The study results suggest that the majority of congenital CMV infection in Turkey occurs following nonprimary maternal infection. We believe that congenital CMV infection and its long-term effects have been underestimated in our country, as infected infants are usually asymptomatic at birth.
There is a need for extended studies with a larger number of patients from specific groups to better understand the reactivation frequency and identify risk factors. EBV and CMV reactivation should be taken into consideration in critically ill patients with fever, without specific symptoms and unresponsive to the treatment.
Purpose
:
To evaluate the prognostic value of the vertebral bone mineral density (BMD) on chest computed tomography (CT) in COVID-19 patients.
Methods
:
The chest CT of hospitalized patients with COVID-19 pneumonia were evaluated for Pneumonia Severity Score (PSS) as the ratio of the volume of involved lung parenchyma to the total lung volume. In addition, BMD was manually measured from the vertebral corpus using axial CT images. The relationships of clinical variables, PSS and vertebral BMD with patient outcomes, namely mortality, intensive care unit (ICU) admission and mechanical ventilation were investigated. Lower BMD was defined as ≤100 HU.
Results
:
The study included 209 patients (118 males, 56.4%). As a result of the univariate analysis, the rates of mortality, ICU admission and mechanical ventilation were 17.2% (n=36), 24.8% (n=52), and 20.6% (n=43), respectively, and they were significantly higher among the patients with lower BMD (38.1 vs. 13.0%,
p
<0.001; 33.4 vs. 21.2%,
p
=0.002; and 38.1 vs. 8.2%,
p
<0.001, respectively). In the mortality group, PSS was significantly higher (median, 9 vs. 5;
p
<0.001) and vertebral BMD was significantly lower (median, 83 vs. 139;
p
<0.001). Severe clinical incidence was significantly higher in patients with lower BMD compared to those with higher BMD (39.7 vs 24.7% and
p
=0.028). There was a significant correlation between clinical classification and lower BMD (r=0.152 and
p
=0.028). The multivariate analysis revealed vertebral BMD [odds ratio (OR), 1.028; 95% CI, 1.011-1.045,
p
=0.001) and lower BMD (OR, 4.682; 95% CI, 1.784-12.287,
p
=0.002) as significant independent predictors of mortality.
Conclusion
:
Vertebral BMD is a strong independent predictor of mortality that is reproducible and can be easily evaluated on the chest CT images of COVID-19 patients.
According to our results, a maternal screening-based approach would be useful for only a very small group that is at risk of primary infection. Considering the cost of the scan, a routine maternal-based screening program is unadvisable in developing societies, but it is necessary for studies of different cohort groups and infectious diseases.
ÖZETMoleküler tanı yöntemlerindeki gelişmelerin bir sonucu olarak, 1980'li yılların başlarında servikal kanser (SK) biyopsileri ve SK hücre kültürlerinin insan papillomavirus (HPV) DNA dizilerini içerdiği göste-rilmiştir. Sonraki yıllarda, HPV E6 ve E7 genlerinin SK dokularında eksprese edildiği ve bu onkoproteinlerin pRb ve p53 başta olmak üzere çeşitli hücresel proteinlerle etkileşim içinde olduğu keşfedilmiştir. Serviks kanseri ile HPV enfeksiyonları arasındaki ilişkinin gösterilmesi bu konuya olan ilginin artmasına neden olmuş ve yapılan geniş ölçekli epidemiyolojik çalışmalarda bazı HPV tiplerinin SK için majör risk faktörü olduğu belirlenmiştir. Yüksek riskli veya onkojenik tipler olarak adlandırılan bu tiplerin diğer anogenital kanserler ve baş-boyun kanserlerinin bir alt grubu ile de ilişkili olduğu belirlenmiştir. Bu süreçte her biri farklı yaklaşım ve uygulamaları içeren çok sayıda ticari ve kullanıcı tasarımlı (in-house) tanı yöntemi geliş-tirilmiş ve HPV-DNA tespitine dayalı testler SK tarama programlarının önemli bir parçası haline gelmiştir. HPV enfeksiyonlarının tanı ve takibinde (yönetiminde) kullanılan tarama-tiplendirme yöntemlerinin ve kombine yaklaşımların her birinin kendilerine özgü güçlü ve zayıf yönleri mevcuttur. Yöntem seçimini kritik ve karmaşık hale getiren, gerek metodoloji gerekse virusun doğasından kaynaklanan çeşitli zorluklar bulunmaktadır. Çok sayıda farklı HPV tipinin genital bölgede enfeksiyon etkeni olarak saptanabilmesi, viral genomun aynı HPV tipi içinde bile çok sayıda dizi varyasyonları gösterebilmesi, mevcut yöntemlerin saptama duyarlılığının HPV tiplerine ve incelenen örnekte bulunan viral DNA kopya sayılarına bağlı olarak değişebilmesi ve son olarak çoklu enfeksiyonları saptama ve tip tayini yapabilme yeteneğinin yöntemler arasında farklılıklar göstermesi bu zorluklardan bazılarıdır. Bu derleme yazıda, HPV enfeksiyonlarının tanı ve takibi için kullanılan yöntemlerin özetlenmesi ve yöntem seçiminde yol gösterebileceği düşünülen güncel bilgilerin paylaşılması hedefl enmiştir. Bu amaçla, geçmiş yıllarda kullanılan moleküler temelli HPV testleri ve bu testlerin gelişim sürecine kısaca değinilmiş, günümüzde kabul gören ve yaygın olarak kullanılan tanı, tarama ve tiplendirme yöntemlerinin avantaj ve dezavantajları ayrıntılı olarak tartışılmıştır.
