To assess the impact of the endotracheal tube (ETT) and of different flow waveforms on estimates of alveolar cyclic recruitment (CR) and overdistension (AO). Numerical simulation of the respiratory system plus ETT (inertance L plus a flow-dependent resistance, K (1) and K (2)), with the following non-linear equation of motion PAW(t)= ((K1 + K2 x/V(t)/) x V(t) + L x V(t)) + Rrs x V(t) + (E1 + E2 x V(t) x V(t) + P0 (PAW pressure at the airways opening, V volume), under volume-controlled mechanical ventilation. An index %E2 = 100 x (E2 x V(T))/(E1 + E2)x V(T)) can be calculated where %E(2) > 30% represents AO and %E(2) < 0% represents CR. Parameters were estimated by the least-squares method, either with the complete equation or suppressing L, K(2) or both. %E(2) is always underestimated (down to -152 percent points) with incomplete equations of motion. The estimation of %E (2) may be strongly biased in the presence of an ETT excluded from the estimation model.
A covid-19, doença causada pelo novo coronavírus, apresenta grande preocupação em razão do elevado padrão de letalidade, mortalidade e transmissibilidade, e um desafio aos profissionais de saúde por apresentar quadros gripais que podem evoluir rapidamente para pneumonia severa ou síndrome respiratória aguda grave (Sars-CoV-2). O câncer é uma doença com fatores de riscos muito semelhantes às doenças cardiorrespiratórias e metabólicas que, associadas aos efeitos colaterais do tratamento antineoplásico ou progressão tumoral, tornam esses pacientes suscetíveis a complicações mais graves de infecção por Covid-19. Além disso, complicações comuns aos pacientes oncológicos podem ser agravadas pelos mecanismos fisiopatológicos envolvidos na Covid-19, como: comprometimento respiratório gerado por produção de citocinas inflamatórias e os pneumoconiose alveolares que aceleram a evolução para quadro de pneumonia; aumento na produção de D-dimero e redução de fibrinogênio, fazendo com que o fator trombolítico seja maior, justificando eventos por tromboembolismo; uma possível causa hemolítica em que a hemoglobina perde o Ferro e diminui sua capacidade de carrear oxigênio, sendo causa da dessaturação de oxigênio e hipoxemia refratária. A fisioterapia tem como principal linha de atuação a adequação do suporte ventilatório, nos pacientes mais graves. Com enfoque no paciente oncológico, deve-se basear nos sintomas e limitações clínicas para a adequação das atividades, favorecer a profilaxia mecânica para redução do risco de trombose venosa profunda, avaliar progressão lenta e monitorização dos sinais vitais, minimizar as perdas e melhorar o status funcional do paciente para tolerância ao tratamento.
