A small proportion of children with Gartland type III supracondylar humeral fracture (SCHF) experience troubling limited or delayed recovery after operative treatment. We hypothesised that the fracture level relative to the isthmus of the humerus would affect the outcome. We retrospectively reviewed 230 children who underwent closed reduction and percutaneous pinning (CRPP) for their Gartland type III SCHFs between March 2003 and December 2012. There were 144 boys and 86 girls, with the mean age of six years (1.1 to 15.2). The clinico-radiological characteristics and surgical outcomes (recovery of the elbow range of movement, post-operative angulation, and the final Flynn grade) were recorded. Multivariate analysis was employed to identify prognostic factors that influenced outcome, including fracture level. Multivariate analysis revealed that a fracture below the humeral isthmus was significantly associated with poor prognosis in terms of the range of elbow movement (p < 0.001), angulation (p = 0.001) and Flynn grade (p = 0.003). Age over ten years was also a poor prognostic factor for recovery of the range of elbow movement (p = 0.027). This is the first study demonstrating a subclassification system of Gartland III fractures with prognostic significance. This will guide surgeons in peri-operative planning and counselling as well as directing future research aimed at improving outcomes.

Latar belakang: Teknik radiasi ekstrakorporeal merupakan alternatif tindakan pembedahan penyelamatan ekstremitas pada kasus osteosarkoma khususnya di pusat layanan dengan keterbatasan endoprostesis dan alograf. Penelitian ini bertujuan untuk mengevaluasi hasil tindakan pembedahan penyelamatan ekstremitas dengan otograf yang diradiasi secara ekstrakorporeal pada pasien-pasien osteosarkoma yang datang ke RSCM. Metode: Studi ini menggunakan desain kohort retrospektif yang dilakukan pada 20 pasien osteosarkoma stadium IIB yang diterapi dengan pembedahan penyelamatan ekstremitas dengan otograf yang diradiasi secara ektrakorporeal selama periode 1995-2008. Dilakukan evaluasi terhadap angka kesintasan, rekurensi lokal, metastasis, komplikasi, lamanya unifi kasi, dan skor fungsional menurut sistem skor Musculoskeletal Tumor Society Scoring System (MSTS). Metode Kaplan-Meier digunakan untuk mendeskripsikan kesintasan, angka rekurensi lokal, kesintasan bebas tumor, dan kesintasan bebas metastasis. Hubungan variabel-variabel seperti usia, jenis kelamin, lokasi tumor, ukuran tumor, tipe osteosarkoma, kadar alkali fosfatase serum, jenis biopsi, dan klasifi kasi Huvos dianalisis dengan uji log rank. Uji chi-square digunakan untuk menganalisis hubungan antara skor MSTS dan karakteristik pasien, angka rekurensi, metastasis, serta komplikasi.

Introduction Patients with open fracture Gustillo-Anderson grade 3 had undergone several surgical procedures, but still ended up with expose long dead bone or infected. Illizarov method was used to address long bone and soft tissue defect after re-debridement with radical resection of long dead bone or infected segment. Methods We included 14 patients (mean age: 30.86 ± 11.49) with non-union tibial fracture with long dead and infected bone segment who had undergone several debridement, bone grafting or spacer and soft tissue closure procedure due to open fracture of tibia grade 3. The subjects underwent re-debridement with radical resection of dead or infected bone segment followed by Illizarov method to perform bone transport procedure for bone defect filling and simultaneously restore severe soft tissue loss and bone lengthening procedure. Results All subjects had achieved satisfactory results with mean docking period of bone transport 3.78 ± 0.54 months, union time at the docking side 7 (5.5–9) months. Soft tissue was covered and no recurrence of infection. Three subjects had Leg Length Discrepancy (LLD) of 1 cm, whereas the remaining had zero discrepancy. No significant pain was observed at final follow-up and 4 patients had ankle joint stiffness. Conclusion The Illizarov method can effectively address long bone and soft tissue defects by distraction osteogenesis through bone transport procedure that filling the defect gradually without bone graft and simultaneously enhancing soft tissue closure without tertiary soft tissue procedure subsequently followed with bone lengthening procedure to correct the limb length discrepancy.
