A pneumonia (PNM) é uma das doenças infectocontagiosas com a quartamaior taxa de morbimortalidade do país, sendo esta taxa ainda maior em pacientesimunodeprimidos, crianças e idosos. Caso ocorra no ambiente hospitalar, entre 48h da admissão e até 72h de alta hospitalar é chamada Pneumonia Hospitalar (PH), caso ocorra fora do ambiente hospitalar ou nas primeiras 48h de internação é chamada Pneumonia Adquirida na Comunidade (PAC). Entre janeiro/2018 e janeiro/2019, essa infecção pulmonar representou nacionalmente cerca de 53,11% das internações e 60,7% dos óbitos referentes a patologias do sistema respiratório. As PNMs são consideradas grandes desafios no controle de dados referentes à morbimortalidade e, principalmente, nos dispendiosos valores gastos. Isso ocorre devido certa escassez de dados e estudos epidemiológicos com informações conclusivas sobre a incidência, mortalidade e possíveis fatores relacionados ao prognóstico dessa patologia. Objetivo:investigar o perfil epidemiológico, métodos de diagnóstico, fatores de gravidade,morbimortalidade e tratamento, à partir da análise de prontuários, dos pacientes comdiagnóstico de Pneumonia Nosocomial e Comunitária no Box de Emergência do HospitalRegional da Asa Norte (HRAN). Metodologia: É um estudo retrospectivo e prospectivatransversal, descritiva, com revisão de prontuários de pacientes internados no Box deEmergência do Hospital Regional da Asa Norte (HRAN) da Secretaria de Estado de Saúde doDistrito Federal, no período de janeiro/2018 a setembro/2019. Resultados: No período dejaneiro/2018 a setembro/2019, foram analisados 592 prontuários e incluídos nesta análise 80pacientes, segundo critérios de participação. 55% eram do sexo feminino, com média de idadefoi de 70,48 ± 16,64. Quanto ao tipo de pneumonia, 54,5% pacientes possuíam o acometimentocomunitário enquanto 47,5% o nosocomial. Em relação a comorbidades, foi visto que 91,25%dos pacientes possuíam comorbidades, sendo que cerca de 71,2% apresentavam pelo menosduas delas. As principais patologias listadas foram hipertensão arterial sistêmica, diabetesmellitus tipo 2, doença pulmonar obstrutiva crônica e insuficiência cardíaca. O escore CURB-65 da admissão, para PAC, mostrou que 61,90% (N=26) atingiram score maior ou igual a três,que indica mortalidade alta. Para o acometimento comunitário, os antibióticos mais utilizadosforam Meropenem, Piperacilina com Tazosulbactam, Cefepime e Ampicilina com Sulbactam,.No acometi enquanto no hospitalar, foram Meropenem, Ampicilina com Sulbactam,Vancomicina e Cefepime. A mortalidade geral do estudo foi de 86,25%, sendo especificamentepara PAC de 85,61% e 86,84% para PH. Considerações finais: A pneumonia bacteriana, emsua forma comunitária e hospitalar, é considerado um acometimento relevante no Box deEmergência, visto sua incidência e sua alta taxa de mortalidade. É importante ressaltar queidade avançada, a presença de comorbidades e associação ao hábito do tabagismo sãoconsiderados fatores de gravidade e que estão relacionados com a alta taxa de mortalidade. APH determinou-se como uma complicação grave em pacientes da instituição, levando ao maiorperíodo de internação e maior demanda por suporte invasivo e maior número de antibióticos. Oscore CUBR-65 comportou-se como um preditor importante de gravidade e de mortalidade. Oalto número de amostras de cultura sem resultados mostra-se como uma limitação importantepara a constituição do perfil etiológico da instituição.
A infecção pelo SARS-CoV-2 apresenta elevada prevalência devido ao rápido modo de transmissibilidade. O quadro clínico varia de leve, moderado e grave, sendo que os mais graves evoluem com Síndrome Respiratória Aguda Grave (SRAG) e necessidade de internação em unidade de terapia intensiva. Diversas alterações em exames de imagem, laboratoriais e sinais e sintomas dos pacientes na admissão, podem ser fortes indicativos de evolução grave da doença. Este trabalho objetivou Investigar o manejo clínico e terapêutico em pacientes graves acometidos por COVID-19 em um hospital público do Distrito Federal. Trata-se de um estudo transversal, retrospectivo, comparativo com revisão de prontuários de pacientes internados no Box de Emergência e na UTI do Hospital Regional da Asa Norte (HRAN) da Secretaria de Estado de Saúde do Distrito Federal, no período de março/2020 a dezembro/2020. Foram incluídos pacientes ≥ 18 anos portadores de COVID-19, comprovados por exame positivo de reação em cadeia de polimerase via transcriptase reversa (RT-PCR) de amostras de swab nasais e faríngeos, de ambos os sexos. Foram investigadas as variáveis sexo, idade, teste RT-PCR confirmatório para COVID-19, tempo de internação, desfecho clínico, comorbidades, exames de imagem de tomografia computadorizada (TC) de tórax, exames laboratoriais e principais medicamentos utilizados. A amostra foi composta por 143 pacientes estratificados em alta (39/27,3%) e óbitos (104/72,7%). O estudo revelou uma média etária acima dos 50 anos e prevalência do sexo masculino em ambos os grupos. As principais comorbidades encontrados foram: HAS, DM, DPOC e doenças cardiovasculares de forma estatisticamente significante no grupo de óbitos e obesidade no grupo de alta. Exames de TC de tórax revelaram predominância de vidro fosco, abrangência bilateral e comprometimento 25% nos dois grupos analisados (alta e óbito). Em relação aos tratamentos medicamentosos, o uso de antimicrobianos foi empregado para todos os pacientes da amostra, sendo os principais a azitromicina, a piperacilina/tazobactam, a ceftriaxona e o meropenem para ambos os grupos. As principais associações antimicrobianas utilizadas nos grupos de pacientes que evoluíram para alta e óbito foram: azitromicina + ceftriaxona, amicacina + meropenem, azitromicina + piperacilina/tazobactam. Outros medicamentos empregados na amostra do estudo incluiu: cloroquina/ hidroxicloroquina (8,39%), corticosteroides (86,7%) e anticoagulantes (97,2%). Os exames laboratoriais mais alterados foram: leucócitos; bastões, neutrófilos, linfócitos, proteína C reativa, TGO (AST), TGP (ALT), LDH, creatinina, ferritina, gama GT e fosfatase alcalina. A COVID-19 é uma doença nova e potencialmente fatal, com pior prognóstico para pacientes graves, sobretudo internados na UTI. Este trabalho permitiu conhecer os comprometimentos pulmonares mais frequentes, alterações mais importantes nos exames laboratoriais, os tratamentos instituídos e o perfil dos pacientes graves acometidos por COVID-19 em um hopital público referência no tratamento. Esses achados almejam colaborar com as equipes de assistência em saúde e comunidade científica no manejo do paciente grave acometido por COVID-19.
