Resumo A pandemia causada pelo novo coronavírus (SARS-CoV-2) configura quadro de emergência de saúde pública mundial. Algumas categorias ocupacionais têm risco elevado de exposição à infecção, como os(as) trabalhadores(as) da saúde. Neste artigo, objetiva-se sumarizar e sistematizar aspectos relativos às condições de trabalho e de saúde dos(as) trabalhadores(as) da saúde nessa pandemia, enfatizando a situação no Brasil, experiências exitosas na proteção do trabalho em saúde em outros países e recomendações para o contexto brasileiro. Iniciativas imediatas de proteção e combate à pandemia em outros países incluíram como pontos estratégicos: adequação das condições de trabalho; testagem sistemática e ações específicas de assistência aos(às) trabalhadores(as). Para o enfrentamento da Covid-19 no Brasil, destacam-se como recomendações: revisão de fluxos de atendimento e definição de características e condições para cada etapa de atendimento; estabelecimento da Covid-19 como doença relacionada ao trabalho para os grupos expostos; registro efetivo da ‘ocupação’ nos sistemas de informação; estabelecimento de condições especiais para execução do trabalho na situação de epidemia; atenção às jornadas laborais e ações para redução de estressores ocupacionais. A atuação desses(as) trabalhadores(as) é elemento central no enfrentamento da pandemia, portanto, o plano de combate ao Covid-19 deve incluir proteção e preservação de sua saúde física e mental.
Bombesin/gastrin-releasing peptides (BN/GRP) were shown to bind selectively to cell surface receptors, stimulating the growth of various types of malignancies in murine and human models. The novel BN/GRP synthetic receptor antagonist, RC-3095, was able to produce long-lasting tumor regressions in murine and human tumor models in vitro and in vivo. Animal toxicology studies showed no detectable organ toxicity apart from local irritation at the injection site. The purpose of this study was to determine the safety and feasibility of the administration of RC-3095 by daily subcutaneous injections in patients with advanced and refractory solid malignancies. Twenty-five patients received RC-3095 once or twice-daily at doses ranging from 8 to 96 ug/kg. Dose was escalated in groups of 3-5 patients per dose level. The only toxicity observed was local discomfort in the injection site at the highest doses. A single dose administration of RC-3095 at the highest dose level (96 ug/kg) was tested in a clearly hypergastrinemic individual with the Zollingen-Ellison syndrome and produced a decrease in plasma gastrin down to 50% of basal levels in 6 h. There was no objective tumor responses in patients included in the study. A short-lasting minor tumor response was observed in a patient with a GRP-expressing progressive medullary carcinoma of the thyroid. Due to problems with the analytical method, plasma pharmacokinetic data was obtained only from two patients included at the highest dose level. In these patients, RC-3095 reached plasma concentrations >100 ng/mL for about 8 h, which were within therapeutic levels on the basis of prior data obtained in mice and rats. The plasma elimination half-life was between 8.6-10.9 h. Due to the occurrence of local toxicity at the injection site, the dose escalation procedure could not be fully evaluated up to a maximum tolerated dose. Thus, a recommended dose of RC-3095 for Phase II trials could not be clearly established. Considering the novelty of its mechanism of action and impressive preclinical anti-tumor activity, further studies exploiting new formulations of RC-3095 for human use, such as slow-release preparations, and analogues with a more favorable pharmacokinetics are warranted.
NK1R antagonists increased CINV control in the acute, delayed, and overall phases. They are effective for both moderately and highly emetogenic chemotherapy regimens. Their use might be associated with increased infection rates; however, additional appraisal of specific data from RCTs is needed.
ResumoObjetivoAvaliar a prevalência de vacinação para hepatite B e os fatores associados entre trabalhadores da atenção primária e da média complexidade do setor saúde.MetodologiaEstudo transversal com 506 trabalhadores do setor saúde de um município do Recôncavo Baiano. Utilizou-se questionário para investigar situação vacinal para hepatite B. Análises uni e bivariada foram utilizadas para avaliação dos fatores associados à vacinação.ResultadosHouve predomínio do sexo feminino (78,3%) e do vínculo de trabalho permanente (63,8%); além disso, 78,1% dos trabalhadores fizeram uma avaliação positiva da saúde. A maioria referiu ter recebido três doses da vacina contra hepatite B (59,9%). Enfermeiras, técnicas e médicos estiveram mais imunizados (91,8%). Os fatores associados à vacinação foram: sexo, idade, escolaridade, uso de equipamento de proteção individual e contato com material biológico.DiscussãoA prevalência da vacinação completa contra hepatite B foi de 59,9%, indicando uma cobertura baixa. É preciso estimular os profissionais que não desenvolveram um nível de anticorpos adequado após o esquema vacinal primário a refazer uma quarta dose.ConsideraçõesIndivíduos que não estão imunes à doença devem ser informados que são suscetíveis ao VHB até que tenham comprovação imunológica. Questões sobre biossegurança nos currículos acadêmicos colaboraram para incorporação de atitudes mais positivas frente à adesão à vacinação.

Sistematizou-se a origem das fontes de suprimento sanguíneo para o en-A systematic study of the brain base arteries in the rabbit (Oryctolagus cuniculus) The brains of 30 New Zealand rabbits (Oryctolagus cuniculus) were injected with red stained latex. The arteries of the ventral surface of the brain were systematized on the right (R) and on the left (L) side with the respective percentage of appearance: the aortic arch emitted the braquicephalic trunk and the left subclavian artery (83.3%); or the braquicephalic trunk, the left common carotid artery and the left subclavian artery (16.7%). The braquicephalic trunk emitted the right and the left common carotid arteries and the right subclavian artery (83.3%); or the right common carotid artery and the right subclavian artery (16.7%). The common carotid arteries were divided into external and internal carotid arteries (96.7% on the R, 100% on the L.). The internal carotid artery to the R was present (96.7%) and absent (3.3%), and to the L, was present (100%). The rostral choroidal artery to the R was collateral branch of the rostral branch of the internal carotid artery (83.3%), collateral branch of caudal branch of the internal carotid artery (16.7%), and to the L was collateral branch of the rostral branch of the internal carotid artery (93.3%), collateral branch of the caudal branch of the internal carotid artery (6.7%). The middle cerebral artery to the R and to the L was single (80%) and double (20%). The rostral cerebral artery to the R had middle caliber (90%), thin caliber (6.7%) and too thin caliber (3.3%), and to the L had middle caliber (76.7%), thin caliber (16.7%) and too thin caliber (6.7%). The internal ethmoidal artery was absent (73.3%), present and single (26.7%). The caudal cerebral artery to the R was single (66.7%), double (26.7%) and triple (6.7%), and to the L was single (63.3%) and double (36.7%). The terminal branches of the right and left vertebral arteries were present (100%, and formed the basilar artery (100%). The ventral spinal artery was present (100%). The caudal cerebellar artery, to the R was single (43.3%), single with labyrinthic artery isolated (26.7%) and double (30%), and to the L was single (50%), single with labyrinthic artery isolated (6.7%), double (40%) and triple (3.3%). COVID-19 vaccine hesitancy (VH) has caused concerns due to the possible fluctuations that may occur directly impacting the control of the pandemic. In this study, we aimed to estimate the prevalence and factors associated with COVID-19 VH in Portuguese-speaking countries. We developed a web survey (N:6,843) using an online, structured, and validated questionnaire. We used Measurement Models, Exploratory Factor Analysis, Exploratory Structural Equation Models, and Confirmatory Factor Analysis for the data analysis. The overall prevalence of COVID-19 VH in Portuguese-speaking countries was 21.1%. showed a statistically significant direct effect for VH: vaccine-related conspiracy beliefs (VB) (β = 0.886), perceived stress (PS) (β = 0.313), COVID-19 Misinformation (MIS) (β = 0.259) and individual responses to COVID-19 (CIR) (β = −0.122). The effect of MIS and CIR for VH was greater among men and of PS and VB among women; the effect of PS was greater among the youngest and of VB and CIR among the oldest. No discrepant differences were identified in the analyzed education strata. In conclusion, we found that conspiracy beliefs related to the vaccine strongly influence the decision to hesitate (not to take or to delay the vaccine). Specific characteristics related to gender, age group, social and cognitive vulnerabilities, added to the knowledge acquired, poorly substantiated and/or misrepresented about the COVID-19 vaccine, need to be considered in the planning of vaccination campaigns. It is necessary to respond in a timely, fast, and accurate manner to the challenges posed by vaccine hesitancy.
