Ante el diagnóstico histológico se solicita un estudio de extensión por lo que la paciente es remitida al servicio de oncología médica. En la tomografía computarizada (TC) tóraco-abdominal se objetiva una gran masa mamaria izquierda de 13 × 8 cm con otras lesiones nodulares de menor tamaño en ambas mamas, múltiples densidades nodulares pulmonares y una lesión vascular esplénica de 2,6 cm sugestivas de afectación metastásica masiva, y densidades nodulares subcutáneas en pared torácica izquierda de hasta 2 cm de diámetro sugestivas de letálides. Con el diagnóstico de angiosarcoma de mama con metástasis múltiples pulmonares, esplénicas y cutáneas, la paciente es tratada de forma paliativa con quimioterapia. A los 6 meses del inicio del tratamiento la paciente fallece por hemorragia digestiva alta secundaria a hemorragia pulmonar.Véase contenido relacionado en DOI: http://dx.
DiscusiónLos angiosarcomas son neoplasias malignas que afectan a los tejidos blandos, concretamente el endotelio vascular. Son poco frecuentes, con un comportamiento agresivo a nivel local, altas tasas de metástasis a distancia y asociados a un mal pronóstico 1,2 . Se ha señalado una mayor prevalencia en varones de raza blanca, sobre todo en pacientes mayores de 60 años, aunque existen casos descritos en pacientes con edad pediátrica y juvenil 2,3 .La patogénesis sigue siendo desconocida, considerándose como posibles agentes causales la radioterapia 4 , la exposición solar y las mutaciones genéticas, siendo este el de mayor importancia en la edad infantil 2 .
