ResumoA Atenção Domiciliar (AD) é uma tecnologia utilizada pelas equipes de Saúde da Família (eSF) para ampliar o cuidado a saúde. Trata-se de um processo complexo e que ocorre sob critérios ainda não sistematizados, em especial quanto ao nível individual. Objetiva-se sistematizar as ações da eSF a partir da complexidade de caso através de uma ferramenta de avaliação para a AD ao adulto e em especial ao idoso. Esta ferramenta deriva de protocolos sistematizados de avaliação do idoso e critérios de inclusão para serviços de AD de maior complexidade, resultando em uma escala aplicável para o nível da Atenção Primária a Saúde (APS), pela avaliação da autonomia e dependência, do risco cardiovascular, ulcerações, estado nutricional, humor e da funcionalidade familiar. A partir desta avaliação inicial, são definidos três níveis de cuidado, integrando as eSF à rede de cuidados em AD. Os parâmetros estabelecidos são discutidos a luz da casuística e da realidade da APS, apresentando-se como opção para a priorização de visitas domiciliares das eSF em diferentes contextos. AbstractThe Home Care (HC) is a technology used by the Family Health Teams (FHT) to expand health care. This is a complex process that occurs under criteria not systematized yet, in particular regarding the individual level. This article aims to systematize the actions of FHT from the complexity of the case through an assessment tool for HC to adults and elderly in particular. This tool derives from systematized protocols for evaluating the elderly and inclusion criteria for AD services of greater complexity, resulting in a scale applicable to the Primary Health Care (PHC) level, through the assessment of autonomy and dependence, cardiovascular risk, ulcers, nutritional status, mood and family functioning. From this initial assessment are defined three levels of care, integrating the FHT care network in HC. The parameters set here are discussed in light of the case reports and the reality of PHC, presenting itself as an option to prioritize home visits of the FHT in different contexts.Descritores: assitência domiciliar. visita domiciliar. medição de risco
