográfico e epidemiológico dos municípios do Rio Grande do Sul.
Application of single transient forebrain ischemia (ISC) in adult Wistar rats, lasting 2 or 10 min, caused inhibition of Na+,K+-ATPase activity in cytoplasmic membrane fractions of hippocampus and cerebral cortex immediately after the event. In the 2-min ISC group followed by 60 min of reperfusion, the enzyme inhibition was maintained in the cortex, while there was an increase in hippocampal enzyme activity; both effects were over 1 day after the event. However, in the 10-min ISC group enzyme inhibition had been maintained for 7 days in both cerebral structures. Interestingly, ischemic preconditioning (2-min plus 10-min ISC, with a 24-hour interval in between) prevented the inhibitory effect of ischemia/reperfusion on Na+,K+-ATPase activity observed either after a single insult of 2 min or 10 min ischemia. We suggest that the maintenance of Na+,K+-ATPase activity afforded by preconditioning be related to cellular neuroprotection.
Resumo A Atenção Primária à Saúde (APS) é o capaz de atingir ampla cobertura e resolutividade. Esta pesquisa avaliou os fatores associados a priorização que os gestores municipais da saúde do estado do Rio Grande do Sul (RS) dada à APS. Realizou-se estudo analítico seccional entre agosto de 2017 a maio de 2019 através da aplicação de um questionário via FormSUS aos 497 secretários municipais de saúde do RS, composto por 44 perguntas divididas em 4 blocos. Efetuou-se regressão logística múltipla para analisar fatores relacionados a formação com a priorização da APS. De 404 participantes, 66.8% relataram um grau máximo de priorização. Gestores com pós-graduação apresentaram maior a priorização do que aqueles com ensino fundamental (78,5% vs 58,8%, p<0.01). Em relação à área de pós-graduação, gestores com formação em saúde coletiva priorizaram mais a APS do que em outras áreas, ou nenhuma pós-graduação, respectivamente com 84%, 77,1% e 61,8% (p<0.01). Após ajuste por covariadas, ter pós-graduação em saúde coletiva ou em outra área permaneceram associadas a maiores chances de priorização. Os resultados podem fomentar iniciativas que considerem o nível e a área de escolaridade para esse cargo, com enfoque no critério técnico e não estritamente político.
